OBJETIVO: Questões relativas a quais pacientes/doenças que efetivamente deveriam ser submetidos ao tratamento endovascular ainda geram controvérsias. O objetivo deste trabalho visa a questionar o tratamento endovascular nas dissecções crônicas tipo B de Stanford. MÉTODOS: No período de 2003 a 2006, 11 pacientes portadores de dissecção crônica da aorta tipo B de Stanford com dilatação somente no tórax (diâmetro > 5,5cm), foram submetidos à colocação de prótese endovascular autoexpansível pela artéria femoral. Todos os pacientes foram submetidos à angiotomografia de controle com 6 meses, 1 ano e após, anualmente, com o intuito de avaliar a presença de fluxo na falsa luz e estudar a evolução dos diâmetros da aorta torácica descendente e abdominal no decorrer do tempo. Para esta análise foram utilizados os testes Anova de duas vias para medidas repetidas e o qui-quadrado com o programa SPSS 13. RESULTADOS: Não houve mortalidade hospitalar. Nenhum paciente apresentou endoleak imediatamente após a operação. Durante o período de seguimento médio de 28 meses/paciente, não houve óbitos, um paciente foi submetido à substituição da aorta tóraco-abdominal e dois aguardam esta mesma intervenção. A endoprótese interrompeu o fluxo na falsa luz no tórax em 72,7% dos pacientes e, no abdome, somente em 18,2%, porém isto não implicou na redução dos diâmetros da aorta torácica nem abdominal. CONCLUSÃO: O tratamento endovascular nas dissecções crônicas tipo B de Stanford parece não ser suficiente para tratar esses pacientes no curto/médio prazo, apesar do tamanho reduzido da amostra estudada.

OBJECTIVE: Questions regarding the specific patient/disease that should be submitted to the endovascular procedure still remain unclear. The purpose of this report is to evaluate the endovascular treatment in chronic type B aortic dissections. METHODS: Between 2003 and 2006, 11 patients with chronic type B aortic dissection were submitted to endovascular procedure through femoral artery. All of them were monitored with CT within 6 months, 1 year and afterwars anually. We prospectively evaluated false lumen patency and thoracic and abdominal aortic diameters in each time point. The data comparisons were made using Anova and chi-square tests with SPSS 13. RESULTS: The endovascular stent-graft deployment was technically successful for all patients, with no hospital mortality. During the follow-up period the false lumen flows remained persistent in the thorax in 27.3% of the patients and in the abdomen in 81.8%. However, in all patients, in both segments, the aorta diameter was not significantly changed in size and shape. CONCLUSION: Despite the small number of studied patients, the endovascular procedure for chronic type B aortic dissections does not appear to be an option for the treatment of these patients.

INTRODUÇÃO: O choque cardiogênico é responsável por elevados índices de mortalidade na fila de espera para o transplante cardíaco. Na cardiomiopatia chagásica, a alta incidência de disfunção biventricular pode contribuir com a gravidade desta complicação. MÉTODO: Foram estudados 141 pacientes indicados em caráter de prioridade para o transplante. Destes pacientes, 46 eram portadores de cardiomiopatia chagásica e 95 de outras cardiomiopatias. O choque cardiogênico foi tratado farmacologicamente e com o implante ocasional do balão intra-aórtico. Em cinco pacientes chagásicos, foi realizado o implante de dispositivo paracorpóreo de assistência ventricular esquerda. RESULTADOS: Num período médio de 2,8 meses, 58 (41,1%) dos 141 pacientes foram transplantados, 73 (51,7%) faleceram e 10 foram retirados da fila. A mortalidade entre os pacientes chagásicos e não chagásicos foi de 45,6% e 54,7%, respectivamente. No entanto, a expectativa média de vida, sem a realização do transplante cardíaco, dos pacientes chagásicos foi de apenas 1,5 meses, sendo observado risco relativo de mortalidade de 1,6 para estes pacientes em relação aos não chagásicos (p<0,05). Os cinco pacientes chagásicos submetidos ao implante do dispositivo de assistência circulatória foram mantidos por um período médio de 22 dias, sendo que dois foram transplantados, dois faleceram por falência de múltiplos órgãos e um ainda está sob assistência. Nenhum destes pacientes apresentou disfunção do ventrículo direito, não tendo ocorrido qualquer complicação relacionada ao dispositivo. CONCLUSÃO: A evolução do choque cardiogênico parece ser mais rápida na cardiomiopatia chagásica, sendo importante a indicação precoce de dispositivos de assistência mecânica como ponte para a realização do transplante cardíaco.

