Abstract In search of new materials from industrial waste, aiming at cost, efficiency and reduction of environmental liabilities, this study aimed to evaluate the physical, mechanical, acoustic and thermal characteristics of sandwich panels produced with sheets of Schizolobium amazonicum (paricá) and egg carton core made of cardboard, glued with adhesive phenol formaldehyde (FF), urea formaldehyde (UF) and interleaved the sheets glued with both. The panels were evaluated through thermal, acoustic, physical and mechanical tests. The FF and UF adhesives influenced the physical-mechanical properties of the panels. For thermal conductivity and thermal resistance, it was found that all panels met the normative requirements of NBR 15220 (ABNT, 2005) with emphasis on panels produced with FF. Therefore, the use of paricá sheets 32x12x0.2 cm (length, width and thickness, respectively) and egg box as a core with adhesive, proved to be promising for some uses in civil construction, in addition to adding value to the egg crate residues as thermal and acoustic insulators, can minimize the disposal of raw material in the environment.

Resumo Em busca de novos materiais a partir de resíduos industriais, visando ao custo, à eficiência e à redução do passivo ambiental, este trabalho teve como objetivo avaliar as características físicas, mecânicas, acústicas e térmicas de painéis sanduíche produzidos com lâminas de Schizolobium amazonicum (paricá) e núcleo de caixa de ovos feitos de papelão coladas com o adesivo fenol-formaldeído (FF) e com o adesivo ureia-formaldeído (UF), intercalando-se estas com lâminas coladas com ambos (FF+UF). Os painéis foram avaliados por meio de ensaios térmicos, acústicos, físicos e mecânicos. Os adesivos FF e UF influenciaram nas propriedades físico-mecânicas dos painéis. Para a condutividade térmica e resistência térmica, verificou-se que todos os painéis atingiram as exigências normativas da NBR 15220 (ABNT, 2005), com destaque para os painéis produzidos com FF. Sendo assim, a utilização das lâminas de paricá de 32x12x0,2 cm (comprimento, largura e espessura respectivamente) e de caixa de ovo como núcleo com adesivo se mostrou promissora para algumas aplicações na construção civil, além de agregar valor aos resíduos de caixa de ovo como isolantes térmicos e acústicos, por minimizar o descarte de matéria-prima no meio ambiente.

Abstract This study aimed to use the petiole residues of miriti to partially replace pine wood and evaluate the effects of this incorporation on the physical and mechanical properties of the particleboards. Panels were produced with 0, 25, 50, and 75% of miriti in partial replacement of wood and 12% of urea-formaldehyde adhesive. The chemical-physical properties of miriti residues showed potential particleboard production as they were similar to those of pinewood. For the physical properties, the panels were classified as low density. There was an increase in the compression ratio and, consequently, in the hygroscopicity . For the mechanical properties, it was observed that the modulus of elasticity (MOE) and modulus of rupture (MOR) did not show any statistical differences. For perpendicular traction (PT), there was a significant reduction in 50%. Considering a general analysis of the physical and mechanical properties, up to 40% of mirti can be substituted, as in this situation they meet the properties mentioned in CS-236-66, except for PT for commercialization of the panels.

Resumo O objetivo deste trabalho foi utilizar os resíduos do pecíolo do miriti em substituição parcial da madeira de pinus e avaliar os efeitos dessa incorporação nas propriedades físicas e mecânicas dos painéis aglomerados. Os painéis foram produzidos com 0, 25, 50 e 75% de miriti em substituição parcial de madeira e 12% de adesivo uréia-formaldeído. As propriedades físico-químicas dos resíduos de miriti mostraram potencial de produção de aglomerados, uma vez que foram semelhantes às do pinus. Quanto às propriedades físicas, os painéis foram classificados como de baixa densidade. Houve aumento na taxa de compressão e, consequentemente, na higroscopicidade. Para as propriedades mecânicas, observou-se que o módulo de elasticidade (MOE) e o módulo de ruptura (MOR) não houve diferença estatística. Para tração perpendicular (TP) houve redução significativa de 50%. Considerando uma análise geral das propriedades físicas e mecânicas, é possível uma substituição em até 40% de miriti, pois nessa situação atendem as propriedades mencionadas na CS-236-66, com exceção da TP para a comercialização dos painéis.

