Resumo Trata-se da tradução e adaptação de uma intervenção baseada no Modelo Transteórico de Mudança e direcionada para pacientes com risco cardiovascular. Inicialmente, o manual original com 29 sessões foi traduzido para a língua portuguesa, sendo ampliado para 36 sessões, e posteriormente aplicado em um estudo piloto. A partir de agrupamentos pelos processos de mudança, foi reduzido para 12 sessões. Evidencia-se como produtos deste trabalho o aprofundamento dos processos em cada comportamento, a versatilidade do protocolo, o trabalho interdisciplinar, o ótimo custo-benefício e o potencial de efetividade. Por fim, salienta-se a importância de um treinamento consistente em Entrevista Motivacional, Modelo Transteórico e manejo de grupos para sua utilização adequada, e discute-se os custos e os benefícios dos protocolos versus tratamentos mais flexíveis.

Abstract The present study is the translation and adaptation of an intervention based on the Transtheoretical Model of Change directed to patients at cardiovascular risk. The original 29-session manual was translated into Portuguese, extended to 36 sessions, and applied in a pilot study. The protocol was reduced to 12 sessions through the grouping based on the processes of change. The products of the present study were the processes in each behavior, the versatility of the protocol, the interdisciplinary work, the great cost-benefit, and the potential of effectiveness. Finally, the importance of consistent training in Motivational Interview, Transtheoretical Model and group management for proper use are highlighted, and the costs and benefits of protocols versus more flexible treatments are discussed.

RESUMO O avanço científico e tecnológico associado à atuação de equipes multidisciplinares nas unidades de terapia intensiva tem aumentado a sobrevida de pacientes críticos. Dentre os recursos de suporte de vida utilizados em terapia intensiva, está a oxigenação por membrana extracorpórea. Apesar das evidências aumentarem, faltam dados para demonstrar a segurança e os benefícios da fisioterapia concomitante ao uso da oxigenação por membrana extracorpórea. Esta revisão reúne as informações disponíveis sobre a repercussão clínica da fisioterapia em adultos submetidos à oxigenação por membrana extracorpórea. A revisão incluiu as bases MEDLINE®, PEDro, Cochrane CENTRAL, LILACS e EMBASE, além da busca manual nas referências dos artigos relacionados até setembro de 2017. A busca resultou em 1.213 registros. Vinte estudos foram incluídos, fornecendo dados de 317 indivíduos (58 no grupo controle). Doze estudos não relataram complicações durante a fisioterapia. Fratura da cânula durante a deambulação, trombo na cânula de retorno e hematoma na perna em um paciente cada foram relatados por dois estudos, dessaturação e vertigens leves foram relatadas em dois estudos. Por outro lado, foram feitos relatos de melhora na condição respiratória/pulmonar, capacidade funcional e força muscular, com redução de perda de massa muscular, incidência de miopatia, tempo de internação e mortalidade dos pacientes que realizaram a fisioterapia. Analisando o conjunto das informações disponíveis, pode-se observar que a fisioterapia, incluindo a mobilização precoce progressiva, ortostase, deambulação e técnicas respiratórias, executada de forma simultânea à oxigenação por membrana extracorpórea, é viável, relativamente segura e potencialmente benéfica para adultos em condição clínica extremamente crítica.