INTRODUCTION: Heart failure is responsible for high mortality rates of patients on heart transplantation waiting lists. In Chagas cardiomyopathy, the presence of biventricular dysfunction increases the severity of this situation. METHOD: One hundred and forty-one patients suffering from cardiogenic shock, listed as high priority for heart transplantation, were studied. Forty-six patients presented with Chagas cardiomyopathy and 95 with other cardiomyopathies. Heart failure was treated using intravenous inotropic drugs and intra-aortic balloon pump implantation. Five patients with Chagas disease underwent paracorporeal left ventricular assist device implantation. RESULTS: During a mean follow-up of 2.8 months, 58 (41.1%) of the 141 patients were transplanted, while 73 (53.7%) died and 10 were removed from the waiting list. The mortality rates in chagasic and non-chagasic patients were 45.6% and 54.7%, respectively. The mean expected survival of patients with Chagas disease, who did not undergo heart transplantation, was only 1.5 months, with these patients presenting a relative risk for death of 1.6 compared to patients with other heart diseases (p<0.05). The five chagasic patients submitted to left ventricular assist device implantation were maintained on support for a mean of 22 days, with two of them undergoing transplantation, two died due to multiple organ failure and one remains on circulatory support. None of these patients presented right ventricular dysfunction and there were no device related complications. CONCLUSION: The evolution of heart failure seems to be rapid in patients with Chagas cardiomyopathy. Therefore, an early indication of mechanical circulatory support is important as a bridge to heart transplantation in these patients.

No período de março de 1985 a setembro de 1999, 214 pacientes foram submetidos a transplante cardíaco em conseqüência de cardiomiopatia refratária ao tratamento farmacológico. Dois (0,9%) pacientes, com idades de 33 e 49 anos, desenvolveram dissecção da aorta torácica como complicação tardia fatal após o transplante cardíaco ortotópico. Na primeira paciente, com cardiomiopatia idiopática, esta complicação ocorreu no 93º mês de evolução, enquanto que no segundo, com cardiomiopatia isquêmica, a ocorrência foi mais precoce, no 11º mês. A hipertensão arterial sistêmica e o tabagismo estiveram presentes como fatores de risco em ambos os casos. As manifestações clínicas da dissecção ocorreram de forma aguda e catastrófica que impossibilitaram qualquer atitude cirúrgica e, possivelmente, maior atenção desta rara complicação possa modificar a sua péssima evolução natural.

From March 1985 to September 1999, 214 patients were submitted to cardiac transplantation in consequence of refractory cardiomyopathy to medical therapy. Two patients (0.9%) aged 33 and 49 years, had developed thoracic aortic dissection as fatal late complication after the orthotopic cardiac transplantation. In the first patient suffering from idiopathic cardiomyopathy this complication occurred in the 93rd month of evolution, while in the second, with ischemic cardiomyopathy, the occurrence was early, in the 11th month. Systemic arterial hypertension and a smoking history had been showed as factors of risk in both cases. The clinical manifestations of the dissection had occurred of acute form and catastrophic that had prevented any surgical management. Greater attention of this rare complication can affect its bad natural evolution.

OBJECTIVE: Assessment of incidence and behavior of mediastinitis after cardiac transplantation. METHODS: From 1985 to 1999, 214 cardiac transplantations were performed, 12 (5.6%) of the transplanted patients developed confirmed mediastinitis. Patient's ages ranged from 42 to 66 years (mean of 52.3±10.0 years) and 10 (83.3%) patients were males. Seven (58.3%) patients showed sternal stability on palpation, 4 (33.3%) patients had pleural empyema, and 2 (16.7%) patients did not show purulent secretion draining through the wound. RESULTS: Staphylococcus aureus was the infectious agent identified in the wound secretion or in the mediastinum, or both, in 8 (66.7%) patients. Staphylococcus epidermidis was identified in 2 (16.7%) patients, Enterococcus faecalis in 1 (8.3%) patient, and the cause of mediastinitis could not be determined in 1 (8.3%) patient. Surgical treatment was performed on an emergency basis, and the extension of the débridement varied with local conditions. In 2 (16.7%) patients, we chose to leave the surgical wound open and performed daily dressings with granulated sugar. Total sternal resection was performed in only 1 (8.3%) patient. Out of this series, 5 (41.7%) patients died, and the causes of death were related to the infection. Autopsy revealed persistence of mediastinitis in 1 (8.3%) patient. CONCLUSION: Promptness in diagnosing mediastinitis and precocious surgical drainage have changed the natural evolution of this disease. Nevertheless, observance of the basic precepts of prophylaxis of infection is still the best way to treat mediastinitis.