Abstract: This work aimed to evaluate the physical-mechanical properties of Pinus oocarpa wood agglomerated panels produced with different levels (0 %, 25 %, 50 %, 75 % and 100 %) of wood replacement with bean residues. For this purpose, Pinus oocarpa wood and the agricultural residue were reduced into particles and their properties of apparent density, extractives, lignin and ash content of these raw materials were determined. Then, the particles were dried to a 3 % humidity and granulometrically selected to produce panels with a density of 0,60 g/cm3. These particles were bonded using 12 % urea-formaldehyde adhesive, pressed, following normative dimensions (250 mm x 250 mm x 15 mm (width, length, and thickness)), and kept in a climate-controlled environment (20 ºC ± 2 ºC and 65 % ± 5 % RH) until the physical and mechanical tests were carried out. The research results indicated that the particles from bean residues in comparison with Pinus oocarpa wood, presented low density, lignin and holocellulose values, and higher extractives data. Furthermore, regarding the characterization of the panels, there was a trend towards uniformity in the apparent density values, an increase in the properties of compact ratio, water absorption and thickness swelling, and a decrease in their mechanical properties associated with the increase in the by-product addition to the panels. Therefore, new studies are necessary, seeking a larger study and greater knowledge of the addition effects of bean residues in particulate panels, aiming the dissemination of this sustainable process on large scale.

Resumo Introdução: A síndrome de Parsonage-Turner é uma patologia pouco frequente, caracterizada por omalgia intensa de início súbito e parésia da cintura escapular, que é frequentemente subdiagnosticada. O objetivo deste trabalho é revisitar o tema expondo quatro casos clínicos ilustrativos e uma revisão teórica sumária. Casos Clínicos: Foram avaliados os resultados funcionais e satisfação de quatro doentes (idades compreendidas entre 56 e 84 anos) com o diagnóstico de síndrome de Parsonage-Turner, tratados conservadoramente com analgesia e fisioterapia. Apesar da apresentação típica, o diagnóstico foi tardio em todos os casos. Apenas um doente apontou um fator desencadeante: uma infeção viral precedente. A dor resolveu ao fim de 3-8 dias, persistindo apenas o défice motor no membro superior. Todos os doentes recuperaram, total ou parcialmente, no momento da avaliação final. A reabilitação prolongou-se entre 6-15 meses e foi monitorizada pela clínica e eletromiografia. Todos os doentes estavam satisfeitos à data do último follow-up. Conclusão: A síndrome de Parsonage-Turner tem uma incidência superior à previamente conhecida e é frequentemente subdiagnosticada. Deve ser considerada no diagnóstico diferencial de omalgia, especialmente quando acompanhada de défice motor. Apesar de ser uma doença autolimitada, a recuperação é demorada, pelo que um diagnóstico atempado é essencial para tranquilizar o doente e iniciar o tratamento de suporte adequado.

Abstract Introduction: Parsonage-Turner syndrome is a rare disease characterized by acute and severe shoulder pain followed by upper extremity paralysis, that is severely underdiagnosed in clinical practice. The objective is to describe four illustrative clinical cases and provide a summary review of the topic. Case Reports: Four patients (aged 56 to 84 years-old) diagnosed with Parsonage-Turner syndrome were followed up for functional recovery and patient satisfaction after conservative treatment. Despite presenting with typical sudden-onset pain in the shoulder region, followed by weakness in the shoulder/arm, all patients had delayed diagnosis. Only one patient reported a preceding viral infection, which is the most common trigger. Pain subsided after 3-8 days, but shoulder/arm weakness persisted. All patients recovered, totally or partially, with pain management and physical therapy. Recovery took 6-15 months and was monitored through clinical evaluation and electromyography. At last follow-up, all patients were satisfied. Conclusion: Parsonage-Turner syndrome has a higher incidence than previously assumed and is frequently underdiagnosed. It must be considered in the differential diagnosis of the painful shoulder, especially if muscle weakness is present. Despite being a self-limited condition, recovery is usually slow, and a prompt diagnosis is key for patient’s reassurance and starting adequate supportive treatment.