Abstract Scientific and technological advances, coupled with the work of multidisciplinary teams in intensive care units, have increased the survival of critically ill patients. An essential life support resource used in intensive care is extracorporeal membrane oxygenation. Despite the increased number of studies involving critically ill patients, few studies to date have demonstrated the safety and benefits of physical therapy combined with extracorporeal membrane oxygenation support. This review identified the clinical outcomes of physical therapy in adult patients on extracorporeal membrane oxygenation support by searching the MEDLINE®, PEDro, Cochrane CENTRAL, LILACS, and EMBASE databases and by manually searching the references of the articles published until September 2017. The database search retrieved 1,213 studies. Of these studies, 20 were included in this review, with data on 317 subjects (58 in the control group). Twelve studies reported that there were no complications during physical therapy. Cannula fracture during ambulation (one case), thrombus in the return cannula (one case), and leg swelling (one case) were reported in two studies, and desaturation and mild vertigo were reported in two studies. In contrast, improvements in respiratory/pulmonary function, functional capacity, muscle strength (with reduced muscle mass loss), incidence of myopathy, length of hospitalization, and mortality in patients who underwent physical therapy were reported. The analysis of the available data indicates that physical therapy, including early progressive mobilization, standing, ambulation, and breathing techniques, together with extracorporeal membrane oxygenation, is feasible, relatively safe, and potentially beneficial for critically ill adult patients.

RESUMO Racional: Atualmente o estilo de vida expõe a população a diversos fatores de risco relacionados a hábitos alimentares e à inatividade física, contribuindo para o surgimento de doenças crônicas. Objetivo: Analisar a associação entre o cortisol salivar e os componentes da síndrome metabólica. Métodos: Estudo transversal com 28 indivíduos, idade entre 30 e 59 anos apresentando três ou mais dos seguintes achados: circunferência abdominal ≥88 cm (mulheres) e ≥102 cm (homens); pressão arterial sistólica >130 mmHg e pressão arterial diastólica >85 mmHg; glicemia >100 mg/dl; triglicerídeo >150 mg/dl; lipoproteína de alta densidade <40 mg/dl (homens) e <50 mg/dl (mulheres). Foram realizadas coletas do cortisol salivar nos seguintes horários 8 h, 12 h e 20 h e analisadas por radioimunoensaio. Resultados: A média de idade foi 51,9±7,5 anos, 64,3% eram mulheres e a média do índice de massa corporal foi 33,6±3,2 kg/m². O nível de cortisol salivar às 8 h teve média de 18,7±4,8 ng/dl. Os indivíduos com glicemia de jejum >110 mg/dl, apresentaram níveis médios de cortisol significativamente menores que os com glicemia de jejum <110 mg/dl (12,8±5,2 vs. 17,3±4,2). As correlações significativas foram HOMA vs. circunferência abdominal (r=0,465; p˂0,005) e triglicerídeos (r=0,473; p˂0,005), circunferência abdominal vs. glicemia de jejum (r=0,446; p˂0,005) e índice de massa corporal (r=0,730; p˂0,0001), triglicerídeos vs. lipoproteína de alta densidade (r=0,441 p˂0,005) e glicemia de jejum (r=0,440; p˂0,005). Conclusão: O cortisol salivar pela manhã, em indivíduos com glicemia cronicamente elevada, pode representar uma contraregulação do eixo hipotálamo-hipófise-adrenal, sendo achado importante e pouco investigado.

ABSTRACT Background: Actually the lifestyle exposes the population to several risk factors related to alimentary habits and less physical activity that contributes to chronic diseases appearance worldwide. Aim: To analyze the association between salivary cortisol and the components of metabolic syndrome. Methods: This is a cross-sectional study. As part of it, 28 individuals aged 30-59 years presenting three or more of the following findings: CA: ≥88 cm for women and ≥102 cm for men; SBP>130 mmHg and DBP>85 mmHg; GL>100 mg/dl; TG>150 mg/dl; HDL<40 mg/dl for men and <50 mg/dl for women. Was performed analysis of salivary cortisol (by radioimmunoassay) from 25 salivary samples collected throughout the day, for evaluating changes in the circadian rhythm of this hormone (8AM, noon and 8PM). Results: 28 evaluated individuals had a mean age of 51.9±7.5 years, mostly women (64.3%) and a mean of BMI 33.6±3.2 kg/m². The cortisol level from the 8AM averaged 18.7±4.8 ng/dlL. Individuals with FPG>110mg/dl, have significantly lower average levels of cortisol than ones with FPG <110 (12.8±5,2 vs. 17.3±4.2). Significant correlations were HOMA vs. WC (r=0,465; p˂0,005) and TG (r=0,473; p˂0,005), WC vs. FG (r=0,446; p˂0,005) and BMI (r=0,730; p˂0.0001); TG vs. HDL (r=0,441 p˂0,005) and FG (r=0,440; p˂0,005). Conclusion: Morning salivary cortisol in subjects with chronically elevated blood glucose can represent a downregulation of the hypothalamic-pituitary adrenal axis. This is an important finding not yet well investigated.