OBJETIVOS: O objetivo do presente estudo é apresentar e discutir a evolução clínica de 2 pacientes que, no pós-operatório de transplante cardíaco ortotópico, apresentaram formação de abscesso pulmonar em conseqüência de infecção pulmonar complicada. CASUÍSTICA E MÉTODOS: Em ambos os casos, a lobectomia foi indicada como último recurso em virtude da falta de resposta ao tratamento clínico convencional. No ato operatório, houve rigor na observância dos preceitos clássicos adotados, habitualmente, nas ressecções pulmonares para se evitar o aparecimento de fístulas broncopleurais. RESULTADOS: Os pacientes apresentaram boa evolução pós-operatória e resolução definitiva do quadro infeccioso e, atualmente, encontram-se em seguimento pós-operatório em condições clínicas estáveis.

OBJECTIVE: The purpose of this study is to present and discuss the clinical evolution of 2 patients who after orthotopic heart transplantation presented complicated pulmonary infection, resulting in the formation of abscess. MATERIAL AND METHODS: In both cases lobectomy was indicated due to failure to respond to conventional clinical treatment. At operation classical rules of pulmonary resection were strictly adhered to avoid the appearance of bronchopleural fistulas. RESULTS: The patients presented a good postoperative course with definitive resolution of infections and at follow-up are in stable condition.

A técnica do potencial de ação monofásico (MAP) é um método de obtenção de potencial in vivo, por meio de cateteres especiais, muito semelhante ao potencial de ação celular, porém, com a vantagem de não exigir o emprego de microeletrodos transmembrana in vitro na célula miocárdica. Este trabalho apresenta detalhes da construção de um cateter-eletrodo para aquisição de MAP endocárdico in vivo, bem como o desempenho do algorítmo desenvolvido para aquisição e análise automática dos sinais captados. Vários fatores afetam a fase de repolarização miocárdica, sendo pouco visíveis ao eletrocardiograma, porém, podem ser melhor identificados pelo MAP. As miocardiopatias e o transplante são situações que podem modificar o estado morfo-funcional da fibra miocárdica. O algorítmo desenvolvido para análise do MAP baseia-se na discriminação de características morfológicas e temporais dos potenciais. Inicialmente foram estudados 23 pacientes com indicação para realização de biópsia endomiocárdica, sendo que em 8 (34,8%) deles a biópsia foi indicada para complementar a avaliação da miocardiopatia idiopática e em 15 (65,2%) para controle de rejeição após transplante cardíaco ortotópico. Foram constituídos dois grupos de 5 pacientes, um com portadores de miocardiopatia (excluindo-se miocardite em atividade) e outro com transplante (estudados em duas situações distintas, sem e com rejeição em grau 3A ou superior). Com base nos resultados obtidos desta investigação, pode-se concluir que: 1) o cateter-eletrodo ora apresentado confirmou, na investigação clínica, o bom desempenho apresentado em pesquisa experimental para aquisição do MAP; 2) o algorítmo computadorizado mostrou-se adequado e com igual desempenho apresentado em estudo experimental; 3) as interferências que o MAP sofre nos corações com miocardiopatia ou transplantados, sem ou com rejeição, determinam alterações proporcionais de tal forma que conservam a morfologia da onda de repolarização; 4) durante os episódios de rejeição a velocidade de despolarização do miócito dobra ou triplica.

Monophasic Action Potential (MAP) is a technique which assesses in vivo cellular potencial using special catheters without the need of using in vitro transmembrane cellular electrodes. We studied the implementation and the results of in vivo endocardial MAPs using electrode-catheters as well as the algorithm used for computed analysis of captured signals. Myocardial repolarization may be affected by several factors that are not detected by the electrocardiogram and are detected by MAP. The algorithm used to analyse the MAP uses morphological and temporal characteristics to assess cellular potencial. Heart transplantation and any cardiomyopathy may alter myocardial fiber structure. We studied 23 patients who were submitted to endomyocardial biopsies, of whom 8 (34.8%) underwent this procedure to evaluate their cardiomyopathy, and 15 (65,2%) in order to evaluate postoperative rejection of the graft. After the biposies we formed 2 groups of 5 patients each; one group was formed of 5 patients with myocardiopathies without active myocarditis, and the other one was formed by 5 patients with and without grad 3A rejection. Our results demonstrated that: 1) the electrode-catheter presented similar results to previous experimental reports; 2) the computed algorithm was efficient and presented similar results to previous experiments; 3) MAP interference due to cardiomyopathy or rejection induces proportional alterations that maintain repolarization morfology; 4) depolarization speed during rejection undergoes a two or three-fold increase.

