ABSTRACT: Using portable blood glucose meters (PBGMs) to measure blood glucose (BG) concentration is a common procedure in veterinary practice. Our objective was to evaluate the analytical and clinical accuracy of a human PBGM (Accu-Chek Performa®), (AC) and a veterinary PBGM (GlucoCalea®), (GC) in feline patients. Central venous blood samples were collected from 48 cats at a Brazilian Veterinary teaching hospital. Two devices from each model were used and compared to a reference method (RM). Analytical accuracy was assessed according to ISO 15197:2013 requirements for human PBGMs. Data were compared using Wilcoxon’s nonparametric test and represented by Bland-Altman plots. Hematocrit’s effect on BG measurements was evaluated by the Spearman correlation coefficient. Clinical accuracy was determined using error grid analysis (EGA). Values of BG were significantly higher in all PBGMs compared to the RM. Although ISO’s analytical accuracy requirements could not be met by any of the devices, AC meters were more accurate than GC meters. All AC measurements - but not GC ones - were within zones A and B of the EGA, meeting ISO requirements for clinical accuracy. Significant hematocrit interference was observed in all devices. Therefore, AC showed greater accuracy compared to GC using feline whole blood samples.

RESUMO: O uso de glicosímetros portáteis (GPs) para aferição da glicemia é um procedimento comum na rotina clínica veterinária. O objetivo deste trabalho é avaliar a acurácia analítica e clínica de um GP humano (Accu-Chek Performa®), (AC) e um GP veterinário (GlucoCalea®), (GC) em gatos. Amostras de sangue venoso central foram coletadas de 48 gatos atendidos em um hospital veterinário-escola no Brasil. Foram utilizados dois GPs de cada modelo e comparados a um método de referência (MR). A acurácia analítica foi avaliada de acordo com os requisitos estipulados pela ISO 15197:2013 para GPs de uso humano. Os dados foram comparados pelo teste não-paramétrico de Wilcoxon e representados em gráficos de Bland-Altman. O efeito do hematócrito sobre os valores de glicemia foi avaliado pelo coeficiente de correlação de Spearman. A acurácia clínica foi avaliada pela análise da grade de erros (AGE). Em comparação com o MR, os valores de glicemia foram maiores em todos os GPs avaliados. Nenhum deles atendeu aos requisitos da ISO quanto à acurácia analítica, mas o AC mostrou-se mais acurado que o GC. Todos os valores de glicemia obtidos pelos GPs humanos - mas não pelos GPs veterinários - estiveram dentro das zonas A e B da AGE, demonstrando acurácia clínica de acordo com as exigências da ISO. A interferência do hematócrito da amostra mostrou-se significativa em todos os aparelhos testados. Portanto, o AC apresentou maior acurácia quando comparado ao GC em amostras de sangue total em felinos.

Abstract: Introduction: In the last decades, pedagogical practices and teaching methodologies have experienced radical changes, culminating today with the exponential growth of active teaching methodologies. Despite the growing use of these active methodologies in medical courses, the theoretical evaluation models changed little, if at all, in the last decades, which should not occur, as the evaluation systems need to evolve concomitantly to ensure a cohesive, quality teaching-learning process. Objective: To demonstrate how open-book examination is inserted in medical courses. Methods: An integrative review was developed by searching for publications indexed in the last 10 years in the databases of the U.S. National Library of Medicine (PubMed), “Scientific Electronic Library Online” (SciELO), Science Direct, and the “Education Resources Information Center” (ERIC), as full articles written in Portuguese or English. The following descriptors were used in combination, respectively in English and Portuguese: “medicine”, “assessment”, “open book examination”, “open book exam”, “open book test” and “open book assessment”; “medicina”, “avaliação”, “prova com consulta”, “avaliação com consulta” and “teste com consulta”. Results: For this review, a total of eleven publications that met the eligibility criteria were selected, and after their theoretical immersion and analysis, two main categories emerged: “Open-book examination as an educational resource for medical education” and “Open-book examination as a pedagogical alternative in the pandemic scenario”, which were analyzed in the light of an integrative review. Conclusion: Although there are still doubts and further studies are required, it is evident that Open-Book examination is very pertinent to medical education, as it contributes to the training of professionals with high capacity for questioning and debating, who are prepared for permanent study and education routines, and who understand that there is constant evolution of knowledge in the medical field.

