Resumen Los pentatómidos (Hemiptera: Pentatomidae) son un grupo de unas 5.000 especies distribuidas por todo el mundo, muchas de ellas fitófagas con implicaciones económicas como plagas de cultivos. El arroz (Oryza sativa L.) es uno de los productos agrícolas más relevantes para el consumo humano. En los arro zales neotropicales, los hemípteros son las principales plagas, siendo los pentatómidos los que más afectan a la productividad del cultivo, destacando el género Tibraca Stål en cuanto al impacto económico. Además, los cultivos de arroz pueden representar importantes sitios de alimentación y apareamiento para otras especies de chinches taxonómicamente relacionadas con Tibraca, que podrían desempeñar el papel de potenciales plagas, lo que hace necesario el estudio de los pentatómidos asociados a este cultivo en el Neotrópico. Este trabajo tuvo como objetivo reportar la presencia de Glyphepomis adroguensis Berg, Hypatropis inermis (Stål) y Paratibraca spinosa (Campos & Grazia) asociadas al arroz en las principales zonas productoras de Argentina. Se identificó el material recolec tado durante 2017-2018 en arrozales comerciales del Noreste argentino, principal región productora. Asimismo, se consideró la variedad de arroz sobre la que se recolectaron los especímenes, el estado del cultivo (temporada de cultivo - postcosecha) y la fenología del cultivo. Como resultado, se reporta la asociación de las especies mencionadas con el arroz en las provincias de Chaco y Corrientes, Argentina. Además, se reporta por primera vez en el país el género Paratibraca Campos & Grazia y la especie P. spinosa.

Abstract Stink bugs (Hemiptera: Pentatomidae) are a group of about 5,000 species distributed worldwide, many of them phytophagous with economic implications as crop pests. Rice (Oryza sativa L.) is one of the leading ag ricultural products for human consumption. In neotropical rice fields, hemipterans are the primary pests, with stink bugs being the worst affecting crop productivity, standing out the genus Tibraca Stål in terms of economic damage. In addition, rice crops may represent important feeding and mating sites for other stink bug species taxonomically related to Tibraca, which could play the role of potential pests, making it necessary to study the pentatomids associated with this crop in the Neotropics. This work aimed to report the presence of Glyphepomis adroguensis Berg, Hypatropis inermis (Stål) and Paratibraca spinosa (Campos & Grazia) associated with rice in the main rice-growing areas of Argentina. Material collected during 2017-2018 from commercial fields in north eastern Argentina, the central rice-producing region, was identified. The rice variety on which the specimens were collected, crop status (growing season - post-harvest) and crop phenology were considered. As a result, the association of the mentioned species with rice in the provinces of Chaco and Corrientes, Argentina, is reported. Moreover, the genus Paratibraca Campos & Grazia and the species P. spinosa are reported for the first time in the country.

Abstract Introduction: malnutrition and sarcopenia are frequent in the population with liver cirrhosis and have a negative impact on the performance status and life expectancy of these patients. There are multiple assessment tools for malnutrition and sarcopenia in cirrhosis. Some of these tools are reproducible and easy to apply, which facilitates their global application for screening malnutrition and sarcopenia. Objective: to assess malnutrition and sarcopenia in liver cirrhosis and to compare the accuracy of diagnostic tools in this population. Method: a cross-sectional analytical study was conducted with convenience sampling by using continuous inclusion of patients with liver cirrhosis in a tertiary care center during December 2018 to May 2019. The nutritional assessment was carried out with arm anthropometry, body mass index (BMI), and the algorithm of the Royal Free Hospital Subjective Global Assessment (RFH-SGA). For the evaluation of sarcopenia, the hand grip strength test with a hand dynamometer was applied. The results were reported in measures of central tendency expressed in frequency and percentage. A Kendall's Tau-b rank correlation coefficient was performed with non-parametric variables, considering a p < 0.05 as a statistically significant value. Results: a total of 103 patients were included with a predominance of the male gender (79.6 %) and a mean age of 51 years (± 10). The etiology of liver cirrhosis corresponded more frequently to alcohol consumption (68 %) and most of the patients were Child-Pugh C (57.3 %) with a mean MELD of 21.9 (± 8.9). A mean BMI with dry weight of 25.2 kg/m2 was reported, and with respect to the WHO classification by BMI, 7.8 % were underweight and 59.2 % were malnourished by RFH-SGA. Sarcopenia was present in 88.3 % using the hand grip strength test, for which a mean of 18.99 kg was found. A Kendall's Tau-b rank correlation coefficient was performed between BMI and RFH-SGA, which showed no statistically significant association, as well as between mean arm muscle circumference percentiles and hand grip strength. Conclusions: global assessment in liver cirrhosis should include screening for malnutrition and sarcopenia, for which validated, accessible and safe application tools should be used, such as anthropometric assessment, RFH-SGA, and hand grip strength.