A cardiomioplastia tem sido proposta, como uma alternativa ao transplante cardíaco, no tratamento de pacientes com insuficiência miocárdica em fase avançada. O objetivo deste trabalho é estudar a evolução clínica e o comportamento da função ventricular no pós-operatório tardio desse procedimento em 34 pacientes portadores de cardiomiopatia dilatada, que foram operados no período de maio de 1988 a setembro de 1994. Vinte e sete pacientes estavam em classe funcional III e 7 pacientes em classe IV no preoperatorio, apesar do uso de terapêutica clínica otimizada. A mortalidade hospitalar foi de 2,9% e 1 paciente que evoluiu em choque cardiogênico foi submetido a transplante cardíaco 42 dias após a cardiomioplastia. O tempo de seguimento pós-operatório variou entre 2 e 73 meses, com média de 27,4 meses. Aos 6 meses de pós-operatório, 12 pacientes estavam em classe funcional 1,15 pacientes em classe 11 e 3 pacientes em classe III (p=0,001) em relação ao pré-operatório. Quatorze pacientes faleceram até 5 anos de pós-operatório e os índices de sobrevida foram 84,7% em 1 ano, 67,7% em 2 anos e 39,6% aos 5 anos de seguimento, sendo que, em 9 pacientes, os óbitos ocorreram por progressão da insuficiência cardíaca, e 5 pacientes faleceram subitamente. A análise de regressão de Gox mostrou que a mortalidade nos pacientes operados em classe funcional IV foi 5,5 vezes maior do que nos pacientes operados em classe III (p=0,006), cuja sobrevida foi de 52,7% aos 5 anos de pós-operatório. O estudo sistemático da função ventricular através da angiografia com radioisótopos, da ecocardiografia com Doppler e do cateterismo cardíaco direito documentou a melhora da fração de ejeção do ventrículo esquerdo (V E) de 19,8 ± 3 para 23,9 ± 7,2% (p<0,01), associada a modificações significativas do encurtamento segmentar do V E, do volume e do índice sistólico, da pressão arterial média, das pressões pulmonares e do índice de trabalho sistólico do V E, aos 6 meses de seguimento. No período pós-operatório tardio, a fração de ejeção e o encurtamento segmentar do V E tenderam a cair e retornaram a valores semelhantes aos do pré-operatório em 5 anos, sendo observada no mesmo período a manutenção dos benefícios hemodinâmicos demonstrados após o procedimento. A eletrocardiografia dinâmica mostrou que a incidência de arritmias ventriculares e de fibrilação atrial não se modificou após a cardiomioplastia. Finalmente, dos 5 pacientes que completaram 5 anos de seguimento, 3 faleceram por progressão de insuficiência cardíaca e 1 por morte súbita até os 73 meses de pós-operatório. Além disso, o estudo anatomopatológico mostrou a existência de infiltração gordurosa no enxerto muscular em 9 pacientes que faleceram entre 20 e 73 meses de pós-operatório, sendo observadas alterações mais intensas do enxerto em pacientes com maior tempo de seguimento. Em conclusão, a cardiomioplastia melhora a condição clínica e a função ventricular esquerda de pacientes com cardiomiopatia dilatada. A sobrevida no pós-operatório tardio do procedimento é limitada, no entanto, em pacientes operados em classe funcional IV. Além disso, os benefícios da cardiomioplastia sobre a função sistólica do VE podem estar restritos a poucos anos de pósoperatório, fato que parece estar associado à degeneração tardia do enxerto muscular.