Abstract: Non-destructive techniques for characterizing materials in-service have been increasing in importance. Thus, it is relevant to assess the potential of non-destructive techniques for solid materials. This work aimed to determine the modulus of elasticity of Bertholletia excelsa wood using the particle image velocimetry technique and the ultrasound method to compare the results with the conventional methodology. For this purpose, samples of Bertholletia excelsa were made using a circular saw. The samples were evaluated for sound propagation to calculate the modulus of elasticity using ultrasound equipment. Subsequently, they were subjected to the compression parallel to grain test in a universal testing machine. The samples were marked and monitored during the loading session, with the repeated capture of images using a professional camera. The deformation values obtained were used to estimate the modulus of elasticity using the particle image velocimetry technique. The mean values of modulus of elasticity found were 17403 MPa for ultrasound, 15589 MPa for the particle image velocimetry technique, and 15333 MPa for the universal testing machine. The particle image velocimetry technique was considered to be statistically similar (Tukey α = 0,05) to the other methods tested. The linear coefficient of determination (R2) between the particle image velocimetry technique and the universal testing machine was 0,95, a high and satisfactory value. Thus, the particle image velocimetry technique and the ultrasound method are valid to estimate the modulus of elasticity of Bertholletia excelsa wood and possibly of woods with similar technological characteristics.

RESUMO O presente trabalho tem como objetivo analisar o efeito de diferentes espécies de madeiras e de bambu (Pinus taeda, Cereus jamacaru e Bambusa vulgaris) na hidratação de matrizes cimentícias. Para tanto, os materiais lignocelulósicos foram moídos e caracterizados antes e depois de tratamento em água quente. Posteriormente, o efeito da adição de diferentes níveis (5, 10 e 33,3%) de madeiras, pré-tratadas ou não, em pastas de cimento foram analisados a partir dos ensaios de calorimetria isotérmica e termogravimetria. Os resultados demonstraram que as partículas de cada espécie lignocelulósica apresentaram um aumento das propriedades de umidade e absorção concomitante a diminuição de suas densidades e níveis de extrativos a partir do pré-tratamento aplicado. Os outros componentes químicos destes elementos, majoritariamente, todavia, não foram afetados. Por outro lado, foi observado que quanto maior o teor de tais materiais lignocelulósicos, pré-tratados ou não, nas matrizes de cimento, maior o impacto na hidratação do cimento, segundo o ensaio de calorimetria. Já com relação ao resultado do ensaio de termogravimetria observou-se uma tendência geral, com ou sem pré-tratamento de partículas ou adição de água de compensação, de aumento dos produtos hidratados, fato atribuído, principalmente, a presença de água livre e gases nos materiais lignocelulósicos.

ABSTRACT The present work aims to analyze the effect of different species of wood and bamboo (Pinus taeda, Cereus jamacaru and Bambusa vulgaris) on the hydration of cementitious matrices. For this, the lignocellulosic materials were ground and characterized before and after treatment in hot water. Subsequently, the effect of adding different levels (5, 10 and 33.3%) of wood, pre-treated or not, in cement pastes were analyzed using isothermal calorimetry and thermogravimetry tests. The results showed that the particles of each lignocellulosic species showed an increase in moisture and absorption properties concomitant with a decrease in their densities and extractive levels from the pre-treatment applied. The other chemical components of these elements, however, were mostly unaffected. On the other hand, it was observed that the higher the content of such lignocellulosic materials, pre-treated or not, in the cement matrices, the greater the impact on cement hydration, according to the calorimetry test. Regarding the result of the thermogravimetry test, a general trend was observed, with or without pre-treatment of particles or addition of make-up water, of an increase in hydrated products, a fact mainly attributed to the presence of free water and gases in the lignocellulosic materials.

Resumo A duplicação do tubo digestivo é uma patologia rara, geralmente diagnosticada em idade jovem. O diagnóstico tardio pode associar-se a complicações como a oclusão intestinal, a hemorragia digestiva ou a perfuração de víscera oca. Apresentamos o caso de uma doente de 60 anos de idade que recorreu ao serviço de urgência por quadro de dor abdominal e obstipação. Na tomografia computorizada abdominopélvica apresentava volumoso fecaloma do cólon sigmoideu que condicionava oclusão intestinal, pelo que foi submetida a cirurgia urgente. No intraoperatório verificou-se presença de duplicação incompleta do cólon esquerdo, que terminava em fundo cego ao nível do reto médio. Esta duplicação levou à formação de um volumoso fecaloma no lúmen duplicado, condicionando oclusão por compressão do cólon funcionante. Foi realizada uma ressecção tipo Hartmann, com colostomia em cano de espingarda, com o lúmen duplicado proximalmente. O pós-operatório decorreu sem intercorrências, tendo sido submetida a cirurgia de restabelecimento da continuidade intestinal ao fim de 6 meses.

