Abstract Trauma is a leading cause of death, permanent disability, and health care cost worldwide. The young and economically active are the most affected population. Exsanguination due to noncompressible torso hemorrhage is one of the most frequent causes of early death, posing a significant challenge to trauma and vascular surgeons. The possibility of limb loss due to vascular injuries must also be considered. In recent decades, the approach to vascular injuries has been significantly modified. Angiotomography has become the standard method for diagnosis, endovascular techniques are currently incorporated in treatment, and damage control, such as temporary shunts, is now the preferred approach for the patients sustaining physiological derangement. Despite the importance of this topic, few papers in the Brazilian literature have offered guidelines on vascular trauma. The Brazilian Society of Angiology and Vascular Surgery has developed Projetos Diretrizes (Guideline Projects), which includes this publication on vascular trauma. Since treating trauma patients is a multidisciplinary effort, the Brazilian Trauma Society (SBAIT) was invited to participate in this project. Members of both societies reviewed the literature on vascular trauma management and together wrote these guidelines on vascular injuries of neck, thorax, abdomen, and extremities.

Resumo Trauma é uma causa importante de morbimortalidade, que acomete principalmente jovens. A hemorragia incoercível é o principal mecanismo de óbito precoce nessas vítimas, e as lesões vasculares não compressíveis representam grandes desafios para os cirurgiões. O traumatismo vascular impacta diretamente a viabilidade de membros traumatizados, aumentando o risco de amputação. Nas últimas décadas, muitas condutas de diagnóstico e tratamento de lesões vasculares traumáticas foram modificadas. A angiotomografia suplantou a angiografia como padrão ouro para diagnóstico, as técnicas endovasculares foram incorporadas ao arsenal terapêutico e o conceito de “controle de danos” foi estabelecido. No entanto, há lacunas na literatura nacional sobre a normatização de condutas em trauma vascular, principalmente considerando as limitações do Brasil. Por isso, a Sociedade Brasileira de Angiologia e de Cirurgia Vascular e a Sociedade Brasileira de Atendimento Integrado ao Traumatizado revisaram a literatura disponível sobre trauma vascular e organizaram diretrizes sobre o diagnóstico e tratamento dessas lesões.

RESUMO Objetivo: análise crítica da investigação diagnóstica de lesões em segmento cefálico de motociclistas vítimas de acidentes de tráfego. Método: estudo observacional prospectivo incluindo motociclistas adultos vítimas de trauma, sem intoxicação exógena, em um período de 12 meses. A tomografia de crânio (TC) foi indicada de acordo com uma modificação dos “critérios canadenses”. Os pacientes que não foram submetidos a TC de crânio tiveram acompanhamento telefônico por três meses. A presença de lesões foi correlacionada com as varáveis coletadas através dos testes Qui-quadrado, t de Student ou Fisher, considerando p<0,05 como significativo. Resultados: dos 208 inicialmente incluídos, 206 (99,0%) estavam usando capacete. Dezessete estavam com sinais de intoxicação exógena e foram excluídos, restando 191 para análise. Noventa pacientes (47,1%) realizaram TC e 12 (6,3%) apresentaram lesões craniencefálicas, que se associaram significativamente a Escala de Coma de Glasgow (ECG) <15 (52,3% vs. 2,8% - p<0,001) e alterações ao exame físico da região cefálica/neurológico (17,1% vs. zero - p<0,05). Quatro pacientes (2,1%) precisaram tratamento cirúrgico de lesões intracranianas. Nenhum dos pacientes admitidos com ECG 15, em uso de capacete e sem alterações no exame físico apresentou TC alterada. Conclusões: para pacientes admitidos com ECG 15, que utilizavam o capacete no acidente e não apresentavam quaisquer alterações no exame físico, a realização da TC de crânio não trouxe mudanças no atendimento ao paciente. .

ABSTRACT Objective: to review the clinical assessment of head injuries in motorcyclists involved in traffic accidents. Method: prospective observational study, including adult motorcyclists involved in traffic accidents in a period of 12 months. Patients sustaining signs of intoxication were excluded. A modification of the Canadian Head CT Rules was used to indicate computed tomography (CT). Patients not undergoing CT were followed by phone calls for three months. Collected variables were compared between the group sustaining head injuries and the others. We used chi-square, Fisher, and Student’s t for statistical analysis, considering p<0.05 as significant. Results: we included 208 patients, 99.0% were wearing helmets. Seventeen sustained signs of intoxication and were excluded. Ninety (47.1%) underwent CT and 12 (6.3%) sustained head injuries. Head injuries were significantly associated with Glasgow Coma Scale<15 (52.3% vs. 2.8% - p<0,001) and a positive physical exam (17.1% vs. zero - p<0,05). Four (2.1%) patients with intracranial mass lesions needed surgical interventions. None helmet-wearing patients admitted with GCS=15 and normal physical examination sustained head injuries. Conclusion: Head CT is not necessary for helmet-wearing motorcyclists admitted with GCS=15 and normal physical examination.