RESUMO Objetivo: Revisar na literatura os efeitos da manobra de compressão torácica expiratória sobre a mecânica ventilatória, a desobstrução brônquica, e os índices de oxigenação e hemodinâmica de pacientes adultos ventilados mecanicamente. Métodos: Foi realizada uma revisão sistemática com metanálise de ensaios clínicos randomizados nas bases de dados MEDLINE (via PubMed), EMBASE, Cochrane CENTRAL, PEDro e LILACS. Foram incluídos estudos com pacientes adultos, internados em unidades de terapia intensiva, ventilados mecanicamente, que comparavam os efeitos da manobra de compressão torácica expiratória com grupo controle (sem manobra de compressão torácica expiratória) e que avaliaram os seguintes desfechos: complacência estática e dinâmica, volume de secreção depurado, pressão arterial sistólica, pressão arterial diastólica, pressão arterial média, frequência cardíaca, saturação periférica de oxigênio e relação entre pressão arterial de oxigênio e fração inspirada de oxigênio. Foram excluídos estudos experimentais com animais e estudos com dados incompletos. Resultados: A estratégia de busca resultou em 5.816 estudos, sendo incluídos apenas três estudos randomizados com crossover, totalizando 93 pacientes. No desfecho de frequência cardíaca, observou-se redução a favor da manobra de compressão torácica expiratória, comparada com o grupo controle [-2,81bpm (IC95%: -4,73 a 0,89; I2: 0%)]. Na complacência dinâmica, não foi observada diferença significativa entre os grupos [-0,58mL/cmH2O (IC95%: -2,98 a 1,82; I2: 1%)]. Nas variáveis, pressão arterial sistólica e pressão arterial diastólica após avaliação descritiva, foram encontradas diferenças significativas, entretanto, para variáveis volume de secreção, complacência estática, relação pressão arterial de oxigênio por fração inspirada de oxigênio e saturação periférica de oxigênio, não foram encontradas diferenças entre os grupos. Conclusão: Faltam evidências que sustentem o uso da manobra de compressão torácica expiratória na rotina assistencial, pois a literatura sobre o tema é de baixa qualidade metodológica e inconclusiva.

ABSTRACT Objective: To review the literature on the effects of expiratory rib cage compression on ventilatory mechanics, airway clearance, and oxygen and hemodynamic indices in mechanically ventilated adults. Methods: Systematic review with meta-analysis of randomized clinical trials in the databases MEDLINE (via PubMed), EMBASE, Cochrane CENTRAL, PEDro, and LILACS. Studies on adult patients hospitalized in intensive care units and under mechanical ventilation that analyzed the effects of expiratory rib cage compression with respect to a control group (without expiratory rib cage compression) and evaluated the outcomes static and dynamic compliance, sputum volume, systolic blood pressure, diastolic blood pressure, mean arterial pressure, heart rate, peripheral oxygen saturation, and ratio of arterial oxygen partial pressure to fraction of inspired oxygen were included. Experimental studies with animals and those with incomplete data were excluded. Results: The search strategy produced 5,816 studies, of which only three randomized crossover trials were included, totaling 93 patients. With respect to the outcome of heart rate, values were reduced in the expiratory rib cage compression group compared with the control group [-2.81 bpm (95% confidence interval [95%CI]: -4.73 to 0.89; I2: 0%)]. Regarding dynamic compliance, there was no significant difference between groups [-0.58mL/cmH2O (95%CI: -2.98 to 1.82; I2: 1%)]. Regarding the variables systolic blood pressure and diastolic blood pressure, significant differences were found after descriptive evaluation. However, there was no difference between groups regarding the variables secretion volume, static compliance, ratio of arterial oxygen partial pressure to fraction of inspired oxygen, and peripheral oxygen saturation. Conclusion: There is a lack of evidence to support the use of expiratory rib cage compression in routine care, given that the literature on this topic offers low methodological quality and is inconclusive.

