OBJETIVO: Comparar a produção do fonema /r/ em crianças com desvio fonológico (DF), e as alterações no sistema estomatognático (SEG) considerando as variáveis de idade, gênero e gravidade do DF. Métodos: A amostra constituiu-se de 113 crianças com DF e alterações do SEG, de ambos os gêneros, e idades entre 4 anos e 8 anos e 11 meses, divididos em dois grupos, considerando a faixa etária: pré-escolar (4 anos a 5 anos e 11 meses) e escolar (6 anos a 8 anos e 11 meses). Realizou-se Avaliação Fonológica da Criança e avaliação do SEG. A amostra foi dividida, quanto à gravidade do DF, em dois grupos, Graus mais Graves (GG) e Graus mais Leves (GL). Quanto ao sistema fonológico (SF), a mesma foi dividida em GI: /r/ estabelecido; e GII: /r/ não estabelecido. Os dados coletados foram tabulados, analisados e, posteriormente, submetidos a tratamento estatístico. RESULTADOS: Não foi observada diferença na postura, tonicidade e mobilidade de língua, lábios e bochechas, bem como nas estruturas e funções do SEG entre GI e GII. O GI obteve melhor desempenho apenas na vibração de língua. Verificou-se relação entre a aquisição do fonema /r/ e o aumento da idade, não havendo correlação com o gênero. Ainda, os GG de DF são mais frequentes no GII, e os GL no GI. CONCLUSÃO: Não há relação entre as alterações do SF e das estruturas e funções do SEG em crianças com DF, havendo relação deste apenas com a idade das mesmas.

PURPOSE: To compare the production of the phoneme /r/ in children with phonological disorder (PD) and the stomatognathic system (SS) alterations considering the variables age, gender, and severity of the PD. METHODS: The sample was composed by 113 subjects with PD and SS alterations, of both genders, and with ages between 4 years and 8 years and 11 months, divided into two groups, considering the age range: pre-school age (4 years to 5 years and 11 months), and school age (6 years to 8 years and 11 months). The Child's Phonological Assessment and a stomatognathic system evaluation were carried out. The subjects were divided into two groups, according to the severity of PD: More Severe Degrees (MSD) and Milder Degrees (MD). Considering the phonological system, subjects were divided into GI: /r/ established, and GII: /r/ not established. The collected data were tabulated, analyzed and submitted to statistical analysis. RESULTS: No differences were observed between GI and GII regarding tongue, lips and cheeks position, tonicity and mobility, as well as in SS structures and functions. GI obtained better performance only in tongue vibration. It was verified a correlation between the acquisition of phoneme /r/ and age increase; no correlation was found between the acquisition of /r/ and gender. MSD of PD were more frequent in GII, and MD in GI. CONCLUSION: There is no relation between alterations in the PS and in structures and functions of the SS in children with PD; PD is related only with children's age.

OBJETIVO: comparar o desempenho em prova específica de vocabulário de crianças com e sem desvio fonológico. MÉTODOS: participaram da pesquisa 150 crianças, 75 do Grupo com Desvio Fonológico (GDF) e 75 do Grupo com Desenvolvimento Fonológico Normal (GDFN), entre 6:0 e 6:11, de ambos os gêneros, pertencentes ao nível socioeconômico médio. Realizaram-se as triagens fonoaudiológica e auditiva, a Avaliação Fonológica da Criança e a avaliação do Vocabulário. As respostas das crianças foram analisadas considerando o número de Designações Verbais Usuais (DVU), Não Designações (ND) e Processos de Substituição (PS), por campo conceitual. Posteriormente, os dados foram tabulados e submetidos à análise estatística através do Teste T para amostras independentes, considerando-se p<0,05. RESULTADOS: verificou-se que o GDF obteve menor número de DVU que o GDFN, sendo esta diferença estatisticamente significante para todos os campos conceituais. Além disso, a ocorrência de PS e ND foi maior para o GDF, em todos os campos conceituais, havendo diferença estatisticamente significante respectivamente, para oito e quatro campos conceituais. Ainda, a ocorrência de PS foi maior que a de ND para ambos os grupos. CONCLUSÃO: o GDF apresentou desempenho inferior na prova do vocabulário, verificado pela menor ocorrência de DVU e maior de ND e PS que o GDFN. Esses achados mostram haver relação entre desvio fonológico e déficit no vocabulário. Assim, é importante avaliar o vocabulário das crianças com desvio fonológico para auxiliar na terapia.