Resumen Introducción: la malnutrición y la sarcopenia son frecuentes en la población con cirrosis hepática y generan un impacto negativo en el estado funcional y la esperanza de vida de estos pacientes. Existen múltiples herramientas para la valoración de la malnutrición y la sarcopenia en los pacientes con cirrosis hepática. Algunas de estas herramientas son de fácil aplicación y reproducibilidad, lo que facilita su aplicación global. Objetivo: valorar la malnutrición y la sarcopenia en la cirrosis hepática y comparar distintas herramientas diagnósticas aplicables en esta población. Método: se realizó un estudio analítico de corte transversal con muestreo a conveniencia mediante inclusión continua de pacientes con cirrosis hepática en un hospital de tercer nivel durante diciembre de 2018 a mayo de 2019. Se realizó la valoración nutricional con la antropometría del brazo, el índice de masa corporal (IMC) y el algoritmo del Royal Free Hospital Subjetive Global Assessment (RFH-SGA). Para la valoración de la sarcopenia se aplicó la fuerza de agarre de la mano con un dinamómetro. Los resultados se reportaron en medidas de tendencia central expresadas en frecuencia y porcentaje. Se realizó un coeficiente de correlación con Tau-b de Kendall en las variables no paramétricas, considerándose como valor estadísticamente significativo una p < 0,05. Resultados: se incluyeron un total de 103 pacientes, predominando el género masculino (79,6 %), con una edad media de 51 años. La etiología más frecuente de la cirrosis hepática fue el consumo de alcohol (68 %), predominando la clase Child-Pugh C (57,3 %) con una media de MELD de 21,9 (± 8,9). Se reportó una media de IMC con peso seco de 25,2 kg/m2 y, respecto a la clasificación de la OMS, un 7,8 % se encontraban en bajo peso y un 59,2 % en malnutrición según la RFH-SGA. Un 88,3 % presentó sarcopenia al utilizar la fuerza de agarre de la mano, cuyo valor medio fue de 18,99 kg. Se realizó una correlación con Tau b de Kendall entre IMC y RFH-SGA sin evidenciarse ninguna asociación significativa, al igual que entre los percentiles de la circunferencia muscular media de brazo (MAMC) y la fuerza de agarre de la mano. Conclusiones: la valoración integral de la cirrosis hepática debe incluir el escrutinio de la malnutrición y la sarcopenia, existiendo herramientas de fácil acceso y aplicación segura validadas en esta población, como la valoración antropométrica, el RFH-SGA y la fuerza de agarre de la mano.

Resumen La presente recomendación busca brindar un marco de seguridad para la prescripción de actividad física en niños, niñas y adolescentes con diabetes mellitus tipo 1 (DM1), considerando la evaluación de las situaciones que puedan presentarse antes, durante y después de su práctica. Incluye las estrategias terapéuticas sobre el tipo de ejercicio, el control glucémico capilar(o mediante el uso de monitoreo continuo de glucosa, MCG) y la adecuación de la insulinoterapia y de la ingesta de hidratos de carbono. Se prioriza que, para optimizar los beneficios del ejercicio como parte del tratamiento de la DM1, es importante una complete y constant educación diabetológica para el paciente y sus cuidadores brindada por un equipo interdisciplinario entrenado en el manejo integral de niños, niñas y adolescentescon DM1.