RESUMO Objetivo: identificar as variáveis relacionadas às complicações pleurais em vítimas de trauma submetidas a drenagem torácica. Método: estudo observacional prospectivo entre maio/2019 e janeiro/2021, incluindo adultos submetidos a drenagem torácica após a admissão hospitalar para tratamento de lesões traumáticas. Pacientes que desenvolveram COVID19 foram excluídos. As complicações pleurais foram caracterizadas como: pneumotórax residual, hemotórax residual / coagulado e empiema pleural. Comparamos as variáveis coletadas entre os grupos de pacientes com e sem estas complicações. Os testes t de Student, Mann Whitney, Chi quadrado e Fisher foram utilizados para análise estatística. Foram considerados significativos valores de p<0,05. Resultados: analisamos 68 casos, com média etária de 36,0 + 12,6 anos, de RTS de 7,0 ± 1,6 e ISS de 15,9 ± 7,6, sendo 62 (91,2%) do sexo masculino. Trinta e quatro pacientes (50,0%) foram vítimas de ferimento por arma branca. O AIS das lesões torácicas foi classificado como 2 (4,4%), 3 (80,9%), 4 (13,2%), e 5 (1,5%). As complicações pleurais ocorreram em 14 casos (20,5%), sendo hemotórax residual / coagulado em 8 casos (11,8%), pneumotórax residual em 3 (4,4%), o empiema pleural em 2 (2,9%) e associações em 1 (1,4%). Estas complicações foram tratadas por videotoracoscopia (5 casos), toracotomia (3), redrenagem de tórax (3) e medidas clínicas apenas (3). A redrenagem de tórax (p<0,001) e o tempo de permanência do dreno (p<0,001) tiveram relação significativa com a presença de complicações pleurais. Conclusão: a redrenagem do tórax e o maior tempo de permanência do dreno foram associados às complicações pleurais.

ABSTRACT Objective: to identify variables related to pleural complications in patients undergoing tube thoracostomies due to traumatic injuries. Method: we conducted a prospective observational study from May/2019 to January/2021 including adult trauma patients submitted to tube thoracostomies after hospital admission. Patients undergoing thoracotomies as the initial treatment were not included. We excluded patients with suspected and confirmed COVID-19 diagnosis during the hospitalization. Pleural complications were defined as clotted hemothorax, residual pneumothorax and empyema. Students t, Mann Whitneys, Chi square and Fishers exact test were used to compare variables between groups. We considered p<0.05 as significant. Results: we analyzed 68 patients. The mean age was 36.0 + 12.6 years and 91.2% were male. The mean RTS and ISS were, respectively, 7.0 ± 1.6 and 15.9 ± 7.6. The most frequent trauma mechanism was stab wounds in 50.0%, followed by blunt trauma in 38.2%. The severity of thoracic injuries was stratified (AIS) as 2 (4.4%), 3 (80.9%), 4 (13.2%), e 5 (1.5%). Pleural complications happened in 14 (20.5%) patients, being clotted / residual hemothorax (11.8%), residual pneumothorax (4.4%), empyema (2.9%) and miscellaneous (1.4%). These patients were treated by thoracoscopy (5), thoracotomy (3), chest re-drainage (3) and clinical measures alone (3). There was a significant association between pleural complications with the time of permanence (p<0,001) and the necessity of relocation (p<0,001) of the drain. Conclusion: the predictors of pleural complications in this series were time of permanence and the necessity of relocation of the drain.

RESUMO Objetivo: Avaliar a utilidade da autópsia no diagnóstico de lesões despercebidas (LD) e no estabelecimento de metas para programa de qualidade em trauma. Método: análise retrospectiva dos laudos de autópsia por trauma entre outubro/2017 e março/2019 provenientes do mesmo hospital. Lesões descritas na autópsia, mas não no prontuário médico, foram consideradas como despercebidas (LD) e classificadas pelos critérios de Goldman: Classe I: mudariam a conduta e alterariam o desfecho; Classe II: mudariam a conduta, mas não o desfecho; Classe III: não mudariam nem a conduta nem o desfecho. As variáveis coletadas foram comparadas entre o grupo com LD e os demais, através de método estatístico orientado por profissional na área. Consideramos p<0,05 como significativo. Resultados: analisamos 192 casos, com média etária de 56,8 anos. O trauma fechado foi o mecanismo em 181 casos, sendo 28,6% por quedas da própria altura. LD foram observadas em 39 casos (20,3%), sendo 3 (1,6%) classe I e 11 (5,6%) classe II. O tórax foi o segmento com maior número de LD (25 casos - 64,1% das LD). Foram associados à presença de LD (p<0,05): tempo de internação menor que 48 horas, mecanismo de trauma grave e a não realização de procedimento cirúrgico ou tomografia. Nos óbitos até 48h, valores de ISS e NISS nas autópsias foram maiores que os da internação. Conclusão: a revisão das autópsias permitiu identificação de LD, na sua maioria sem influência sobre conduta e prognóstico. Mesmo assim, várias oportunidades foram criadas para o programa de qualidade.

ABSTRACT Objective: to assess the role of autopsy in the diagnosis of missed injuries (MI) and definition of trauma quality program goals. Method: Retrospective analysis of autopsy reports and patient’s charts. Injuries present in the autopsy, but not in the chart, were defined as “missed”. MI were characterized using Goldman’s criteria: Class I, if the diagnosis would have modified the management and outcome; Class II, if it would have modified the management, but not the outcome; Class III, if it would not have modified neither the management nor the outcome. We used Mann-Whitney’s U and Pearson’s chi square for statistical analysis, considering p<0.05 as significant. Results: We included 192 patients, with mean age of 56.8 years. Blunt trauma accounted for 181 cases, and 28.6% were due to falls from the same level. MI were diagnosed in 39 patients (20.3%). Using Goldman’s criteria, MI were categorized as Class I in 3 (1.6%) and Class II in 11 (5.6%). MI were more often diagnosed in the thoracic segment (25 patients, 64.1% of the MI). The variables significantly associated (p<0.05) to MI were: time of hospitalization < 48 h, severe trauma mechanism, and not undergoing surgery or computed tomography. At autopsy, the values of ISS and NISS were higher in patients with MI. Conclusion: the review of the autopsy report allowed diagnosis of MIs, which did not influence outcome in their majority. Many opportunities of improvement in quality of care were identified.