ABSTRACT OBJECTIVE: To compare the efficacy of a cycle ergometer-based exercise program to a standard protocol on the increment of the maximum distance walked during the six-minute walk test in the postoperative rehabilitation of patients submitted to coronary artery bypass grafting. METHODS: A controlled clinical trial pilot, blinded to the outcome, enrolled subjects who underwent coronary artery bypass grafting in a hospital from Southern Brazil. Subjects were designated for the standard physical rehabilitation protocol or to an alternative cycle ergometer-based protocol through simple random sampling. The primary outcome was the difference in the maximum distance walked in the six-minute walk test before and after the allocated intervention. RESULTS: Twenty-four patients were included in the analysis, 10 in the standard protocol and 14 in the alternative protocol group. There was an increment in the maximum distance walked in both groups, and borderline superiority in the intervention group comparing to the control group (312.2vs. 249.7; P=0.06). CONCLUSION: There was an increase in the maximum distance walked in the alternative protocol compared to the standard protocol. Thus, it is postulated that the use of a cycle ergometer can be included in physical rehabilitation in the hospital phase of postoperative coronary artery bypass grafting. However, randomized studies with larger sample size should be conducted to assess the significance of these findings.

Fundamento: A síndrome metabólica é um transtorno complexo representado por um conjunto de fatores de risco cardiovascular. A adoção de um estilo de vida saudável está fortemente relacionada à melhora da Qualidade de Vida e interfere de forma positiva no controle dos fatores de risco presentes nessa condição clínica. Objetivo: Avaliar o efeito de um programa de modificação do estilo de vida sobre o Escore de Risco Cardiovascular Global de Framingham em indivíduos com síndrome metabólica. Método: Trata-se de uma subanálise de um ensaio clínico randomizado, controlado, cegado, com duração de três meses. Os participantes foram randomizados em quatro grupos: intervenção nutricional + placebo (INP), intervenção nutricional + suplementação de ácidos graxos ômega 3 (3 g/dia de óleo de peixe) (INS3), intervenção nutricional + atividade física + placebo (INEP) e intervenção nutricional + atividade física + suplementação de ácidos graxos ômega 3 (INES3). O Escore de Risco Cardiovascular Global de Framingham de cada indivíduo foi calculado antes e após a intervenção. Resultados: Participaram do estudo 70 indivíduos. Observou-se uma redução da média do escore após a intervenção de forma geral (p < 0,001). Obteve-se uma redução para risco intermediário em 25,7% dos indivíduos. Após a intervenção, observou-se redução significativa (p < 0,01) da "idade vascular", sendo esta mais expressiva nos grupos INP (5,2%) e INEP (5,3%). Conclusão: Todas as intervenções propostas produziram efeito benéfico para a redução do escore de risco cardiovascular. O presente estudo reforça a importância da modificação do estilo de vida na prevenção e no tratamento das doenças cardiovasculares.