Abstract These recommendations seek to provide a safety framework for the prescription of physical activity in children and adolescents with DM1, considering the evaluation of the situations that may arise before, during and after the practice of physical activity. It includes therapeutic strategies on the type of exercise, intensive capillary glycemic control or through the use of continuous glucose monitoring (CGM) and the adequacy of insulin therapy and carbohydrate intake. It is prioritized that to optimize the benefits of exercise as part of the treatment of DM1, a complete and constant diabetes education is important, provided by an interdisciplinary team trained in the comprehensive management of children and adolescents with DM1.

Resumen Objetivo: Analizar la literatura disponible sobre las propiedades psicométricas de los instrumentos para medir Conocimientos, Actitudes y Prácticas del Proceso de Cuidado de Enfermería. Métodos: Revisión de tipo narrativa según las recomendaciones de la declaración PRISMA. La estrategia de búsqueda se realizó en dos etapas; a partir de la búsqueda en bases de datos por parte de 2 revisores y, posteriormente, tres revisores identificaron de forma independiente los estudios y evaluaron la calidad metodológica de los instrumentos de medición utilizando la COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN). Resultados: De 71 estudios identificados para la revisión de texto completo, solo 7 cumplieron los criterios de inclusión que representan 4 instrumentos diferentes (Q-DIO, D-CATCH, NP-CDSS, PPE). Se encontró que los instrumentos continúan en procesos de validación y apropiación de los mismos a la realidad en los servicios de salud. Conclusión: A pesar de la evidente evolución de los instrumentos para evaluar la implementación del Proceso de Cuidado de Enfermería, aún sigue vigente la necesidad de un instrumento que mida los aspectos de Conocimientos, Actitudes y Prácticas en todas las etapas del proceso.

Abstract Objective: To analyze the literature available on the psychometric properties of the instruments to measure knowledge, attitudes, and practices of the nursing care process. Methods: This was a narrative-type review conducted by following the recommendations of the PRISMA declaration. The search strategy was executed in two stages; through the search in databases by two reviewers and - thereafter - three reviewers identified independently the studies and evaluated the methodological quality of the measurement instruments by using the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) property checklist boxes. Results: Of 71 studies identified for the full-text review, only seven complied with the inclusion criteria that represent four instruments (Q-DIO, D-CATCH, NP-CDSS, PNP). It was found that the instruments continue in their validation and appropriation processes to reality in health services. Conclusion: In spite of the evident evolution of the instruments to evaluate the implementation of the nursing care process, the need is still valid for an instrument that measures aspects of knowledge, attitudes, and practices in every stage of the process.

A estratégia de busca foi realizada em duas etapas; por meio da busca nas bases de dados CINAHL, MEDLINE, BVS e Google Scholar por 2 revisores e, posteriormente, três revisores identificaram os estudos de forma independente e avaliaram a qualidade metodológica dos instrumentos de medição usando a COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN). Resultados: Dos 71 estudos identificados para revisão de texto completo, apenas 7 preencheram os critérios de inclusão representando 4 instrumentos diferentes (Q-DIO, D-CATCH, NP-CDSS, PPE). Constatou-se que os instrumentos continuam em processos de validação e apropriação dos mesmos à realidade nos serviços de saúde. Conclusão: Apesar da evidente evolução dos instrumentos de avaliação da implementação do Processo de Cuidar em Enfermagem, persiste a necessidade de um instrumento que mensure os aspectos de Conhecimento, Atitudes e Práticas em todas as etapas do processo

Resumen En la vereda el Tunal en el municipio de Paipa, Boyacá, se presenta escasez de agua en temporada de estiaje debido al aumento poblacional, a la ubicación de los puntos de captación, a los sistemas de bombeo y a los usuarios ya que están en una montaña. Debido a esto, la población necesita un abastecimiento con sistemas de bombeo que suponen un costo energético alto. En el presente artículo, se utilizan herramientas computacionales para establecer la viabilidad técnica de un rediseño del sistema de suministro de agua, proponiendo finalmente una solución que disminuye costos energéticos y la escasez de agua en la temporada de sequía al mismo tiempo. Se pudo llegar a la conclusión que la propuesta puede llegar a ser viable desde el punto de vista técnico, ya que es posible utilizar la tubería existente, ubicando un nuevo embalse en un punto más cercano al punto de suministro concluyendo que hidrológicamente el punto de captación puede abastecer a la población según el registro histórico de 14 años de lluvia de la estación IDEAM más cercana al proyecto.