RESUMO Objetivo: analisar relação entre comprometimento de Filtros de Qualidade (FQ) com complicações e mortalidade entre vítimas de trauma grave. Métodos: análise dos dados coletados para o Registro de Trauma entre 2014 e 2015, sendo incluídos os traumatizados com Injury Severity Score (ISS) > 16 e analisados os FQ: (F1) drenagem de Hematoma Subdural Agudo (HSA) > 4 horas com Escala de Coma de Glasgow (ECG) <9, (F2) transferência da sala de emergência sem via aérea definitiva e com ECG<9, (F3) reintubação traqueal em até 48 horas, (F4) tempo entre admissão e laparotomia exploradora maior que 60 minutos em pacientes instáveis com foco abdominal, (F5) reoperação não programada, (F6) laparotomia > 4 horas, (F7) fratura de diáfise de fêmur não fixada, (F8) tratamento não operatório em Ferimento por Arma de Fogo (FAF) abdominal, (F9) tempo entre admissão e tratamento de fraturas expostas de tíbia > 6 horas, (F10) operação > 24 horas. Testes de Chi quadrado e Fisher para a análise estatística, considerando significativo p<0,05, foram usados. Resultado: foram incluídos 127 pacientes com ISS entre 17 a 75 (28,8 + 11,5). As complicações ocorreram em 80 casos (63%) e 29 morreram (22,8%). Vinte e seis pacientes apresentaram algum FQ comprometido (20,6%). Dos 101 doentes sem FQ comprometido, 22% faleceram, o que ocorreu em 7 dos 26 doentes com comprometimento dos FQ (26,9%) (p=0,595). Dos doentes sem FQ comprometido, 62% tiveram alguma complicação. Entre os pacientes com FQ comprometido, 18 (65,4%) tiveram complicações (p=0,751). Conclusão: os FQs não devem ser utilizados como preditor de mortes ou complicações evitáveis nas vítimas de traumas graves.

ABSTRACT Purpose: to analyze the relation between Trauma Quality Indicators (QI) and death, as well as clinical adverse events in severe trauma patients. Methods: analysis of data collected in the Trauma Register between 2014-2015, including patients with Injury Severity Score (ISS) > 16, reviewing the QI: (F1) Acute subdural hematoma drainage > 4 hours with Glasgow Coma Scale (GCS) <9; (F2) emergency room transference without definitive airway and GCS <9; (F3) Re-intubation within 48 hours; (F4) Admission-laparotomy time greater than 60 min in hemodynamically instable patients with abdominal bleeding; (F5) Unprogrammed reoperation; (F6) Laparotomy after 4 hours; (F7) Unfixed femur diaphyseal fracture; (F8) Non-operative treatment for abdominal gunshot; (F9) Admission-tibial exposure fracture treatment time > 6 hours; (F10) Surgery > 24 hours. T the chi-squared and Fisher tests were used to calculate statistical relevance, considering p<0.05 as relevant. Results: 127 patients were included, whose ISS ranged from 17 to 75 (28.8 + 11.5). There were adverse events in 80 cases (63%) and 29 died (22.8%). Twenty-six patients had some QI compromised (20.6%). From the 101 patients with no QI, 22% died, and 7 of 26 patients with compromised QI (26.9%) (p=0.595). From the patients with no compromised QI, 62% presented some adverse event. From the patients with any compromised QI, 18 (65.4%) had some adverse event on clinical evolution (p=0.751). Conclusion: the QI should not be used as death or adverse events predictors in severe trauma patients.

RESUMO A apendicite aguda (AA) é causa frequente de abdome agudo cirúrgico. Durante a pandemia de COVID-19, devido às incertezas na evolução da doença, sociedades consideraram outras opções terapêuticas. Nosso objetivo é descrever o tratamento da AA por membros do CBC e SBAIT neste período. O questionário foi enviado em 2020. Houve 382 respostas. A maioria dos profissionais tinha mais de 15 anos de profissão (68,3%) e atendia mais de cinco casos por mês (44,8%). Cerca de 72,5% realizariam TC de tórax para investigação de COVID-19 em pacientes com AA. Nos com AA não complicada, sem COVID-19, 60,2% optariam pela apendicectomia videolaparoscópica (AVL), seguido de apendicectomia aberta (AAB) (31,7%) e tratamento não operatório (TNO) (1,3%). Nos com COVID-19 leve, AAB foi proposta por 51,0%, seguido da AVL (29,6%) e TNO (6,0%). Nos com COVID-19 grave, a AAB foi proposta por 35,3%, seguido de TNO (19,9%) e AVL (18,6%). Nos com AA complicadas com abscesso, sem COVID-19, AVL foi sugerida por 54,2%, seguida da AAB (33,2%) e TNO (4,4%). Nos com COVID-19 leve, a AAB foi proposta em 49,5%, seguidos da AVL (29,3%) e TNO (8,9%). Nos com COVID-19 grave, a AAB foi proposta em 36,6%, seguido de TNO (25,1%) e AVL (17,3%). Estas são opções de cirurgiões de duas sociedades cirúrgicas reconhecidas e podem auxiliar o colega que está na linha de frente a definir a melhor conduta individualmente.