Abstract Duplication of the digestive tract is a rare condition, usually diagnosed at a young age. Late diagnosis can be associated with complications such as bowel obstruction, hemorrhage, or perforation. We present the case of a 60-year-old patient admitted to the emergency department due to abdominal pain and constipation. In the computed tomography scan, she presented a large fecaloma of the sigmoid colon causing large bowel obstruction. Intraoperatively there was an incomplete duplication of the left colon, which ended in a cul-de-sac at the mid-rectum. This duplication led to the formation of a large fecaloma in the duplicated colon that compressed and obstructed the functioning lumen. Hartmann’s operation with a double barrel colostomy was performed. The postoperative period was uneventful; bowel continuity was restored 6 months later.

SUMMARY INTRODUÇÃO Os serviços de fisioterapia são necessários para o tratamento de pacientes hospitalizados com COVID-19 e também para pacientes crônicos. Assim, os fisioterapeutas apresentam um maior risco de exposição ao coronavírus. O objetivo deste estudo foi determinar o número de fisioterapeutas que interromperam seus serviços devido à pandemia do COVID-19 e verificar os procedimentos adotados pelos fisioterapeutas que continuam trabalhando. METODOLOGIA A amostra foi constituída por 619 fisioterapeutas, que trabalhavam em Portugal, sendo 154 (24,9%) do sexo masculino e 465 (75,1%) do sexo feminino, com idades compreendidas entre os 22 e 67 anos (34,47 ± 8,70 anos). O instrumento de medida utilizado consistiu num questionário online, tendo sido aplicado em finais de março de 2020, através de contatos e redes sociais. RESULTADOS 453 (73.2%) fisioterapeutas interromperam as suas atividades laborais de forma presencial por causa da pandemia e 166 (26.8%) continuam a trabalhar presencialmente. As principais medidas adotadas pelos fisioterapeutas que continuam a trabalhar presencialmente incluíram: lavagem das mãos (21.5%), uso de máscaras (20.3%), desinfecção do material (19.3%) e uso de luvas (19.3%). Dos fisioterapeutas que não estão a trabalhar presencialmente (n=453), 267 (58.9%) continuam a acompanhar os seus pacientes à distância e 186 (41.1%) não estão a acompanhar os pacientes. As principais medidas utilizadas pelos fisioterapeutas para acompanhar os seus pacientes à distância incluíram: prescrição do tratamento por escrito (38%), realização de vídeos explicativos (26.7%) e tratamento por videoconferência de forma síncrona (23.5%). CONCLUSÕES Os dados do presente estudo revelaram que a maioria dos fisioterapeutas interromperam as suas práticas presenciais em virtude da pandemia COVID-19, no entanto, apesar de não estarem a acompanhar os seus pacientes pessoalmente, a maioria deles elaborou ferramentas para monitorar seus pacientes à distância.

SUMMARY BACKGROUND Physiotherapy services are necessary for hospitalized patients of COVID-19 as well as chronic patients. Thus, physiotherapists present an increased risk of exposure to coronavirus. This study aimed to determine the number of physiotherapists who interrupted their services because of the COVID-19 pandemic and to verify the procedures adopted by the ones who are still working. METHODS The sample comprised 619 physiotherapists who worked in Portugal, 154 (24.9%) male and 465 (75.1%) female, aged between 22 and 67 years (34.47±8.70). The measurement instrument was an on-line questionnaire applied in late March 2020 through contacts and social networks. RESULTS 453 (73.2%) physiotherapists interrupted their work activities in person because of the pandemic and 166 (26.8%) continue to work in person. The main measures adopted by physical therapists who continue to work in person included: hand washing (21.5%), mask use (20.3%), material disinfection (19.3%) and, glove use (19.3%). Of the physiotherapists who are not working in person (n = 453), 267 (58.9%) continue to monitor their patients at a distance, and 186 (41.1%) are not monitoring the patients. The main measures used by physiotherapists to monitor their patients at a distance included: written treatment prescription (38%), making explanatory videos (26.7%), and synchronous video conference treatment (23.5%). CONCLUSIONS Our data revealed that most of the physiotherapists interrupted their face-to-face practices because of the COVID-19 pandemic, however, once they do not follow up their patients’ treatment in person, most of them adapted to monitor their patients from a distance.