Background: Metabolic syndrome is a complex disorder represented by a set of cardiovascular risk factors. A healthy lifestyle is strongly related to improve Quality of Life and interfere positively in the control of risk factors presented in this condition. Objective: To evaluate the effect of a program of lifestyle modification on the Framingham General Cardiovascular Risk Profile in subjects diagnosed with metabolic syndrome. Methods: A sub-analysis study of a randomized clinical trial controlled blind that lasted three months. Participants were randomized into four groups: dietary intervention + placebo (DIP), dietary intervention + supplementation of omega 3 (fish oil 3 g/day) (DIS3), dietary intervention + placebo + physical activity (DIPE) and dietary intervention + physical activity + supplementation of omega 3 (DIS3PE). The general cardiovascular risk profile of each individual was calculated before and after the intervention. Results: The study included 70 subjects. Evaluating the score between the pre and post intervention yielded a significant value (p < 0.001). We obtained a reduction for intermediate risk in 25.7% of subjects. After intervention, there was a significant reduction (p < 0.01) on cardiovascular age, this being more significant in groups DIP (5.2%) and DIPE (5.3%). Conclusion: Proposed interventions produced beneficial effects for reducing cardiovascular risk score. This study emphasizes the importance of lifestyle modification in the prevention and treatment of cardiovascular diseases.

INTRODUÇÃO: Os principais fatores de risco para as doenças cardiovasculares hipertensivas e isquêmicas da síndrome metabólica (SM) podem ser favoravelmente modificados pelo estilo de vida (EV). OBJETIVO: Avaliar o efeito de um programa de modificação do estilo de vida sobre os fatores de risco cardiovascular de voluntários com síndrome metabólica (SM). MATERIAIS E MÉTODOS: Ensaio clínico em que oito voluntários (cinco mulheres) com diagnóstico de SM foram submetidos, ao longo de três meses, à intervenção nutricional quinzenal e a um programa de exercício físico, realizado em esteira rolante com intensidade de 65% a 75% da frequência cardíaca máxima, com duração de 30 minutos, três vezes por semana. Após o programa os dados foram comparados pelo teste t de Student. RESULTADOS: Houve redução significativa da pressão arterial sistólica (-7,5%), triglicerídeos (-6,7%) e lipoproteínas de alta densidade (-8%). Além disso, o índice de trabalho cardíaco avaliado em repouso reduziu significativamente (-11%), ao passo que o condicionamento físico expresso pelo trabalho muscular e consumo de oxigênio aumentaram significativamente (29% e 24%, respectivamente). CONCLUSÃO: Até o momento, esses resultados indicam que a mudança nos hábitos alimentares associados à prática regular de exercício físico pode reduzir beneficamente as concentrações de triglicerídeos, bem como a PAS e o DP. Além disso, devido à relevância dos resultados obtidos ao final do programa, acredita-se que a continuidade das atividades propostas neste estudo possa beneficiar outros indivíduos com SM.

INTRODUCTION: The main risk factors for hypertensive and ischemic cardiovascular disease of the metabolic syndrome (MS) can be favorably change by the life style (LS). OBJECTIVE: Evaluate the effect of a lifestyle modification program (LMP) on cardiovascular risk factors of the Metabolic Syndrome (MS). MATERIALS AND METHODS: In this clinical trial, 08 volunteers (05 women) with MS underwent a biweekly nutritional intervention and a physical exercise program, using a treadmill, at 65-75 % of maximum heart rate, during 30 minutes, three times a week. After tree mouths on LPM the data were compared using Student's t-test. RESULTS: There was a dramatic reduction in systolic blood pressure (-7.5%), triglycerides (-6.7%) and high density lipoprotein (-8%). Moreover, there was a reduction in resting double product (-11%) and an increase in physical conditioning expressed by the workload in watts (29%), as well as in the oxygen consumption (24%). CONCLUSION: Until this moment, these results indicate that food habits change, associated to frequently physical exercise can reduce the triglycerides concentration as well as SBD and DP. Besides, since the relevance of the results obtained at the end of the program, we believe that the continuity of the activities proposed by this study can benefit other individuals with MS.