ABSTRACT Acute appendicitis (AA) is a frequent cause of abdominal pain requiring surgical treatment. During the COVID-19 pandemic, surgical societies considered other therapeutic options due to uncertainties in the evolution of the disease. The purpose of this study is to assess the treatment of AA by members of two Brazilian surgical societies in this period. A common questionnaire was sent in 2020. There were 382 responses. Most surgeons had more than 15 years of profession (68.3%) and treated more than five cases per month (44.8%). About 72.5% would indicate chest CT to investigate COVID-19 in patients with AA. For those patients sustaining uncomplicated AA, without COVID-19, 60.2% would indicate laparoscopic appendectomy (VLA), followed by open appendectomy (OA) (31.7%) and non-operative management (NOM) (1.3%). For those with mild COVID-19, OA was suggested by 51.0%, followed by VLA (29.6%) and NOM (6.0%). For those with severe COVID-19, OA was proposed by 35.3%, followed by NOM (19.9%) and VLA (18.6%). For patients with periappendiceal abscesses, without COVID-19, VLA was suggested by 54.2%, followed by OA (33.2%) and NOM (4.4%). For those with mild COVID-19, OA was proposed in 49.5%, followed by VLA (29.3%) and NOM (8.9%). In those with severe COVID-19, OA was proposed in 36.6%, followed by NOM (25.1%) and VLA (17.3%). This information, based on two recognized Brazilian surgical societies, can help the surgeon to select the best approach individually.

RESUMO Em dezembro de 2019, em Wuhan na China, foram descritos os primeiros casos do que seria conhecida como a COVID-19, doença causado por um RNA vírus denominado SARS-CoV-2. A disseminação foi rápida e ampla, levando a Organização Mundial de Saúde a decretar pandemia em março de 2020. A doença tem apresentação clínica variada, desde portadores assintomáticos até casos críticos, com alta letalidade. Paralelamente a isto, pacientes com urgências cirúrgicas não traumáticos, como apendicites agudas e colecistites agudas, continuam a ser atendidos nos serviços de emergências. Neste contexto, surgiram várias dúvidas sobre a conduta nestes casos, entre essas: como identificar rapidamente o paciente com COVID-19, qual o impacto da doença cirúrgica abdominal e o tratamento na evolução dos pacientes com COVID-19, além da discussão sobre o emprego de tratamento não operatório para a doença abdominal nestas circunstâncias. Nesta revisão, trazemos a discussão destes problemas sob a luz das evidências disponíveis.

ABSTRACT In December 2019, in Wuhan, China, the first cases of what would be known as COVID-19, a disease caused by an RNA virus called SARS-CoV-2, were described. Its spread was rapid and wide, leading the World Health Organization to declare a pandemic in March 2020. The disease has distinct clinical presentations, from asymptomatic to critical cases, with high lethality. Parallel to this, patients with non-traumatic surgical emergencies, such as acute appendicitis and cholecystitis, continue to be treated at the emergency services. In this regard, there were several doubts on how to approach these cases, among them: how to quickly identify the patient with COVID-19, what is the impact of the abdominal surgical disease and its treatment on the evolution of patients with COVID-19, in addition to the discussion about the role of the non-operative treatment for abdominal disease under these circumstances. In this review, we discuss these problems based on the available evidence.

RESUMO Objetivo: identificar, baseados em critérios clínicos, grupo de vítimas de trauma fechado com baixa probabilidade de apresentar fraturas na radiografia simples de pelve à admissão (RXP). Método: análise retrospectiva dos dados de registro de trauma em um período de 24 meses. Foram selecionados adultos vítimas de trauma fechado que realizaram RXP à admissão. A frequência de fraturas de pelve foi calculada nos seguintes grupos: exame neurológico normal à admissão (ExNN), estabilidade hemodinâmica (EH), exame da pelve normal à admissão (ExPN), idade inferior a 60 anos (ID<60) e ausência de lesões distrativas (ALD). Estas variáveis foram sobrepostas, na tentativa de identificar um grupo com a menor frequência de fraturas de pelve. Por meio de regressão logística, foi criado modelo preditivo de “ausência” de fraturas de pelve. Resultados: foram identificados 101 (3,3%) RXP positivos dentre os 3.055 realizados. Nos 1.863 pacientes com ExNN, identificamos 39 RXP alteradas (2,1%). Nos 1.535 com ExNN e EH, observaram-se 28 RXP alteradas (1,8%). Nos 1.506 com ExNN, EH e ExPN, identificamos 21 com RXP positiva (1,4%). Dos 1.202 com ExNN, EH, ExPN e ID<60, 11 tinham RXP alteradas (0,9%). Dos 502 com ExNN, EH, ExPN, ID<60 e ALD, houve apenas 2 RXP anormais (0,4%). O modelo preditivo derivado da regressão logística, apresentou área sob a curva ROC (AUC) de 0,89. Conclusões: é possível identificar grupo vítimas de trauma fechado com probabilidade muito baixa de apresentar fraturas pélvicas com base em critérios clínicos. A necessidade de RXP neste grupo deve ser revista.

ABSTRACT Objective: to identify a subgroup of blunt trauma patients with very low chance of sustaining pelvic fractures based on clinical criteria. Methods: retrospective analysis of the trauma registry data, collected in a period of 24 months. We selected adult blunt trauma patients who had a PXR on admission. The frequency of pelvic fractures was calculated for the following groups: Normal neurological examination at admission (NNE), hemodynamical stability (HS), normal pelvic examination at admission (NPE), less than 60 years old (ID<60) and absence of distracting injuries (ADI). Logistic regression analysis was carried out in order to create a probability model of negative PXR. Results: an abnormal PXR was identified in 101 (3.3%) out of the 3,055 patients who had undergone a PXR at admission. Out of these, 1,863 sustained a NNE, with 38 positive CXRs (2.0%) in this group. Considering only the 1,535 patients with NNE and HS, we found 28 positive PXRs (1.8%). Out of these, 1,506 have NPE, with 21 abnormal PXRs (1.4%). Of these, 1,202 were younger than 60 y, with 11 positive PXRs (0.9%). By adding all these criteria to the ADI, we found 2 abnormal PXRs in 502 (0.4%) cases. The probability model including all these variables had a 0,89 area under the ROC curve. Conclusions: by adding clinical criteria, it is possible to identify a group of trauma patients with very low chance of sustaining pelvic fractures. The necessity of PXR in these patients needs to be reassessed.