Resumo Objetivo Avaliar o tratamento das fraturas de tíbia que evoluíram com perda óssea (traumática ou secundária a infecção) e as complicações ocorridas durante o tratamento com fixador externo e no período imediatamente após sua retirada. Métodos Foram selecionados 40 pacientes tratados entre 2010 e 2017, com a idade média de 33,02 anos, sendo 34 do sexo masculino e 6 do sexo feminino. Todos os pacientes portavam regenerado ósseo da tíbia, foram vítimas de trauma (sobretudo motociclístico), e estavam em seguimento ambulatorial. Resultados Foram obtidos regenerados ósseos da tíbia proximal de até 17 cm e da tíbia distal de 14 cm. O maior transporte trifocal teve a soma do tamanho dos tecidos dos ossos regenerados, medindo 14,5 cm. Como complicações, 29 (72,5%) pacientes tiveram infecção no trajeto dos pinos e fios. Houve 9 (22,5%) casos de refratura, sendo 6 deles tratadas com novo fixador circular, e 2 infecções no osso regenerado. Os pacientes foram submetidos a uma média de 4,72 procedimentos cirúrgicos (2-12), portaram fixador por 20,75 meses (7-55 m.) e permaneceram internados por 53,7 dias (5-183) devido principalmente a complicações de partes moles, a antibioticoterapia intravenosa ou até mesmo a questões sociais. Dois (5%) pacientes apresentaram gonartrose sintomática e outros 2 artrite sintomática do tornozelo. Três apresentaram discrepância de membros inferiores de 3,0; 3,7; e 5,0 cm. Conclusão Apesar de não ser um método de tratamento amplamente disponível, o método de Ilizarov é útil para solucionar a maioria das falhas ósseas da tíbia, independente da sua etiologia.

Abstract Objectives To evaluate the management of tibial fractures resulting in bone loss (traumatic or infection-related) and the complications occurring during treatment with external fixator and immediately after apparatus removal. Methods Forty patients were selected from 2010 to 2017. The mean age of the patients was 33.02 years; 34 subjects were male and 6 were female. All patients had tibial bone regeneration, suffered trauma (mainly related to motorcycle accident) and were followed-up at an outpatient facility. Results Proximal tibial bones of up to 17 cm in length and distal tibial bones of up to 14 cm in length were obtained. The largest trifocal transport had the same length as the regenerated bone tissues, which was 14.5 cm. Regarding complications, 29 (72.5%) patients had infections in the pin and wire paths. There were 9 (22.5%) cases of de novo fracture, 6 of which were managed with the implantation of a new circular fixator, and 2 cases of infection of the regenerated bone. On average, patients were subjected to 4.72 procedures (ranging from 2-12), had the fixator for 20.75 months (ranging from 7-55 months), and stayed at the hospital for 53.7 days (ranging from 5-183 days), mainly because of soft-tissue complications, intravenous antibacterial therapy, and even social issues. Two (5%) patients presented symptomatic gonarthrosis, and two other patients had symptomatic ankle arthritis. Three of the patients showed lower limb discrepancy of 3.0, 3.7, and 5.0 cm. Conclusion Despite not being widely available, the Ilizarov method is useful for solving the majority of tibial bone losses, regardless of their etiology.

Stainless steel and nickel alloy have high corrosion resistance in high-temperature environments due to the high Cr content present in their chemical composition, being widely used in components of nuclear reactors, petrochemical industries, etc. Through proper processes and procedures, it becomes possible to join these alloys. However, this union can generate detrimental factors in its performance, among them, the residual stresses. In this work, the residual stresses generated by the autogenous GTAW process, due to different interpass temperatures on the weld bead geometry, were analyzed by the Hole-Drilling technique in dissimilar welding joints of stainless steel AISI 316L and Inconel 718 alloy. In addition, the Vickers microhardness measurements were carried out to evaluate the hardness profile in the cross section of the weld bead covering base metal (BM), heat affect zone (HAZ) and weld metal (WM). We found that in the interface region between BM and HAZ of each dissimilar joint metal, residual stresses increased above 300 MPa, while hardness increased above 160 HV.

