A síndrome do anticorpo antifosfolípide (SAF), mais comum em mulheres, manifesta-se clinicamente como trombose e/ou abortamentos de repetição. Anemia hemolítica autoimune e manifestações neurológicas, cardíacas e cutâneas são comuns. Relata-se o caso de um paciente do gênero masculino cuja manifestação inicial da doença foi gangrena em pavilhão auricular, e o diagnóstico de SAF se deu por meio de biópsia de pele do membro inferior, que mostrava vasculopatia trombótica, sem evidência de vasculite. Esse resultado é um dos dois critérios maiores que, associados a um critério menor, fecham o diagnóstico dessa doença. Discutem-se neste caso os possíveis diagnósticos diferenciais e como eles se diferenciam da doença em foco, além da importância que a biópsia teve no diagnóstico de SAF nesse indivíduo.

Antiphospholipid syndrome (APS), more common in females, manifests clinically as thrombosis and/or recurrent fetal loss. Hemolytic autoimmune anemia and neurological, cardiac and cutaneous manifestations are common. This is the case report of a male patient whose first manifestation of the disease was gangrene of the auricle. The diagnosis of APS was established by biopsy of the lower limb skin, which showed thrombotic vasculopathy with no evidence of vasculitis. This is one of the two major criteria, which, along with a minor criterion, establishes the diagnosis of APS. Possible differential diagnoses are discussed. The importance of the biopsy in the APS diagnosis of this male patient is emphasized.

OBJETIVO: Investigar a correlação de fadiga com dor em pacientes com artrite reumatoide e de fadiga com incapacidade em pacientes com osteoartrite. MÉTODOS: Foram avaliados 20 pacientes com artrite reumatoide e 20 com osteoartrite. Fadiga foi avaliada com escala visual analógica e questionário de avaliação multidimensional de fadiga. Utilizou-se a escala visual analógica para avaliar dor e para a avaliação global da doença pelo paciente. Incapacidade foi avaliada pelo questionário de avaliação de saúde. Idade, gênero, duração da doença, escolaridade, renda mensal, uso de drogas antirreumáticas e comorbidades foram obtidos. A análise estatística incluiu teste exato de Fisher, Shapiro-Wilk, Kruskal-Wallis e Spearman. O nível de significância foi de 0,05. RESULTADOS: A fadiga mostrou-se significativamente aumentada na osteoartrite em comparação à artrite reumatoide quando se utilizou o questionário de avaliação multidimensional de fadiga (P < 0,05). Dor correlacionou-se com fadiga ao se utilizar a escala visual analógica ou o questionário de avaliação multidimensional de fadiga em artrite reumatoide (r = 0,46; P < 0,05). O questionário de avaliação de saúde foi associado à fadiga por escala visual analógica em osteoartrite (r = 0,54; P < 0,05). Avaliação global da doença pelo paciente correlacionou-se com fadiga pela escala visual analógica (r = 0,44; P < 0,003). Todas as pacientes eram mulheres, predominantemente idosas, com médias de idade semelhantes, longa duração de doença e baixa renda. CONCLUSÃO: Nossos resultados confirmam que em pacientes com artrite reumatoide a fadiga correlaciona-se com dor, enquanto em pacientes com osteoartrite ela associa-se à incapacidade. Portanto, fadiga tem diferentes correlatos em osteoartrite e artrite reumatoide. Sugerimos que incapacidade, e não dor, seja o correlato da fadiga em pacientes com osteoartrite.