PURPOSE: to compare the performance in vocabulary test of children with or without phonological disorder. METHODS: the study involved 150 children, 75 of the Phonological Disorder Group (PDG) and 75 of the Typical Phonological Developmental Group (TPDG), with age group ranging from 6:0 to 6:11 years, of both genders, with medium socioeconomic level. Auditory and speech-language screening were accomplished, and after that the children were submitted to the Child’s Phonological Assessment and a Vocabulary Test. The answers were analyzed considering the number of Usual Verbal Designations (UVD), No Designations (ND) and Processes of Substitutions (PS) by conceptual field. Finally, data were tabulated and submitted to statistical analysis using the T-Test for independent samples and the significance level was considered 5%. RESULTS: PDG has obtained a smaller number of UVD that the TPDG, being difference statistically significant for all conceptual fields. Besides this, the occurrence of ND e PS were higher for PDG in all of conceptual fields, being the difference statistically significant for four and eight conceptual fields, respectively. Still, the occurrences of PS were larger than the ND for the two groups. CONCLUSION: the results indicated that PDG showed lower performance in the vocabulary test, and it were verified by the smaller occurrence of UVD and larger of ND and PS than the TPDG. These findings showed that have a relation between the phonological disorder and vocabulary deficit. So, it is important to evaluates the vocabulary of the children with phonological disorders in order to assist the therapy.

TEMA: terapia fonológica. OBJETIVO: avaliar a eficácia do tratamento em três diferentes modelos de terapia quanto às mudanças no sistema fonológico de sujeitos com diferentes gravidades do Desvio Fonológico (DF). MÉTODO: a amostra constituiu-se de 66 sujeitos, com idades entre 4:4 e 8:2, sendo 43 do sexo masculino e 23 do feminino, integrantes do banco de dados de uma clínica escola. Todos foram avaliados, antes e após um período de 15 a 25 sessões de terapia fonológica, utilizando-se a Avaliação Fonológica da Criança, a partir da qual foi determinada a gravidade do DF conforme o Percentual de Consoantes Corretas - PCC, o número de Segmentos Não Adquiridos - SNA , e o percentual de Segmentos Adquiridos (SA) após o período de tratamento. Os sujeitos foram tratados pelos modelos ABAB-Retirada e Provas Múltiplas, Oposições Máximas Modificado e Ciclos Modificado. Posteriormente, realizou-se análise estatística dos dados, utilizando o Teste T para amostras iguais, considerando-se p < 0,05. RESULTADOS: verificou-se um aumento do PCC e do percentual de SA, bem como redução do número de SNA em todos os grupos tratados pelos diferentes modelos de terapia. Estes resultados foram estatisticamente significativos para a maioria dos grupos. CONCLUSÃO: os resultados permitem afirmar que os três modelos de terapia aplicados foram eficazes no tratamento de crianças com DF, para as diferentes gravidades do desvio. Além disso, as maiores mudanças no sistema fonológico ocorreram para os grupos com DF de grau mais acentuado.

BACKGROUND: phonological therapy. AIM: to evaluate the effectiveness of three different therapy models regarding changes in the phonological system of subjects with different phonological disorder (PD) severity levels. METHOD: the research group was composed by 66 subjects, 43 males and 23 females, with ages between 4:4 and 8:2 years, who were part of a school clinic database. All subjects were evaluated, before and after a period of 15 to 25 sessions of phonological therapy, using the Child's Phonological Assessment. Based on this assessment, the severity level of the PD was determined using the Percentage Consonants Correct - PCC, and by calculating the number of Non-Acquired Segments -NAS and the percentage of Acquired Segments (AS) after treatment. The subjects were treated using the following approaches: ABAB-Withdrawal and Multiple Probes, Modified Maximum Opposition and Modified Cycles Models. For the statistical analysis the T-Test was used with a significance level of 5%. RESULTS: the statistical analysis indicated an increase in the PCC and in the percentage of AS, as well as a reduction in the number of NAS for all groups. These results were statistically significant for the majority of the groups. CONCLUSION: the results indicate that the three therapy approaches were effective in the treatment of children with PD, and were effective in treating different severity levels. Besides that, most of the changes occurred in the phonological system of the groups with more severe PD.