Dynamic cardiomyoplasty has been proposed as an alternative to heart transplantation in the treatment of severe cardiomyopathies. In this investigation, its results were investigated in 34 patients with idiopathic dilated cardiomyopathy submitted to this procedure from May 1988 to September 1994. Twenty seven patients were in New York Heart Association functional class III and 7 were in class IV before the operation, despite the use of maximal medical therapy. Hospital mortality was 2.9% and 1 patient presenting heart failure progression was submitted to urgent heart transplantation 42 days after cardiomyoplasty. The other patients were followed-up from 2 to 73 months (mean, 27.4 months). At 6 months of follow-up, 12 patients were in functional class 1,15 patients in class II and 3 patients in class III (p=0.001, in relation to preoperative data). Similar findings were observed from one to four years of follow-up. On the other hand, 14 patients died up to 5 years after the operation and the survival rates were 84.7% at 1 year, 67.7% at 2 years and 39.9% at 5 years of follow-up. The cause of death was progression of heart failure in 9 patients and 5 patients died suddenly. By stepwise Cox regression method, patients operated in functional class IV had relative risk of death 5.5 times higher than class III patients (p=0.006), whose survival rate was 52.7% at 5 years of followup. Regarding cardiomyoplasty influence on ventricular function, radioisotopic left ventricular (LV) ejection fraction improved from 19.8 + 3.1% to 23.6 + 6.2%, at 6 months of follow-up. Doppler-echocardiography documented similar changes in LV wall shortening. Also, right heart catheterization showed significant increases in mean arterial pressure, stroke index and LV stroke work index, which were associated with the decrease of mean pulmonary wedge pressure. On the other hand, it was not found any significant postoperative change in ventricular arrhythmia and atrial fibrillation incidence. At the late postoperative period, the hemodynamic improvement was maintained up to 5 years of follow-up, while LV ejection fraction and wall shortening tended to decrease after the second postoperative year returned to the preoperative levels. Finally, 3 of the 5 patients who completed 5 years of follow-up died due to heart failure progression and other died suddenly up to 73 months after the operation. Furthermore, histological examinations showed important fat tissue infiltration in the skeletal muscle flap of 9 patients submitted to necropsy studies from 20 to 73 months of follow-up. These histological changes were more severe in patients with longer follow-up. In conclusion, dynamic cardiomyoplasty improves congestive heart failure and LV function in patients with idiopathic dilated cardiomyopathy. However, long-term survival after this procedure is limited in patients operated upon in functional class IV. Furthermore, the cardiomyoplasty benefits on left ventricular function appear to be limited to a midterm period and this fact may be associated with late muscle flap degeneration.

O presente estudo tem por finalidade analisar tardiamente o padrão anatômico evolutivo das artérias coronárias do coração transplantado. Para cumprir tal proposição foram selecionados 22 pacientes submetidos ao transplante cardíaco ortotópico, com seguimento pós-operatório superior a 36 meses. As variáveis eleitas para este fim foram aferidas anualmente até o quinto ano de pós-operatório. A análise da ventriculografia mostrou a estabilidade da fração de ejeção (p=0,99) em valores normais. A cineangiocoronariografia seqüencial evidenciou incidência crescente de lesões arteriais com comprometimento da função contrátil. As lesões obstrutivas acometeram as artérias coronárias difusamente, com predomínio no território distai. Os episódios de rejeição aguda e a etiologia da cardiomiopatia não modificaram a evolução natural da aterosclerose coronária.

The purpose of this study was to analise late anatomical status of coronary arteries in patients (pts) in whom orthotopic heart transplantation(OHT) was perfomed. We studied 22 pts submitted to OHT with post-operative follow-up longer than 36 months. They were evaluated by cinecoronariography every year in a 5 years follow-up. Ventriculography analisis showed stability (in normal values) of the left ventricular ejection fraction (p= 0,99) during the follow-up period. The sequential coronary studies showed a progressive increase on incidence of abnormalities in coronary arteries with disorders of contratile function. The obstructive abnormalities were present in all segments but more in distal territories. The acute rejection episodes and cardiomiopathy etiologies didn't change the natural evolution of coronary atherosclerosis.

A presente investigação tem o propósito de analisar, retrospectivamente, os resultados obtidos em 18 pacientes neonatos prematuros, submetidos a operação para oclusão da PCA, no período entre julho de 1990 e dezembro de 1993 (42 meses), sendo 12 (66,6%) pacientes do sexo feminino, com idade que variou entre 10 e 44 (20,8±8,3) dias, idade gestacional entre 26 e 28 (27,2±0,9) semanas. No dia da operação o peso dos pacientes esteve compreendido entre 700 e 1380 (985,8±181,6) gramas. A insuficiência respiratória aguda ocorreu em todos os pacientes sendo a principal indicação cirúrgica, estando em 6 (33,3%) pacientes associada à insuficiência cardíaca congestiva. A indometacina endovenosa foi utilizada no período pré-operatório em 9 (50%) pacientes na tentativa de se obter o fechamento farmacológico, sem sucesso, do canal arterial e apesar de não influenciar nos resultados pós-operatórios, apresentou como principal efeito secundário a redução significativa da diurese (p<0,001), que se normalizou após quatro dias do uso da droga. A técnica operatória empregada foi a toracotomia póstero-lateral esquerda trans-pleural com fixação de três ou quatro clipes metálicos para oclusão do canal arterial. O exame ecocardiográfico pré-operatório, realizado em 15 (83,2%) pacientes, mostrou que a relação entre o diâmetro do átrio esquerdo/aorta estava aumentada em todos os pacientes, sendo observado em 4 (22,2%) com até 30 dias da operação e apenas 1 (55%) paciente com quatro meses após a operação, evidenciando tendência de normalização da função cardíaca. Um (5,5%) paciente apresentou reabertura do canal arterial no pós-operatório imediato, sendo submetido à reoperação para ligadura do canal. Não foi observada diferença significativa no tempo necessário de intubação orotraqueal (IOT) (p=0,586) bem como do tempo de permanência com fração inspiratória de oxigênio (FIO2) < 40% e > 40% < 60% no período pré e pós-operatório (p=0,841 e p=0,692, respectivamente), mas com redução significativa do tempo de permanência com FIO2 > 60% (p=0,033). O período de internação hospitalar variou de 43 a 157 (96,0±24,8) dias. Não houve mortalidade operatória, observando-se ainda baixa morbidade com este método. As causas de óbito, no período pós-operatório, não estiveram relacionadas com o tratamento cirúrgico. Pode-se concluir que a ligadura cirúrgica da PCA, nos pacientes neonatos e prematuros, é método eficaz e seguro podendo ser praticado com baixa morbidade e mortalidade.