RESUMO Objetivo: analisar a utilidade de “filtros de qualidade” pré-estabelecidos para a identificação de oportunidades de melhora no atendimento ao idoso traumatizado. Método: análise prospectiva dos dados coletados entre 2014-2015 e armazenados em software iTreg (Ecossistemas). Foram revisados vítimas de trauma maiores de 60 anos e filtros de qualidade propostos, baseados naqueles idealizados pela SBAIT em 2013: (F1) Drenagem de hematoma subdural agudo após 4 horas da admissão em pacientes com ECG<9; (F2) transferência da sala de emergência sem via aérea definitiva e ECG<9; (F3) Reintubação em até 48 horas da extubação; (F4) Tempo admissão-laparotomia exploradora maior que 60 min. em pacientes instáveis com foco abdominal; (F5) Reoperação não programada; (F6) Laparotomia após 4 horas da admissão; (F7) Fratura de diáfise de fémur não fixada; (F8) Tratamento não operatório de ferimento abdominal por PAF; (F9) Tempo admissão-tratamento de fraturas expostas de tíbia superior a 6 horas; (F10) Operação após 24 horas da admissão. Foi analisada relação dos filtros com tratamentos, complicações e óbitos, pelo software SPSS, utilizando-se o teste qui-quadrado e Fisher para calcular a relevância estatística. Resultados: dos 92 casos, 36 (39,1%) tiveram complicações e 15 (16,3%) morreram. A frequência da quebra dos filtros foi de 12% em pacientes que sobreviveram e 55,6% nos entre os que faleceram (p=0,005). A incidência de complicações foi de 77,8% (7/9) nos doentes com FQ comprometido, contra 34,9% (28/83) nos não comprometidos (p=0,017). Conclusões: o comprometimento dos filtros de qualidade se relaciona diretamente com a ocorrência de complicações e óbitos em idosos traumatizados.

ABSTRACT Purpose: to trauma quality indicators as a tool to identify opportunities of improvement in elderly trauma patient’s’ treatment. Methods: prospective analysis of data collected between 2014-2015, and stored in the iTreg software (by Ecossistemas). Trauma victims, aged older than 60 years and trauma quality indicators were assessed, based on those supported by SBAIT in 2013: (F1) Acute subdural hematoma drainage after 4 hours from admission, in patients with GCS<9; (F2) emergency room transference without definitive airway and GCS <9; (F3) Re-intubation within 48 hours from extubation; (F4) Admission-laparotomy time greater than 60 min. in hemodynamically uinstable patients with abdominal bleeding; (F5) Unprogrammed reoperation; (F6) Laparotomy after 4 hours from admission; (F7) Unfixed femur diaphyseal fracture; (F8) Non-operative treatment for abdominal gunshot; (F9) Admission-tibial exposure fracture treatment time greater than 6 hours; (F10) Surgery after 24 from admission. The indicators, treatments, adverse effects and deaths were analyzed, using the SPSS software, and the chi-squared and Fisher tests were used to calculate the statistical relevance. Results: from the 92 cases, 36 (39,1%) had complications and 15 (16,3%) died. The adequate use of quality indicator’s were substantially different among those who survived (was of 12%) compared to those who died (55,6%). The incidence of complications was of 77,8% (7/9) in patients with compromised indicators and 34,9% (28/83) in those without (p=0.017). Conclusions: trauma quality indicators are directly related with the occurrence of complications and deaths, in elderly trauma patients.

RESUMO Objetivo: identificar vítimas de trauma fechado de abdome nas quais as lesões intra-abdominais possam ser excluídas por critérios clínicos e por ultrassonografia abdominal completa. Métodos: análise retrospectiva de vítimas de trauma fechado em que se analisou as seguintes variáveis clínicas: estabilidade hemodinâmica, exame neurológico normal à admissão, exame físico do tórax, do abdome e da pelve normais à admissão e ausência de lesões distrativas (Abbreviated Injury Scale >2 em crânio, tórax e/ou extremidades). Em seguida estudou-se o resultado da ultrassonografia no grupo de pacientes com todas as variáveis clínicas avaliadas. Resultados: estudamos 5536 vítimas de trauma fechado. Lesões intra-abdominais com AIS>1 foram identificadas em 144 (2,6%) casos; em pacientes com estabilidade hemodinâmica, estavam presentes em 86 (2%); naqueles com estabilidade hemodinâmica e exame neurológico normal à admissão em 50 (1,8%); nos casos com estabilidade hemodinâmica, exame neurológico e do tórax normais à admissão em 39 (1,5%); em pacientes com estabilidade hemodinâmica e com exame neurológico, do tórax, do abdome e da pelve normais em 12 (0,5%); naqueles com estabilidade hemodinâmica e com exame neurológico, do tórax, do abdome e da pelve normais e ausência de lesões distrativas, em apenas dois (0,1%) pacientes. Nos pacientes com todas as variáveis clínicas, 693 apresentavam ultrassonografia abdominal completa normal e, neste grupo, não foram identificadas lesões intra-abdominais posteriormente. Conclusão: pela somatória de critérios clínicos e ultrassonografia abdominal completa, é possível identificar um grupo de vítimas de trauma fechado com baixa chance de apresentar lesões intra-abdominais significativas.