Abstract In El Tunal, Paipa, Boyacá, there is a shortage of water during the low water season due to the increase in population, the location of the collection points, pumping systems and the users, who are located on a mountain. Due to this, the population needs a supply with pumping systems that suppose a high energy cost. In this article, computer tools are used to find out the technical feasibility of a water supply system redesign, and finally propose a solution that reduces energy costs and water scarcity in the dry season at the same time. It is possible to conclude that the proposal can be technically viable, since it is possible to use the existing pipeline, by locating a new reservoir at a closer point to the supply point, therefore the hydrological point can supply the population according to the historical record of 14 years of rainfall at the IDEAM station closest to the project.

Abstract: The control of acute pain in hip surgery is a challenge for the anesthesiologist, not only because of the complexity of the pain derived from osteotomies in a larger joint; but because of the multipathology and fragility that accompanies the geriatric patient. The pericapsular nerve block, known as «PENG block» is a purely sensitive ultrasound-guided block that inhibits the surgical nociceptive impulse of three nerves of the hip joint: the femoral nerve, the obturator nerve and the accessory obturator nerve. This block has an average duration of 10 to 12 hours, prolonging this efficiency and the safety margin provided by this technique led us to place an ultrasound-guided catheter in the PENG block for a male patient scheduled for total hip arthroplasty with the objective of controlling pain up to 48 hours after the surgical procedure, optimizing resources and reducing their hospital stay and reducing the risk of delirium and adverse effects derived from the use of opioids commonly associated in elderly patients. Continuous PENG block is part of multimodal management coupled with general anesthesia or other regional anesthesia techniques, with more hours of analgesia compared to a single dose.

Resumen: El control de dolor agudo en cirugía de cadera es un reto para el anestesiólogo, no sólo por la complejidad del dolor derivado de las osteotomías en una articulación mayor, sino por la multipatología y fragilidad que acompaña al paciente geriátrico. El bloqueo de los nervios pericapsulares, conocido como «bloqueo PENG» es un bloqueo guiado por ultrasonido puramente sensitivo que inhibe el impulso nociceptivo quirúrgico de tres nervios de la articulación coxofemoral: el nervio femoral, el nervio obturador y el nervio obturador accesorio. Este bloqueo tiene una duración promedio de 10 a 12 horas, prolongar esta eficacia y el margen de seguridad provistos por esta técnica fue el motivo para realizar la colocación de un catéter ecoguiado en el bloqueo PENG en un paciente masculino programado para artroplastía total de cadera con el objetivo de controlar el dolor hasta 48 horas después del procedimiento quirúrgico, optimizar recursos y disminuir su estancia intrahospitalaria y el riesgo de presentación de delirio y efectos adversos derivados del uso de opioides comúnmente asociados en pacientes de edad avanzada. El bloqueo PENG continuo forma parte del manejo multimodal aunado a anestesia general u otras técnicas de anestesia regional, con más horas de analgesia comparado con una dosis única.

RESUMEN Se estudió el efecto de citocininas y auxinas en la producción de brotes adventicios de Agave durangensis in vitro. La germinación se hizo en medio de cultivo Murashige y Skoog (MS) al 50% de concentración, sin reguladores de crecimiento. La producción y desarrollo de brotes adventicios se realizó en medio de cultivo MS al 100%, con citocininas (KIN y BAP) y ANA como auxina, evaluando diferentes concentraciones en 12 tratamientos. La concentración de 4.0 mg L-1 de BAP sin auxinas presentó mejores resultados en altura del explante, número de hojas por explante, número de brotes por explante y número de hojas por brote. La presencia de auxinas sólo favoreció la altura del brote en el tratamiento de 4.0 mg L-1 KIN y 0.4 mg L-1 ANA. Por lo anterior, resulta viable utilizar solo citocininas en medio de cultivo en la etapa de producción de brotes de A. durangensis.