The purpose of our study was to analyse the results obtained in 18 neonate premature patients who underwent surgical closure of the patent ductus arteriosus, between July 1990 and December 1993 (42 months). Twelve (66.6%) patients were female, with age between 10 and 44 (20.8±8.3) days, gestacional age ranged from 26 to 28 (27.2±0.9) weeks. In the surgery day the birth weight was between 700 and 1380 (985.8 ±181.6) grams. Acute respiratory insufficiency was present in all patients, as the principal surgical indication. In six (33.3%) patients congestive cardiac insufficiency was present. The pharmacological treatment with indometacin, before the operation, was used in nine (50%) patients without success, and although have had no influence in the postoperative results, was associated with a significant reduction of the urine debit (p<0.001). The operative procedure to perform the ligation of the ductus arteriosus, was the fixation of three to four metallic clips, and it has proved to be easily praticable. The preoperative ecocardiography investigation was realized in 15 (83.2%) patients, showing an increase in the relationship between the left atrium and the aortic diameter in all the patients. This relationship was increased in only four (22.2%) patients 30 days after the operation, and only in one (5.5%) patient four months after the operation, showing a tendency of normalization of the cardiac function. In one (5.5%) patient there was a reopening of the ductus in immediate postoperative period, beeing submitted to a reoperation for the ligation of the ductus arteriosus. There was no significant difference in the period of orotraqueal intubation (p=0.586) as well as in the period with oxigen inspiratory fraction < 40% and > 40% < 60% in the pre and postoperative time, (p=0.841 and p=0.692, concerning), but with significant reduction in the period with oxigen inspiratory fraction > 60% (p=0.033). The period of hospitalization was comprehended between 43 and 157 (96.0±24.8) days. The causes of the mortality in the postoperative period have not been related to the surgical treatment. We conclude that the surgical ligation of the patent ductus arteriosus in neonate premature patients is an effective and safe method that can be performed with low morbidity and mortality.

Nas formas cardíacas da doença de Chagas que evoluem com insuficiência cardíaca refratária ao tratamento clínico, o transplante é a única alternativa, ao lado da cardiomioplastia. Os autores apresentam a evolução tardia de seis pacientes com miocardiopatia chagásica terminal submetidos a transplante cardíaco ortotópico. O período médio de observação foi de 25,2 meses. O diagnóstico de reativação da doença de Chagas apoiou-se na observação clínica, na investigação laboratorial do parasita, nas biópsias endomiocárdicas e dos nódulos de subcutâneo. A análise dos resultados demonstra que: 1) os testes laboratoriais mostraram-se ineficazes no diagnóstico da reativação da doença, sendo que as biópsias mostraram maior índice de positividade; 2) a pulsoterapia com corticóide predispõe à reativação; 3) a doença linfoproliferativa apresenta alta incidência na doença de Chagas, sendo a principal complicação tardia. Possivelmente, o benzonidazol apresente seu efeito oncogênico potencializado. Tendo em vista o caráter endêmico da doença e a falta de alternativa terapêutica, tornou-se obrigatória a analise do esquema imunossupressor, do tratamento da reativação e a maior experiência clínica, para posições mais definidas.