Objetivo: Os quistos sinoviais da articulação acromioclavicular (AC), podem ocorrer associadas à artrose AC ou como consequência da rotura massiva da coifa dos rotadores. Com o presente caso, os autores pretendem apresentar e discutir uma situação pouco frequente. Relato de caso: Homem de 81 anos, referenciado à consulta de Ortopedia por tumefação da região superior do ombro direito, indolor e de aumento progressivo nos últimos 2 anos. Avaliação clínica e radiológica compatível com volumoso quisto sinovial da acromioclavicular, associado a rotura massiva da coifa dos rotadores; submetido a tratamento cirúrgico com excisão marginal da lesão; a análise anatomopatológica confirmou o diagnóstico de quisto sinovial. Comentários: Os quistos sinoviais da articulação AC são entidades raras e pouco abordadas na literatura. A sua etiologia permanece por esclarecer ainda que um dos tipos descritos pareça estar associada à omatrose com rotura massiva da coifa dos rotadores (artropatia da coifa); nestes, a rotura da coifa permite a passagem de grandes quantidades de líquido sinovial para o espaço subacromial com formação de um volumoso quisto. Atendendo à sua raridade, o tratamento permanece controverso. O caso vem alertar para a ocorrência dos quistos da articulação AC, como apresentação invulgar de artropatia das coifa dos rotadores, discutindo a sua etiologia e abordagem terapêutica adequada.

Objectives: Synovial cysts of the acromioclavicular (AC) usually occur in the setting of AC arthrosis, or more frequently, as a consequence of massive rotator cuff tears. With the following case report, the authors pretend to presente and discuss an unusual situation. Case report: An 81-year-old male, presented to our outpatient clinic due to a non-traumatic, painless lump on the superior aspect of his right shoulder, with progressive enlargement in the last 2 years. Clinical examination and imagiologic assessment showed a large acromioclavicular joint cyst associated with a massive rotator cuff tear; the patient was submitted to surgical treatment with excision of the lesion; anatomopathological exams confirmed the diagnosis of joint cyst. Discussion: AC joint cysts are rare and sparsely discussed in the literature. Their etiology remains to be clarified, although, one of the types described is thought to be associated with glenoumeral arthrosis with massive rotator cuff tears (cuff arthropathy). In these, the rotator cuff tear allows the passage of large amounts of synovial fluid to the subacromial space, leading the formation of a large cyst. Treatment remains controversial given its rarity. The case should raise awareness for the occurrence of AC synovial cysts as unusual presentations of rotator cuff arthropathy, discussing their etiology, diagnostic and therapeutic approach.

RESUMO A pneumonite secundária à imunoterapia é uma complicação rara, com incidência estimada em cerca de 3%. No entanto, trata-se de uma intercorrência de difícil diagnóstico e com grande morbidade, que tem se tornado um desafio para oncologistas e emergencistas. Foram revisados os casos de cinco pacientes que fizeram uso de anti-PD1 (program cell death receptor antagonist 1) para tratamento antineoplásico e que evoluíram com quadro de pneumonite induzida pelo tratamento. Todos os pacientes apresentaram sintomas respiratórios em vigência de tratamento, com imunoterapia e presença de alteração radiológica em vidro fosco. Dentre estes pacientes, apenas um apresentou pneumonite grau 5, com atraso na introdução de corticoidoterapia, indo a óbito em decorrência do quadro. Os outros quatro pacientes apresentaram pneumonite grau 2, sintomática, sendo tratados com corticoidoterapia e evoluindo com melhora clínica e radiológica. Dois pacientes mantiveram o tratamento após o episódio de pneumonite, sem novas complicações pulmonares posteriores, até o momento. A pneumonite induzida por imunoterapia, apesar de ser um evento pouco frequente, pode acarretar grande morbidade, além de ser potencialmente fatal, cabendo à equipe médica ter atenção aos sintomas mais comuns, como tosse e dispneia, para diagnóstico precoce e tratamento efetivo, com uso precoce de corticoide.