ABSTRACT Objective: to identify victims of blunt abdominal trauma in which intra-abdominal injuries can be excluded by clinical criteria and by complete abdominal ultrasonography. Methods: retrospective analysis of victims of blunt trauma in which the following clinical variables were analyzed: hemodynamic stability, normal neurologic exam at admission, normal physical exam of the chest at admission, normal abdomen and pelvis physical exam at admission and absence of distracting lesions (Abbreviated Injury Scale >2 at skull, thorax and/or extremities). The ultrasound results were then studied in the group of patients with all clinical variables evaluated. Results: we studied 5536 victims of blunt trauma. Intra-abdominal lesions with AIS>1 were identified in 144 (2.6%); in patients with hemodynamic stability they were present in 86 (2%); in those with hemodynamic stability and normal neurological exam at admission in 50 (1.8%); in patients with hemodynamic stability and normal neurological and chest physical exam at admission, in 39 (1.5%); in those with hemodynamic stability, normal neurological, chest, abdominal and pelvic physical exam at admission, in 12 (0.5%); in patients with hemodynamic stability, normal neurological, chest, abdominal and pelvic physical exam at admission, and absence of distracting lesions, only two (0.1%) had intra-abdominal lesions. Among those with all clinical variables, 693 had normal total abdominal ultrasound, and, within this group, there were no identified intra-abdominal lesions. Conclusion: when all clinical criteria and total abdominal ultrasound are associated, it is possible to identify a group of victims of blunt trauma with low chance of significant intra-abdominal lesions.

RESUMO Objetivo: analisar a correlação do mecanismo de trauma com a frequência e a gravidade das lesões. Métodos: análise retrospectiva das informações do registro de trauma em período de 15 meses. O mecanismo de trauma foi classificado em seis tipos: ocupantes de veículo de quadro rodas envolvidos em acidente de tráfego (AUTO), pedestres vítimas de atropelamento (ATRO), motociclistas vítimas de acidentes de tráfego (MOTO), vítimas de quedas de altura (QUED), vítimas de agressão física com instrumentos contundentes (AGRE) e vítimas de queda do mesmo nível (QMN). Resultados: o mecanismo de trauma foi classificado em 3639 casos, sendo 337 (9,3%) AUTO, 855 (23,5%) ATRO, 924 (25,4%) MOTO, 455 (12,5%) QUED, 424 (11,7%) AGRE e 644 (17,7%) QMN. Houve diferença significativa na comparação entre os grupos das médias dos índices do Revised Trauma Score (RTS), do Injury Severity Score (ISS) e da Abbreviated Injury Scale (AIS) do segmento cefálico, torácico, abdominal e extremidades (p<0,05). Lesões graves em segmento cefálico foram mais frequentes nas vítimas de ATRO, seguidos de AGRE e QUED (p<0,001). Lesões graves em tórax foram mais frequentes em AUTO, seguidos de QUED e ATRO (p<0,001). As lesões abdominais foram menos frequentes nas vítimas de QMN (p=0,004). Lesões graves em extremidades foram mais frequentes em ATRO, seguidos de MOTO e QUED (p<0,001). Conclusão: com a análise do mecanismo de trauma é possível prever a frequência e a gravidade das lesões em vítimas de trauma fechado.

ABSTRACT Objective: to study the correlation of trauma mechanism with frequency and severity of injuries in blunt trauma patients. Methods: retrospective analysis of trauma registry in a 15-month period was carried out. Trauma mechanism was classified into six types: occupants of four-wheeled vehicles involved in road traffic accidents (AUTO), pedestrians struck by road vehicles (PED), motorcyclists involved in road traffic accidents (MOTO), falls from height (FALL), physical assault with blunt instruments (ASSA) and falls on same level (FSL). Injuries with AIS>2 were considered severe. One-way ANOVA, Students t and Chi-square tests were used for statistical analysis, considering p<0.05 significant. Results: trauma mechanism was classified by group for 3639 cases, comprising 337 (9.3%) AUTO, 855 (23.5%) PED, 924 (25.4%) MOTO, 455 (12.5%) FALL, 424 (11.7%) ASSA and 644 (17.7%) FSL. There was significant difference among groups when comparing the Revised Trauma Score (RTS), the Injury Severity Score (ISS) and the Abbreviated Injury Scale (AIS) of the head, thorax, abdomen and extremities (p<0.001). Severe injuries in the head and in the extremities were more frequent in PED patients (p<0.001). Severe injuries to the chest were more frequent in AUTO (p<0.001). Abdominal injuries were less frequent in FSL (p=0.004). Complex fractures of the pelvis and spine were more frequent in FALL (p<0.001). Lethality was greater in PED, followed by FALL and AUTO (p<0.001). Conclusion: trauma mechanism analysis predicted frequency and severity of injuries in blunt trauma patients.

Objetivo: avaliar os indicadores de lesões intra-abdominais em vítimas de trauma fechado admitidas sem dor abdominal ou alterações no exame físico do abdome. Método: estudo retrospectivo das vítimas de trauma fechado com idade superior a 13 anos, admitidas no período de 2008-2010. Selecionamos para estudo todos que foram submetidos à tomografia computadorizada de abdome e/ou laparotomia exploradora e que, à admissão, não apresentavam dor abdominal ou alterações ao exame físico do abdome. Os doentes foram separados em: Grupo 1 (com lesões intra-abdominais) e Grupo 2 (sem lesões intra-abdominais). As variáveis foram comparadas entre os grupos, considerando p<0,05 como significativo. Em um segundo passo, selecionamos as variáveis com p<0,20 na análise bivariada para criar modelo de regressão logística pelo método forward stepwise. Resultados: foram incluídos 268 casos. Os doentes com lesão abdominal caracterizaram-se por apresentar, significativamente (p<0,05), menor média de AIS em segmento cefálico (1,0 ± 1,4 vs. 1,8 ± 1,9), bem como, maior média de AIS em tórax (1,6 ± 1,7 vs. 0,9 ± 1,5) e de ISS (25,7 ± 14,5 vs. 17,1 ± 13,1). A frequência de lesões abdominais foi significativamente maior nas vítimas de atropelamentos (37,3%) e motociclistas (36%) (p<0,001). A regressão logística construiu um modelo utilizando as seguintes variáveis: motociclista como mecanismo de trauma (p<0,001- OR=5,51; IC95% 2,40-12,64), presença de fraturas de costelas (p<0,003 - OR=3,00; IC95% 1,47-6,14), atropelamento como mecanismo de trauma (p=0,008 - OR=2,85; IC95% 1,13-6,22) e exame físico neurológico anormal a admissão (p=0,015 - OR=0,44; IC95% 0,22-0,85). Conclusão: as lesões intra-abdominais foram relacionadas principalmente com o mecanismo de trauma e a presença de lesões torácicas.