In the cardiac forms of Chagas' Disease that develop with refractory cardiac failure under clinical treatment, the transplant is the only alternative along with the cardiomyoplasty. The authors present the six patient late evolution with terminal chagasic myocardiopathy submitted under on orthopic heart transplantation. The average period of observation was of 25.2 months. The diagnosis of Chagas' Disease reativation relies on the clinical observation, laboratory investigation of parasito, endomyocardial biopsy and subcutaneous nodules. The analyses of the results show that: 1) the laboratory exams were useless in the diagnosis of the disease reativation, but the biopsy presented hight positivity; 2) the pulse therapy with steroid predisposes the reativation; 3) the lymphoproliferative disease presents hight incidence in the Chagas' Disease which is the main late complication; possibly the benzonidazol shows its potentially oncogenic effect. Having in mind the endemic character of the disease, the lack of alternative therapy becomes compulsory the analysis of immunosuppressive therapy, reativation treatment and increases the clinical experience to more defined position.

O transplante cardíaco tem tido ampla aplicação no tratamento da cardiomiopatia em fase terminal. Grande interesse existe no estudo das alterações hemodinâmicas imediatas e na identificação dos fatores que determinam essas alterações. Quarenta e três pacientes transplantados foram estudados com esse objetivo. Os seguintes dados foram obtidos: índice cardíaco, as pressões nas câmaras cardíacas, capilar pulmonar, aorta, artéria pulmonar, volume sistólico, fração de ejeção do ventrículo esquerdo (VE), resistência vascular pulmonar e sistêmica, índice do trabalho sistólico do ventrículo esquerdo e direito (VD) e o tríplice produto. Esses valores foram comparados de acordo com os episódios de rejeição e com diferentes valores do gradiente transpulmonar. Verificou-se que no pós-operatório imediato há depressão da função dos ventrículos decorrente de uma série de fatores. O índice cardíaco se mantém em valore adequados através de vários mecanismos e adaptação dos ventrículos, que ocorre mais precocemente para o VE do que para o VD. Tardiamente as alterações hemodinâmicas dependem do aparecimento de hipertensão arterial sistêmica e da aterosclerose coronária. A presença e os valores mais elevados do gradiente transpulmonar não tiveram influência estatisticamente significativa nas condições hemodinâmicas dos pacientes.

heart transplantation has been widely employed in treatment of end stage cardiomyopathy. Great interest exists in the study of early and late hemodynamic alterations following the procedure. Fourty three patients submited to heart transplantation were studied with this aim. The data obtained were: cardiac index, pressure in the heart chambers, capillary wedge pressure, aorta and pulmonary artery pressure, systolic volume and ejection fraction of left ventricle, pulmonary and systemic vascular resistances; left and right work systolic indexes; triple product. The data were compared in groups with and without rejection and in groups with higher and lower transpulmonary gradients. It was observed that in the immediate period there was impairment of ventricle function due to several factors. The cardiac index is maintained in adequate levels through several mechanisms, adaptation of the ventricles occur earlier in the right than in the left ventricle; lately hemodynamic alterations depend on the appearance of systemic arterial hypertension and coronary atherosclerosis. The presence of rejection and higher values for transpulmonary gradient had no statistically significant influence in hemodynamic condition.

Os autores apresentam a experiência de 27 pacientes portadores de cardiomiopatia em fase terminal, que foram submetidos a transplante cardíaco ortotópico, no período de março de 1985 a fevereiro de 1987 (23 meses), no Instituto do Coração do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. As principais indicações foram cardiomiopatia dilatada em 12 casos (44,5%), isquêmica em 10 casos (37,08%), chagásica em 4 casos (14,8%) e reumática em 1 caso (3,7%). A mortalidade imediata foi de 3,7% (1 caso), por falência do enxerto, e a tardia, de 7,4% (2 casos), por rejeição e endocardite. Atualmente, 19 pacientes (70,3%) encontram-se em classe funcional I, 1 paciente (3,7%) em classe funcional II e 4 (14,8%) em pós-operatório recente, embora já realizando exercício programado.

The authors present the innitial experience with 27 patients, submitted to orthotopic heart transplantation, during the period of 23 months (march 85 to february 87), at the Heart Institute, University of São Paulo Medical School. Indications were idiopathic cardiomiopathy in 12 cases (44.5%), ischemic cardiomiopathy in 10 cases (37%), Chagas' cardiomiopathy in 4 cases (14.8%) and rheumatic heart valve disease in 1 case (37%). The early mortality was 37% (1 case), caused by inespecific graft failure long distance hearvesting and the late mortality was 7.4% (2 cases), caused by rejection and infective endocarditis. The 24 surviving patients were (70.3%) in NYHA class I, 1 patient (37%) is in functional class II and 4 patients are in early post-operative period.