OBJECTIVE: To investigate the correlation of fatigue with pain in rheumatoid arthritis patients and with disability in osteoarthritis patients. METHODS: Twenty patients with rheumatoid arthritis and 20 patients with osteoarthritis were evaluated. The degree of fatigue was evaluated with a visual analogue scale and the Multidimensional Assessment of Fatigue. Pain was evaluated with a visual analogue scale as well as Patient Global Assessment. For disability evaluation, the Health Assessment Questionnaire was performed. Age, gender, disease duration, education, income, antirheumatic drugs used and comorbidity were also obtained. Statistical analysis included Fisher exact, Shapiro-Wilk, Kruskal-Wallis and Spearman tests. The significance level was 0.05. RESULTS: Fatigue was more significantly increased in patients with osteoarthritis than in patients with rheumatoid arthritis when evaluated with Multidimensional Assessment of Fatigue (P < 0.05). Pain was found to correlate with fatigue evaluated with visual analogue scale or Multidimensional Assessment of Fatigue in patients with rheumatoid arthritis (r = 0.46; P < 0.05). Health Assessment Questionnaire was associated with fatigue visual analogue scale in patients with osteoarthritis (r = 0.54; P < 0.05). Patient Global Assessment correlates with fatigue visual analogue scale (r = 0.44; P < 0.003). Patients were similar in both groups: all females, similar mean age, with long disease duration and low income. CONCLUSION: Our results corroborate that fatigue in rheumatoid arthritis patients correlates with the degree of pain, while in osteoarthritis patients it is associated with disability. Therefore, we found that fatigue has different correlates in osteoarthritis and rheumatoid arthritis, and we suggest that disability, not pain, is a correlate of fatigue in osteoarthritis patients.

OBJETIVO: comparar as características clínicas e o perfil dos autoanticorpos entre o lúpus eritematoso crônico discóide (LECD) e o lúpus eritematoso sistêmico (LES), com o intuito de identificar algum marcador associado ao envolvimento sistêmico no LECD. MÉTODOS: foram estudados 16 pacientes com LECD e comparados a 17 pacientes com LES, segundo os critérios do Colégio Americano de Reumatologia (ACR). Foram avaliadas as características demográficas, clínicas e pesquisados os seguintes autoanticorpos séricos, como fator antinúcleo (FAN), anti-DNA, anti-Sm, anti-Ro/SS-A, anti-La/SS-B, anti-ENA/RNP e anticardiolipina IgG e IgM. Nos pacientes com LECD foi realizada biópsia da pele e estudo histopatológico. Para comparação entre LES e LECD foi aplicado o teste do qui-quadrado. Foi considerado significativo p<0,05. RESULTADOS: pacientes com LECD tiveram idade e duração da doença significantemente mais longa do que os pacientes com LES (p<0,04). Não houve diferenças com relação ao número de critérios de diagnóstico de LES entre pacientes com LES e LECD. Entre os parâmetros clínicos, observou-se associação da hipertensão arterial com a forma sistêmica de lúpus eritematoso (p<0,03). Entre os autoanticorpos pesquisados somente o autoanticorpo anti-DNA mostrou estar significantemente associado ao diagnóstico de LES (p<0,04). CONCLUSÕES: a análise dos dados clínicos e dos autoanticorpos no soro dos pacientes com LES e LECD identificaram hipertensão arterial e presença de anti-DNA no soro como os parâmetros que se mostraram associados ao envolvimento sistêmico no lúpus eritematoso.

OBJECTIVE: to assess the clinical characteristics of CDLE, its interrelationship with SLE as well as the significance of some serum autoantibodies as markers of systemic involvement in CDLE. METHODS: sixteen patients with CDLE were compared to seventeen patients with SLE according to the ACR criteria. Clinical and demographics characteristics and serum autoantibodies like antinuclear antibodies (ANA), antibodies to dsDNA, Sm, Ro/SS-A, La/SS-B, ENA/RNP, and anticardiolipin antibodies IgG and IgM were determined. The statistical evaluation was carried out using Chi-square test. A probability level of =0.05 was considered significant. RESULTS: age and disease duration in CDLE patients were significantly longer than in patients with SLE (p<0.04). No difference was found regarding the number of ACR diagnostic and classification criteria between CDLE and SLE patients. Arterial hypertension was clearly associated with sistemic involvement (p<0.03). Only antibody to dsDNA showed to be significantly associated with SLE diagnosis (p<0.04). CONCLUSIONS: the clinical and laboratorial evaluation of CDLE and SLE patients showed that arterial hypertension and serum antibody to dsDNA were the laboratorial and clinical parameters associated to systemic involvement in CDLE.