ABSTRACT The effect of cytokinins and auxins on the production of adventitious shoots of Agave durangensis in vitro was studied. Germination was done in Murashige and Skoog (MS) culture medium at 50% concentration without growth regulators was used. The production and development of adventitious shoots was carried out in a 100% MS culture medium with cytokinins (KIN and BAP) and ANA as auxin, evaluating different concentrations in 12 treatments. The concentration of 4.0 mg L-1 of BAP without auxins presented better results in explant height, number of leaves per explant, number of shoots per explant and number of leaves per shoot. The presence of auxins only favored the height of the sprout in the treatment of 4.0 mg L-1 KIN and 0.4 mg L-1 ANA. Therefore, it is feasible to use only cytokinins on culture medium in the shoots production stage of A. durangensis.

Abstract The knowledge of species diversity, the associated ecosystem functions, and the network of community structure are key for effective decision-making for conservation legislation. In Mendoza Province, the study of mammals has been relevant for the development of theoretical models, but such information is scattered and patchy. In this paper, we evaluate the pattern of organization and the conservation status of the diversity of mammals in the Monte desert of Mendoza, integrating these data into two Provincial Conservation Strategies (PCS): Protected Areas and Native Forest Management Law. We found that both specific and functional richness were equal inside and outside the PCS locations. However, these variables are more strongly correlated within the PCS. In addition, rarefied richness and species of outstanding value were higher in localities within the PCS. Finally, the modular structure of the regional system, together with the presence of a large number of locations forming connections within the PCS network, indicate that the PCS is currently effective in protecting not only mammalian biodiversity, but also the connectivity of the regional system. This large number of corridors strengthens the structural continuity of mammalian biodiversity throughout the environmental gradient of Monte, within Mendoza Province.

Resumen El conocimiento de la diversidad de especies, las funciones que ellas cumplen en el ecosistema y la red de organización comunitaria es clave para la toma de decisiones en conservación. En la Provincia de Mendoza, el estudio de la fauna de mamíferos ha sido relevante en el desarrollo de modelos teóricos, pero dicha información se encuentra dispersa y parchada. En este trabajo evaluamos el patrón de organización y el estado de conservación de la diversidad de mamíferos del Monte mendocino integrando localidades pertenecientes a dos Estrategias Provinciales de Conservación (ECP): Áreas protegidas y Ley de Presupuestos Mínimos de Protección Ambiental de los Bosques Nativos. Encontramos que la riqueza específica y la funcional fueron iguales dentro y fuera de las localidades de la ECP aunque se correlacionaron más fuertemente dentro de la ECP. Además, la riqueza rarefaccionada y el número de especies de valor sobresaliente fue mayor en localidades dentro de la ECP. Por último, la organización modular del sistema regional, junto con la presencia de un gran número de localidades conectoras en la red espacial de la ECP, indicarían que la ECP está actualmente siendo eficaz en conservar no sólo la biodiversidad de mamíferos, sino también la conectividad del sistema regional, ya que el gran número de localidades conectoras reaseguran la continuidad estructural de la biodiversidad de mamíferos a través del gradiente ambiental del Monte dentro de la provincia de Mendoza.

Abstract Osteoarthritis (OA) is a complex disease with a multifactorial etiology. The genetic component is one of the main associated factors, resulting from interactions between genes and environmental factors. The aim of this study was to identify gene-gene interactions (epistasis) of the articular cartilage extracellular matrix (ECM) in knee OA. Ninety-two knee OA patients and 147 healthy individuals were included. Participants were genotyped in order to evaluate nine variants of eight genes associated with ECM metabolism using the OpenArray technology. Epistasis was analyzed using the multifactor dimensionality reduction (MDR) method. The MDR analysis showed significant gene-gene interactions between MMP3 (rs679620) and COL3A1 (rs1800255), and between COL3A1 (rs1800255) and VEGFA (rs699947) polymorphisms, with information gain values of 3.21% and 2.34%, respectively. Furthermore, in our study we found interactions in high-risk genotypes of the HIF1AN, MMP3 and COL3A1 genes; the most representative were [AA+CC+GA], [AA+CT+GA] and [AA+CT+GG], respectively; and low-risk genotypes [AA+CC+GG], [GG+TT+GA] and [AA+TT+GA], respectively. Knowing the interactions of these polymorphisms involved in articular cartilage ECM metabolism could provide a new tool to identify individuals at high risk of developing knee OA.