Objective: to assess predictors of intra-abdominal injuries in blunt trauma patients admitted without abdominal pain or abnormalities on the abdomen physical examination. Methods: We conducted a retrospective analysis of trauma registry data, including adult blunt trauma patients admitted from 2008 to 2010 who sustained no abdominal pain or abnormalities on physical examination of the abdomen at admission and were submitted to computed tomography of the abdomen and/or exploratory laparotomy. Patients were assigned into: Group 1 (with intra-abdominal injuries) or Group 2 (without intra-abdominal injuries). Variables were compared between groups to identify those significantly associated with the presence of intra-abdominal injuries, adopting p<0.05 as significant. Subsequently, the variables with p<0.20 on bivariate analysis were selected to create a logistic regression model using the forward stepwise method. Results: A total of 268 cases met the inclusion criteria. Patients in Group I were characterized as having significantly (p<0.05) lower mean AIS score for the head segment (1.0±1.4 vs. 1.8±1.9), as well as higher mean AIS thorax score (1.6±1.7 vs. 0.9±1.5) and ISS (25.7±14.5 vs. 17,1±13,1). The rate of abdominal injuries was significantly higher in run-over pedestrians (37.3%) and in motorcyclists (36.0%) (p<0.001). The resultant logistic regression model provided 73.5% accuracy for identifying abdominal injuries. The variables included were: motorcyclist accident as trauma mechanism (p<0.001 - OR 5.51; 95%CI 2.40-12.64), presence of rib fractures (p<0.003 - OR 3.00; 95%CI 1.47-6.14), run-over pedestrian as trauma mechanism (p=0.008 - OR 2.85; 95%CI 1.13-6.22) and abnormal neurological physical exam at admission (p=0.015 - OR 0.44; 95%CI 0.22-0.85). Conclusion Intra-abdominal injuries were predominantly associated with trauma mechanism and presence of chest injuries.

RESUMOObjetivo:comparar, entre vítimas de atropelamento e de outros mecanismos de trauma fechado, a frequência e gravidade das lesões identificadas.Métodos:análise retrospectiva das informações do registro de trauma, incluindo as vítimas de trauma fechado, com idade superior a 13 anos de idade, admitidas entre 2008-2010. Avaliamos o mecanismo de trauma, dados vitais à admissão e lesões diagnosticadas. A estratificação de gravidade da amostra foi realizada pelos índices RTS, AIS-90, ISS e TRISS. As vítimas de trauma fechado foram separadas em dois grupos: Grupo A- pedestres vítimas de atropelamento; Grupo B- vítimas dos demais mecanismos de trauma fechado. As variáveis foram comparadas entre os dois grupos.Resultados:foram incluídos no estudo 5785 casos, sendo que, 1217 (21,0%) foram vítimas de atropelamento. Observamos que os traumatizados do grupo A apresentaram, significativamente (p<0,05), maior média etária, de frequência cardíaca à admissão, de ISS, de AIS no segmento cefálico, torácico, abdominal e em extremidades, bem como, menor média de escala de coma de Glasgow, pressão arterial sistólica a admissão, RTS e TRISS. As vítimas de atropelamento também apresentaram (p<0,05), maior frequência de hematomas extradurais, hematomas subdurais agudos, hemorragia subaracnoidea, Brain Swelling, contusão cerebral, lesão axonal difusa, fraturas de arcos costais, pneumotórax, tórax flácido, contusão pulmonar, bem como, fraturas de pelve, de membros superiores, inferiores e expostas de membros inferiores.Conclusão:as vítimas de atropelamento apresentam maior frequência e gravidade de lesões intracranianas, torácicas, abdominais e em extremidades quando comparadas às vítimas de outros mecanismos de trauma fechado em conjunto.

ABSTRACTObjective:to compare the frequency and the severity of diagnosed injuries between pedestrians struck by motor vehicles and victims of other blunt trauma mechanisms.Methods:retrospective analysis of data from the Trauma Registry, including adult blunt trauma patients admitted from 2008 to 2010. We reviewed the mechanism of trauma, vital signs on admission and the injuries identified. Severity stratification was carried using RTS, AIS-90, ISS e TRISS. Patients were assigned into group A (pedestrians struck by motor vehicle) or B (victims of other mechanisms of blunt trauma). Variables were compared between groups. We considered p<0.05 as significant.Results:a total of 5785 cases were included, and 1217 (21,0%) of which were in group A. Pedestrians struck by vehicles presented (p<0.05) higher mean age, mean heart rate upon admission, mean ISS and mean AIS in head, thorax, abdomen and extremities, as well as lower mean Glasgow coma scale, arterial blood pressure upon admission, RTS and TRISS. They also had a higher frequency of epidural hematomas, subdural hematomas, subarachnoid hemorrhage, brain swelling, cerebral contusions, costal fractures, pneumothorax, flail chest, pulmonary contusions, as well as pelvic, superior limbs and inferior limbs fractures.Conclusion:pedestrian struck by vehicles sustained intracranial, thoracic, abdominal and extremity injuries more frequently than victims of other blunt trauma mechanism as a group. They also presented worse physiologic and anatomic severity of the trauma.