OBJETIVOS: Analisar o impacto da hemotransfusão sanguínea na incidência de desfechos clínicos no pós-operatório (PO) de cirurgias cardíacas. MÉTODOS: Estudo de coorte retrospectiva. Foram analisados 4.028 pacientes submetidos à cirurgia de revascularização miocárdica (CRM), troca valvar (TV) ou ambas, em hospital terciário universitário brasileiro, entre 1996 e 2009. Foram comparadas as complicações no PO entre os pacientes hemotransfundidos (n=916) e não-hemotransfundidos (n=3112). Foi realizada análise univariada através do teste t de Student, e análise multivariada com o uso de regressão logística Bivariada (Stepwise Forward). Foram consideradas significativas as variáveis com P<0,05. RESULTADOS: Os pacientes que receberam hemotransfusão apresentaram mais episódios infecciosos como mediastinite (4,9% vs. 2,2%, P<0,001), infecção respiratória (27,8% vs 17,1%, P<0,001), e sepse (6,2% vs. 2,5%, P<0,001). Ocorreram mais episódios de fibrilação atrial (FA) (27% vs. 20,4%, P<0,001), insuficiência renal aguda (IRA) (14,5% vs. 7,3%, P<0,001) e acidente vascular cerebral (AVC) (4,8% vs. 2,6%, P=0,001). O tempo de internação hospitalar no PO foi maior nos transfundidos (13±12,07 dias vs. 9,72±7,66 dias, P<0,001). Porém, a mortalidade não apresentou diferença entre os grupos (10,9% vs. 9,1%, P=0,112). A transfusão mostrou-se como fator de risco para: infecção respiratória (OR: 1,91; IC95%: 1,59-2,29; P<0,001), FA (OR:1,35; IC95%: 1,13-1,61; P=0,01), sepse (OR: 2,08; IC95%: 1,4-3,07; P<0,001), mediastinite (OR: 2,14; IC95%: 1,43-3,21; P<0,001), AVC (OR: 1,63; IC95%: 1,1-2,41; P=0,014) e IRA (OR: 1,8; IC95%: 1,39-2,33; P<0,001). CONCLUSÃO: A hemotransfusão está associada ao aumento do risco de eventos infecciosos, episódios de FA, IRA e AVC, bem como aumentou o tempo de permanência hospitalar, mas não a mortalidade.

OBJECTIVES: To analyze the impact of blood transfusion on the incidence of clinical outcomes postoperatively (PO) from cardiac surgery. METHODS: Retrospective cohort study. We analyzed 4028 patients undergoing coronary artery bypass grafting (CABG), valve (TV), or both, in Brazilian tertiary university hospital between 1996 and 2009. We compared the postoperative complications between patients with blood transfusion (n = 916) and non-blood transfusion (n = 3112). Univariate analysis was performed using the Student t test, and multivariate logistic regression bivariate (stepwise forward). Were considered significant variables with P <0.05. RESULTS: Patients who received blood transfusions had more infectious episodes as mediastinitis (4.9% vs. 2.2%, P <0.001), respiratory infection (27.8% vs 17.1%, P <0.001) and sepsis (6.2% vs. 2.5%, P <0.001). There were more episodes of atrial fibrillation (AF) (27% vs. 20.4%, P <0.001), acute renal failure (ARF) (14.5% vs 7.3%, P <0.001) and stroke (4.8% vs. 2.6%, P = 0.001). The length of PO hospital stay was higher in transfused (13 ± 12.07 days vs. 9.72 ± 7.66 days, P <0.001). However, mortality didn't differ between groups (10.9% vs. 9.1%, P = 0.112). The transfusion was shown to be a risk factor for: respiratory infection (OR: 1.91, 95% CI 1.59-2.29, P <0.001), AF (OR: 1.35, 95% CI 1.13-1.61, P = 0.01), sepsis (OR: 2.08, 95% CI 1.4-3.07, P <0.001), mediastinitis (OR: 2.14, 95% CI: 1.43-3.21, P <0.001), stroke (OR: 1.63, 95% CI 1.1-2.41, P = 0.014) and ARF (OR 1.8, 95% CI: 1.39-2.33, P <0.001). CONCLUSION: The blood transfusion is associated with increased risk of infectious events, episodes of AF, ARF and stroke, as well as the increased length of hospital stay but not mortality.