Resumen Este texto analiza cómo un grupo de mujeres trabajadoras de la ciudad de Bogotá configura unas dinámicas espacio-temporales particulares, mediante su experiencia del transporte público. La construcción de estas dinámicas se fundamenta en la concepción del tiempo como un valor, como una inversión importante que moldea el resto de los aspectos de la vida de estas mujeres. El tiempo, entonces, se asigna alrededor de dos centros de gravedad: el trabajo y el transporte. Estos configuran nodos en el día a día de las mujeres, que son esenciales para la organización de sus vidas cotidianas. Sin embargo, el transporte es fundamental debido a su indeterminación; no es un contexto espacio-temporal de trabajo, a pesar de ser necesario para este, pero tampoco es “tiempo libre”. Esta indeterminación conduce a la pregunta por el papel del derecho laboral en estas dinámicas: si no es tiempo laboral, ni tiempo libre… ¿de quién es ese tiempo?

Abstract This text analyzes the ways in which a group of female workers in the city of Bogotá configure a set of specific spatio-temporal dynamics, giving their experience of public transportation. The basis for these dynamics lies on the conception of time as a value, as an essential investment that shapes the rest of the aspects related to the lives of these women. Time, then, revolves around two centers of gravity: work - in its various forms - and transportation. These centers of gravity are nodes in the organization of their daily lives. However, commuting undertakes this role because of its own indeterminacy. It is neither a spatio-temporal context that is part of work, even if it is necessary for this purpose, nor it is ‘leisure-time’. This indeterminacy leads us to the question of what role does labor law play in these dynamics; if it is not working time, nor leisure-time, then… whose time is it?

Crystalline cellulose nanofibers are obtained from the bark of Cereus Forbesii, a cactus native to the arid areas of South America. The obtaining of cellulose nanofibers was carried out in several steps: pretreatment of the raw material, elimination of hemicellulose and lignin to obtain cellulose, and an acid hydrolysis of cellulose to obtain crystalline cellulose nanofibers. The cellulose nanofibers obtained have a crystallinity index of 82% and a nanofiber diameter of 18 nm. An average crystallite size of 6 nm was calculated for the crystalline domains that form cellulose nanofibers. The high crystallinity of the obtained cellulose nanofibers makes the sample very homogeneous and decomposes in a relatively narrow temperature range (between 290°C and 375°C). The complete degradation of crystalline cellulose polymer chains takes place between 375°C and 600°C. The morphological and structural studies are carried out by scanning electron microscopy of field emission, infrared spectrometry with Fourier transform, and powder X-ray diffraction. The thermal stability of the samples is determined by thermogravimetric analysis.

Resumen Introducción: Staphylococcus aureus es una frecuente causa de bacteriemias y el incremento en las tasas de resistencia dificulta su tratamiento inicial. La incidencia de bacteriemias por S. aureus resistente a meticilina (SAMR) ha aumentado, principalmente en las infecciones adquiridas en el medio hospitalario. Este estudio pretende caracterizar clínicamente a los pacientes con bacteriemia por S. aureus detectados en un hospital de Medellín y definir su perfil de resistencia microbiana. Métodos: cohorte retrospectiva en un hospital de tercer nivel. Se incluyeron pacientes >16 años con aislamiento para Staphylococcus aureus en sangre periférica. Se registró información relacionada con el episodio, el sitio de adquisición, la clasificación microbiológica y las características clínicas. Resultados: de 775 reportes de hemocultivos positivos para S. aureus, finalmente se ingresaron 698 pacientes al estudio. La mediana de edad fue de 58 años (RIQ=42-69), 383 eran hombres (54.9%). En 354 pacientes (50.7%) la bacteriemia era de origen nosocomial y el 26.2% (n=183) eran SAMR. En 123 (17.6%) bacteriemias adquiridas en la comunidad, la frecuencia de SAMR fue 33.3% (n=41). El antecedente más frecuente fue tener un catéter vascular en 321 pacientes (46%). Se inició terapia empírica en 67.2% de los casos (n=469). Respecto a la gravedad, la mediana del puntaje SOFA fue de 4 (RIQ=1-5), APACHE II de 15 (RIQ=10-19) y la mortalidad fue del 24.9% (n=174). Conclusiones: a pesar de que la mayoría de bacteriemias son de origen nosocomial, el reporte más frecuente fue SAMS tanto en el grupo nosocomial como en el de la comunidad. Sin embargo, la mortalidad en el grupo de bacteriemia nosocomial fue inferior comparada con la de la comunidad. (Acta Med Colomb 2018; 43: 200-206).