RESUMOObjetivo:analisar a implantação de registro de trauma em hospital universitário com atendimento ao SUS, bem como, sua capacidade em identificar pontos para melhorada qualidade no atendimento.Métodos:o grupo de coleta de dados foi composto por alunos dos cursos de medicina e enfermagem, orientados pelos coordenadores do projeto. Utilizamos o software itreg (ECO Sistemas-RJ/SBAIT) como ferramenta de banco de dados. Vários "filtros" de qualidade foram propostos no intuito de selecionar os casos a serem revistos no processo de controle de qualidade.Resultados:entre março e novembro de 2014, foram inseridos no itreg dados de 1344 vítimas de trauma. Cerca de 87% foram vítimas de trauma fechado, 59,6% apresentaram RTS>7,0 e, 67%, ISS<9. Os registros foram completos em 292 casos, que foram selecionados para a revisãono programa de qualidade. Os filtros de auditoria mais frequentemente anotadosforam a laparotomia após quatro horas da admissão e a drenagem dos hematomas subdurais agudos após quatro horas da admissão. Identificamos vários pontos a serem desenvolvidos, como o controle da "supertriagem" de doentes, a necessidade de diminuição do número de exames de imagem negativos, o desenvolvimento de protocolos para passagem de acessos venosos centrais e tratamento de trauma craniencefálico grave.Conclusão:o Registro de Trauma traz uma visão clara dos pontos a serem melhorados no atendimento ao traumatizado, contudo, há características específicas na implantação desta ferramenta.

ABSTRACTObjective:to analyze the implementation of a trauma registry in a university teaching hospital delivering care under the unified health system (SUS), and its ability to identify points for improvement in the quality of care provided.Methods:the data collection group comprised students from medicine and nursing courses who were holders of FAPESP scholarships (technical training 1) or otherwise, overseen by the coordinators of the project. The itreg (ECO Sistemas-RJ/SBAIT) software was used as the database tool. Several quality "filters" were proposed to select those cases for review in the quality control process.Results:data for 1344 trauma patients were input to the itreg database between March and November 2014. Around 87.0% of cases were blunt trauma patients, 59.6% had RTS>7.0 and 67% ISS<9. Full records were available for 292 cases, which were selected for review in the quality program. The auditing filters most frequently registered were laparotomy four hours after admission and drainage of acute subdural hematomas four hours after admission. Several points for improvement were flagged, such as control of overtriage of patients, the need to reduce the number of negative imaging exams, the development of protocols for achieving central venous access, and management of major TBI.Conclusion: the trauma registry provides a clear picture of the points to be improved in trauma patient care, however, there are specific peculiarities for implementing this tool in the Brazilian milieu.

OBJETIVO:analisar as lesões diagnosticadas nas vítimas de queda de altura, comparando-as com as diagnosticadas em outros mecanismos de trauma fechado.MÉTODOS:estudo retrospectivo dos protocolos de trauma (coletados prospectivamente) de 2008 a 2010, incluindo as vítimas de trauma fechado com idade superior a 13 anos, admitidas na sala de emergência. A gravidade das lesões foi estratificada pelo Abbreviated Injury Scale (AIS) e Injury Severity Score (ISS). As variáveis foram comparadas entre o grupo de vítimas de quedas de altura (Grupo 1) e as demais vítimas de trauma fechado (Grupo 2). Empregamos os testes t de Student, qui-quadrado e Fisher para a comparação entre os grupos, considerando o valor de p<0,05 como significativo.RESULTADOS:foram analisados 4532 casos de trauma fechado, sendo que 555 (12,2%) foram vítimas de quedas de altura. As lesões graves (AISe"3) foram observadas em extremidades (17,5%), em segmento cefálico (8,4%), torácico (5,5%) e em abdome (2,9%). As vítimas do grupo 1 apresentaram, significativamente (p<0,05), maior média etária, de AIS em extremidades/pelve, de AIS em segmento torácico e de ISS. O grupo 1 também apresentou, significativamente (p<0,05), maior frequência de intubação orotraqueal na admissão, pneumotórax, hemotórax, fraturas de costelas, drenagem de tórax, trauma raquimedular, fraturas de pelve, fraturas complexas de pelve e de fraturas em membros superiores.CONCLUSÃO:As vítimas de queda de altura apresentaram maior gravidade anatômica do trauma, maior frequência e gravidade de lesões em segmento torácico e em extremidades.

OBJECTIVE: To analyze the lesions diagnosed in victims of falls, comparing them with those diagnosed in other mechanisms of blunt trauma.METHODS: We conducted a retrospective study of trauma protocol charts (prospectively collected) from 2008 to 2010, including victims of trauma over 13 years of age admitted to the emergency room. The severity of injuries was stratified by the Abbreviated Injury Scale (AIS) and Injury Severity Score (ISS). Variables were compared between the group of victims of falls from height (Group 1) and the other victims of blunt trauma (Group 2). We used the Student t, chi-square and Fisher tests for comparison between groups, considering the value of p <0.05 as significant.RESULTS: The series comprised 4,532 cases of blunt trauma, of which 555 (12.2%) were victims of falls from height. Severe lesions (AISe"3) were observed in the extremities (17.5%), in the cephalic segment (8.4%), chest (5.5%) and the abdomen (2.9%). Victims of Group 1 had significantly higher mean age, AIS in extremities / pelvis, AIS in the thoracic segment and ISS (p <0.05). The group 1 had significantly (p <0.05) higher incidence of tracheal intubation on admission, pneumothorax, hemothorax, rib fractures, chest drainage, spinal trauma, pelvic fractures, complex pelvic fractures and fractures to the upper limbs.CONCLUSION: Victims of fall from height had greater anatomic injury severity, greater frequency and severity of lesions in the thoracic segment and extremities.