ABSTRACT Immunotherapy-induced pneumonitis is a rare complication with incidence estimated around 3%. This disease is difficult to diagnose and has great morbidity. For this reason, it became a challenge for oncologists and emergencists. We reviewed the case of five patients who used anti-PD1 (program cell death receptor antagonist 1) for antineoplastic treatment and developed treatment-induced pneumonitis. All patients had respiratory problems because of immunotherapy and presence of ground-glass radiologic change. Among all patients, only one had grade 5 pneumonitis, and delaying to begin corticosteroid therapy and worsening in clinical picture led to patient death. Other four patients with symptomatic grade 2 pneumonitis underwent corticosteroid therapy and had improvement in clinical and radiologic picture. Two patients were treated after an episode of pneumonitis, and no new pulmonary complications were observed until the end of this study. Immunotherapy-induced pneumonitis, although uncommon, can be potentially fatal. Medical team has the responsibility to pay attention for most common symptoms of the disease such as cough and dyspnea and conduct an early diagnosis and effective early treatment with corticosteroids.

Objectivo: apresentamos um caso de luxação glenoumeral posterior bilateral complicada de fractura cefálica, na sequência de choque eléctrico, de particular interesse pela raridade da lesão e pela opção tomada no tratamento desta. Caso clínico: paciente do sexo masculino de 34 anos de idade, com luxação glenoumeral posterior bilateral, à esquerda com pequena fratura cefálica e área de impacção totalizando 30% da superfície articular e, à direita, com fratura cefálica e impacção envolvendo no total cerca de 45% da superfície articular. Submetido a tratamento cirúrgico de urgência: à esquerda, com remoção do fragmento osteocondral e transposição do troquino para o defeito cefálico anterior (fixação com 2 parafusos); à direita, com redução e fixação do fragmento osteocondral e reconstrução cefálica com enxerto osteocondral da cabeça umeral contralateral fixado com parafuso. Ao último follow-up aos 3 anos e 8 meses, apresentava um Score de Constant absoluto de 92 à esquerda e 80 à direita, arcos de mobilidade ativos amplos (esquerda: abdução 125º, flexão 140º, rotação externa 45º e rotação interna L2; direita: abdução 90º, flexão 110º, rotação externa 10º e rotação interna L1) e testes de instabilidade negativos bilateralmente. Estudo imagiológico com centragem cefálica, consolidação óssea da fractura e enxertos, sem sinais de necrose avascular ou alterações degenerativas. Discussão: as luxações posteriores do ombro são lesões raras, associadas a lesão por convulsão ou choque eléctrico. A área de impacção da superfície articular, lesões associadas, em conjunto com a idade e atividade do doente, devem orientar o tratamento. No caso apresentado esta opção de tratamento foi possível pela abordagem cirúrgica bilateral no mesmo tempo operatório, o que permitiu restabelecer uma função satisfatória, com bons resultados 6 meses após a cirurgia e que se mantêm ao final de 4 anos.

Objective: we present a case of bilateral glenumeral posterior dislocation complicated with humeral head fracture, after electrical shock. It is particularly relevant because of the infrequency of the lesion and the option taken in its treatment. Case report: male patient, of 34 years of age, with bilateral glenoumeral posterior dislocation, in the left with a small cephalic fracture and impaction area of 30% of the articular surface, and, in the right, with cephalic fracture and impaction of 45% of the articular surface. Submitted to urgent surgical treatment: in the left, removal of the osteochondral fragment and translation of lesser tuberosity to the anterior cephalic defect (fixation with 2 screws); in the right, reduction and fixation of the osteochondral fragment and cephalic reconstruction with osteochondral graft from the contralateral humeral head, fixed with screw. To the latest follow-up, at 3 year and 8 months, the patient presented an absolute Constant Score of 92 in the left and 80 in the right, wide active range of movement (left: abduction 125º, flexion 140º, external rotation 45º and internal rotation L2; right: abduction 90º, flexion 110º, external rotation 10º and internal rotation L1) and negative instability tests bilaterally. Image study showed concentric head, bony union of the fracture and grafts, and no signs of avascular necrosis or degenerative changes. Discussion: posterior glenoumeral dislocations are rare lesions, associated with trauma by seizures or electric shocks. The area of impaction of the articular surface, associated lesions, together with age and function of the patient, should guide the treatment decision. In the presented case, the treatment option was possible due to the bilateral surgical approach in the same surgical time. This allowed to obtain a satisfactory function, with good results at 6 months post-op, that are stable even 4 years after surgery.