Entre 27 pacientes submetidos a transplante cardíaco, no Instituto do Coração do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, 4 eram portadores de cardiomiopatia chagásica. As idades situaram-se entre 28 e 43 anos. Três pacientes estavam em grupo funcional IV e 1, na classe funcional II, mas apresentando taquiarritmia ventricular retrataria. A imunossupressão foi feita com ciclosporina mais corticóide, no 1º paciente, e ciclosporina, azatioprina e corticóide, nos outros três. Três pacientes tiveram episódios de rejeição, tratados com pulsoterapia. Um destes pacientes apresentou doença intestinal linfoproliferativa precoce. Os 3 pacientes tiveram reagudização da doença de Chagas no 59º, 81º e 420º de pós-operatório, respectivamente, caracterizada por febre, lesões cutâneas e miocardite. O Trypanosoma cruzi foi encontrado nas biópsias de pele dos 3 pacientes e nas biópsias do miocárdio em 2 pacientes. Um dos pacientes não teve reagudização. Todos os 4 doentes foram tratados com medicação específica, bem tolerada em todos. As alterações da reagudização da doença de Chagas reverteram em poucos dias. Um dos pacientes, que teve doença linfoproliferativa e cujas doses de imunossupressores foram reduzidas, faleceu no 197º dia, em crise de rejeição. Os demais estão bem e em seguimento por 107, 160 e 500 dias, respectivamente.

Among 27 heart transplanted patients, 4 had chagasic cardiomiopathy. They were male, with ages from 28 to 43 years, 3 in the NYHA class IV and 1 in class II, but with refractory arrhythmias. The immunosuppressive protocol included cyclosporin and steroids in the first patient and cyclosporin, azathioprine and steroids in the last. Three patients had one or more rejection episodes and 1 had no episode. Usual complications were observed and in 1 patient a lymphoproliferative intestinal disease occurred. Three patients had a new acute phase of Chagas disease in the 59th, 81rst and 420th postoperative days with fever, skin lesions and myocarditis. Trypanosoma cruzi were found in skin lesion in all 3 patients and in myocardial byopsies in 2 patients. The laboratory tests were not the expected for the acute phase. All the alterations were reversed by specific therapy in a few days. One patient died in the 197th postoperative day due to rejection; the immunosuppression regimen had been diminished due to the lymphoproliferative disease; the others were well 107, 160 and 500 days, respectively, after the transplant.

No Instituto do Coração, de março de 1985 a fevereiro de 1986,11 pacientes foram submetidos a transplante cardíaco ortotópico. Eram todos do sexo masculino, com idade variando de 39 a 54 anos; 6 com cardiopatia isquémica, 4 com cardiomiopatia dilatada e um com cardiomiopatia chagásica. Foi realizado estudo hemodinâmico através de catéter de Swan-Ganz, no pré-operatório, no pós-operatório, após estabilização na unidade de recuperação, e trinta ou mais dias após o transplante. Os dados mostram melhora progressiva em relação ao índice cardíaco, pressão em artéria pulmonar, pressão de capilar pulmonar, resistência vascular pulmonar e resistência vascular sistêmica. Três dos 11 pacientes apresentaram disfunção renal transitória no pós-operatório imediato e que regrediram até o 15º dia, enquanto que 2 pacientes apresentaram aumento moderado da creatinina plasmática. Apenas 3 pacientes não apresentaram qualquer episódio de rejeição; nos demais, esses episódios foram um diagnóstico histológico sem repercussões clínicas. Complicações infecciosas ocorreram em 9 pacientes e foram de fácil controle clínico. No pós-operatório tardio, a hipertensão esteve presente em 8 pacientes, sendo mais acentuada em 2 deles. Não houve óbitos, nesta série de pacientes; todos estão assintomáticos e os 6 primeiros estão trabalhando.

At the Instituto do Coração, University of São Paulo Medical School, 11 patients were submitted to heart transplantation from march 1985 up to february 1986. All were male, with ages of 39-59 years, 6 with coronary heart disease, 4 with dilated cardiomyopathy and 1 with Chagas cardiomyopathy. The patients were studied hemodynamically with a Swan-Ganz catheter pre-operatively, at the arrival in the intensive care unit, in the first postoperative day and 30 or more days after the transplant. The data showed that there was a progressive increase of cardiac index and decreases of pulmonary artery pressure, capilary pulmonary wedge pressure, pulmonary vascular resistance and systemic vascular resistance. Three of the 11 patients had immediate renal dysfunction that returned to normal by the 15th day. Late postoperatively 2 patients had increase of creatinine levels. Only 3 patients had no rejection episodes; among the others these episodes were represented by hystological alterations with no clinical manifestations. Infections complications occurred in 9 patients and were easily clinically treated. Late postoperatively, hypertension was present in 8 patients; in 2 of them it was moderate. There was no death in these 11 patients; all are symptom free and the first 6 are working.