Abstract Introduction: Staphylococcus aureus is a frequent cause of bacteremia and the increase in resistance rates makes initial treatment difficult. The incidence of bacteraemia due to methicillin-resistant S. aureus (MRSA) has increased, mainly in infections acquired in the hospital environment. This study aims to clinically characterize patients with S. aureus bacteremia detected in a Medellín hospital and to define their microbial resistance profile. Methods: retrospective cohort in a tertiary hospital. Patients >16 years with isolation for Staphylococcus aureus in peripheral blood were included. Information related to the episode, the acquisition site, the microbiological classification and the clinical characteristics were recorded. Results: Of 775 reports of blood cultures positive for S. aureus, 698 patients were finally admitted to the study. The median age was 58 years (RIQ = 42-69), 383 were men (54.9%). In 354 patients (50.7%) the bacteremia was of nosocomial origin and 26.2% (n = 183) were MRSA. In 123 (17.6%) bacteremia acquired in the community, the frequency of MRSA was 33.3% (n = 41). The most frequent antecedent was to have a vascular catheter in 321 patients (46%). Empirical therapy was initiated in 67.2% of the cases (n = 469). Regarding severity, the SOFA score median was 4 (RIQ = 1-5), APACHE II of 15 (RIQ = 10-19) and mortality was 24.9% (n = 174). Conclusions: although the majority of bacteremia are of nosocomial origin, the most frequent report was MSSA in both the nosocomial and community groups. Despite this, mortality in the nosocomial bacteremia group was lower compared with that of the community. (Acta Med Colomb 2018; 43: 200-206).

RESUMEN Introducción: las jornadas de detección de cáncer de piel son una herramienta para la educación e identificación temprana de individuos afectados. Se presentan los resultados de la participación de la Sección de Dermatología de la Universidad de Antioquia en la quinta jornada de la Asociación Colombiana de Dermatología y Cirugía Dermatológica, realizada el 25 y 26 de agosto de 2016. Métodos: pacientes mayores de 18 años y menores con consentimiento de los padres. Se realizó evaluación clínica de toda la piel y análisis histopatológico de las lesiones sospechosas de cáncer de piel, previo consentimiento informado por escrito. Resultados: se adjudicaron 132 consultas y de los asistentes 33 tuvieron lesiones sospechosas de malignidad, a 22 se les tomó biopsia y a 11 se les hizo diagnóstico clínico, de estos, 9 con queratosis actínicas, 1 con nevus atípico y otro paciente con ambos diagnósticos. En 11 pacientes se diagnosticó cáncer de piel, uno de ellos con 2 melanomas malignos, 1 con carcinoma escamocelular y 9 con carcinoma basocelular. Discusión: la cuarta parte de los asistentes presentaron lesiones sospechosas de malignidad. El carcinoma basocelular fue el tipo de cáncer más frecuente, 6,06 %, seguido por el carcinoma espinocelular 0,75 % y el melanoma maligno 0,75 %. Preocupan los comportamientos de riesgo de exposición solar; de los pacientes con lesiones premalignas o malignas, el 40,6 % refirió quemaduras solares antes de los 18 años y el 48,5% practicaban actividades al sol. Predominan los fototipos II y III en el 83,4 %, lo que aumenta el riesgo de cáncer de piel.