Objetivo: a lesão obstétrica do plexo braquial está presente em 1-4 em cada 1000 partos. Apesar da maioria recuperar, até 19% vão apresentar sequelas definitivas, mesmo após cirurgias de reparação primária do plexo braquial, sendo o ombro a região anatómica mais afetada. As cirurgias paliativas do ombro foram desenvolvidas para aumentar a função e qualidade de vida destes pacientes, no entanto não há consenso quanto ao timing cirúrgico e indicações de cada técnica. Avaliamos sistematicamente toda a evidência disponível nos últimos 10 anos com o objetivo de determinar quais as melhorias na função e quais os fatores que influenciam o resultado destas técnicas. Determinar a capacidade de remodelação gleno-umeral foi um objetivo secundário. Fonte: pesquisamos 5 bases de dados - Cochrane Review Library, TRIP, Pubmed, Web of Knowledge e Science Direct. Foram incluídos estudos de doentes com lesões obstétricas do plexo braquial submetidos a técnicas paliativas do ombro publicados nos últimos 10 anos (de janeiro de 2003 a dezembro de 2012). Avaliamos as mobilidades ativas e a classificação de Mallet pós-operatória. Numa análise post-hoc avaliamos o efeito das técnicas na remodelação glenóideia, através das alterações do ângulo de retroversão, percentagem de cobertura da cabeça umeral e da alteração morfológica. A avaliação da qualidade metodológica foi realizada independentemente por 2 autores, através de um formulário de revisão crítica. Síntese dos dados: foram incluídos 1873 doentes num total de 42 estudos: 27 estudos com técnicas de partes moles, 12 estudos com procedimentos ósseos e 3 com combinação de ambas as técnicas. Todos os estudos têm baixo nível de evidência, sendo a maioria séries de casos, com múltiplos viéses e elevada heterogeneidade. A melhoria média da rotação externa foi de 58º e 61º na abdução. A melhoria média na classificação de Mallet foi 5,0 pontos e 1,5 no parâmetro da rotação externa. Observou-se melhoria da retroversão glenóideia entre 9 e 21º e percentagem de cobertura cefálica entre 4 e 25%. Conclusões: considerando a qualidade metodológica dos estudos incluídos, a evidência parece apontar para um efeito benéfico das técnicas paliativas gleno-umerais na mobilidade, função e remodelação glenoumeral. No entanto, não é possível estabelecer indicações claras para as diferentes técnicas, o melhor timing para a cirurgia, nem se o benefício se mantêm após 10 anos da data da cirurgia. Para responder a estas questões é necessário investir em estudos com desenhos apropriados, de elevada qualidade metodológica e com estratégias de diminuição da heterogeneidade.

Objectives: obstetric Brachial Plexus Palsy occurs in 1-4 in every 1000 births. Although most recover, up to 19% will present definitive sequelae, even after primary repair surgeries of the brachial plexus, being the shoulder the most commonly affected area. Palliative surgeries have been developed to increase function and quality of life of these patients, however there’s no agreement in regard to the surgical timing and indications of each technique. We evaluated all the available evidence published in the last 10 years with the goal to determine the improvement in function and the factors that have an influence on the result of these techniques. Determination of the gleno-umeral remodeling capacity was a secondary objective. Source: we have researched 5 databases - Cochrane Review Library, TRIP, Pubmed, Web of Knowledge and Science Direct. We included studies that had obstetric brachial plexus palsy patients who were submitted to palliative techniques of the shoulder, published in the last 10 years (from January 2003 to December 2012). We evaluated the post-operative active range of motion and Mallet classification. In a post-hoc analysis, we evaluated the effect of the technique in glenoid remodelation, through retroversion angle, percentage of humeral head coverage and change in morphology. The methodological quality was assessed independently by two reviewers, through a critical review form. Data syntheses: it was included 1873 patients from a total of 42 studies: 27 studies with soft tissues procedures, 12 studies with bony procedures and 3 with combination of techniques. All the studies have low evidence level, and the majority are case series with multiple biases and high heterogeneity. The main improvement in external rotation was 58º and 61º in abduction. The main improvement in Mallet score was 5,0 points and 1,5 in the parameter of external rotation. We observed improvement in glenoid retroversion between 9 and 21º and percentage of humeral head covereage between 4 and 25%. Conclusions: considering the methodological quality of the included studies, the evidence seems to show an improvement in range of motion, function and gleno-umeral remodelation with palliative techniques. However it’s not possible to establish clear indications to the different techniques, the better surgical timing, neither if the benefits withhold 10 years after surgery. To answer these questions it’s necessary to invest in studies with appropriate designs, of high methodological quality and with strategies to diminish heterogeneity.