SUMMARY Introduction: Skin cancer screening days are a tool for the early identification and education of affected individuals. We present the results of the participation of the Dermatology Service of the University of Antioquia on the fifth campaign of the Colombian Association of Dermatology and Dermatologic Surgery, held on August 25 and 26, 2016. Methods: Patients Older than 18 years and minors with parental consent. We performed clinical evaluation of total body skin and histopathological analysis of suspected skin cancer lesions, with prior written informed consent. Results: A total of 132 patients were attended, 33 were suspected lesions, 22 were biopsied, 9 underwent clinical diagnosis of actinic keratoses, 1 atypical nevus and 1 both diagnosis. In 11 patients skin cancer was diagnosed, one with two malignant melanomas, one with squamous cell carcinoma and nine with basal cell carcinoma. Discussion: 25 % of the attendees presented suspicious injuries. Basal cell carcinoma was the most common type of cancer, 6.06 %, followed by squamous cell carcinoma 0.75 % and malignant melanoma 0.75 %. Concern about risk behavior of sun exposure; of patients with premalignant or malignant lesions, 40.6 % reported sunburn before age 18 and 48.5% practiced activities in the sun. RESUMEN Introducción: la enfermedad de Parkinson es considerada la segunda causa de enfermedad neurodegenerativa, en la que se destacan signos y síntomas motores como temblor, bradicinesia, rigidez e inestabilidad postural, acompañados de síntomas no motores como alteraciones del sueño, autonómicas, cognitivas, gastrointestinales, entre otras. El tratamiento farmacológico de la enfermedad al inicio suele ser útil, pero cuando los síntomas persisten, el tratamiento falla o no se toleran sus reacciones adversas, es necesario considerar alternativas como la estimulación cerebral profunda. Metodología: revisión narrativa con énfasis en los aspectos clínicos de la terapia con estimulación cerebral profunda en los pacientes con enfermedad de Parkinson. Discusión: la estimulación cerebral profunda es una técnica quirúrgica en la que se implantan electrodos en regiones cerebrales específicas, generalmente el núcleo subtalámico, globo pálido interno o núcleo ventral intermedio del tálamo, y se conectan a un marcapasos subcutáneo desde donde se modula eléctricamente la actividad de estas áreas. Esta terapia ha mostrado ser costo-efectiva, aporta beneficios considerables en la mejoría de los síntomas de la enfermedad de Parkinson y cuenta con evidencia clínica en los pacientes que han sido seleccionados correctamente.

SUMMARY Introduction: Parkinson’s disease is considered the second cause of neurodegenerative disease, in which motor signs and symptoms such as tremor, bradykinesia, rigidity and postural instability are highlighted, accompanied by non-motor symptoms such as sleep, autonomic, cognitive, gastrointestinal among others disturbances. The pharmacological treatment of the disease at the beginning is usually useful, but when the symptoms persist, the treatment fails or its adverse reactions are not tolerated, it is necessary to consider alternatives such as deep brain stimulation. Methodology: This is a narrative review with emphasis on the clinical aspects of deep brain stimulation therapy in patients with Parkinson’s disease. Discussion: Deep brain stimulation is a surgical technique in which electrodes are implanted in specific brain regions, usually the subthalamic nucleus, globus pallidus interna or ventral intermediate nucleus of the thalamus, and are connected to a subcutaneous pacemaker from which the activity of these areas is modulated electrically. This therapy has been shown to be cost-effective, provides considerable benefits in improving the symptoms of Parkinson’s disease and has clinical evidence in patients who have been correctly selected.

Abstract: The objective of this study is to describe and analyze how teachers evaluate competencies on using the methodology of project-based learning. From a situated and sociocultural constructivist framework, a model of analysis was employed to explain the situations of evaluation identified during a didactic sequence in which a teacher and her fifth-grade students carried out a project in Spanish class; the project involved the preparation of a nutritional menu and incorporated content studied in their natural science class. The results show four types of situations of evaluation and the presence of elements congruent with the evaluation of competencies. Also offered are orientations to strengthen the design of situations of evaluation in future projects.

Resumen: El objetivo de este estudio es describir y analizar cómo los profesores evalúan competencias a partir de la metodología Aprendizaje Basado en Proyectos. Desde un marco constructivista sociocultural y situado, se utilizó un modelo de análisis para dar cuenta de las situaciones de evaluación identificadas durante una secuencia didáctica, en la que una profesora y sus alumnos de quinto grado de primaria desarrollaron un proyecto en la materia de Español sobre la elaboración de un menú nutritivo, incorporando contenidos revisados en la asignatura de Ciencias naturales. Los resultados muestran cuatro tipos de situaciones de evaluación diferenciados y la presencia de elementos congruentes con una evaluación de competencias. Igualmente, se ofrecen algunas orientaciones para fortalecer el diseño de las situaciones de evaluación en futuros proyectos.