Resumo: Introdução: Nas últimas décadas, as práticas pedagógicas e as metodologias de ensino têm experimentado mudanças radicais, culminando hoje com o crescimento exponencial de metodologias de ensino ativas. Apesar da crescente utilização dessas metodologias ativas nos cursos de Medicina, as formas de avaliação teórica foram pouco, ou nada, alteradas nas últimas décadas, o que não deveria ocorrer, pois os sistemas de avaliação precisam evoluir concomitantemente para garantir um processo de ensino-aprendizagem coeso e de qualidade. Objetivo: Este estudo evidencia como está inserida a avaliação com consulta em cursos de Medicina. Método: Foi desenvolvida uma revisão integrativa por meio de consulta de publicações indexadas no período dos últimos dez anos nas bases de dados da Biblioteca Nacional de Medicina dos Estados Unidos (PubMed), Scientific Electronic Library Online (SciELO), ScienceDirect e Education Resources Information Center (ERIC), em formato de artigos completos nas línguas portuguesa ou inglesa. Utilizaram-se os descritores a seguir de forma combinada, respectivamente em inglês e português: “medicine”, “assessment”, “open book examination”, “open book exam”, “open book test” e “open book assessment”; “medicina”, “avaliação”, “prova com consulta”, “avaliação com consulta” e “teste com consulta”. Resultado: Para a composição deste estudo, selecionaram-se 11 publicações que atenderam aos critérios de elegibilidade traçados, e, após a imersão teórica e análise delas, emergiram duas categorias principais: “A avaliação com consulta como recurso educacional para o ensino médico” e “A avaliação com consulta como alternativa pedagógica no cenário pandêmico”, as quais foram analisados à luz de uma revisão integrativa. Conclusão: Ainda que haja dúvidas e sejam necessários novos estudos, fica evidente que a avaliação com consulta é muito pertinente ao ensino médico, uma vez que contribui para a formação de profissionais com alta capacidade de questionamento e argumentação, preparados para rotinas de estudo e educação permanentes e que entendam que há constante evolução do conhecimento na área médica.

Abstract Background The extent of cardiac damage associated with aortic stenosis has important prognostic implications after transcatheter aortic valve replacement (TAVR). However, the role of tricuspid regurgitation (TR) in this clinical setting is still unclear. Objectives To explore the association between TR and mortality in patients undergoing TAVR and assess changes in TR severity post TAVR and its relationship with short and mid-term mortality. Methods Relevant databases were searched for articles published from inception until August 2020. Out of 414 screened studies, we selected 24 that reported the degree of TR pre or post TAVR. The primary outcome was all-cause mortality, and random effects meta-analysis models were conducted (at a significance level of 5%). Results Seventeen studies reported associations between pre-TAVR TR and all-cause mortality (> 45,000 participants) and thirteen accessed TR severity post TAVR (709 participants). Moderate/severe baseline TR was associated to higher all-cause mortality both at 30 days (HR 1.65; 95% CI, 1.20-2.29) and 1.2 years (HR 1.56; 95% CI, 1.31-1.84). After TAVR, 43% of patients presented a decrease of at least one grade in TR (30 days, 95% CI, 30-56%), sustained at 12.5 months in 44% of participants (95% CI, 35-52%). Persistence of significant TR was associated with a two-fold increase in all-cause mortality (HR 2.12; 95% CI, 1.53-2.92). Conclusions Significant TR pre TAVR is associated with higher mortality. Although TR severity may improve, the persistence of significant TR post TAVR is strongly associated with increased mortality. Our findings highlight the importance of a detailed assessment of TR pre and post TAVR and might help identify patients who may benefit from more careful surveillance in this scenario.

Resumo Fundamento A extensão do dano cardíaco associada à estenose aórtica tem importantes implicações prognósticas após a substituição da valva aórtica transcateter (TAVR). Contudo, ainda não está claro qual é o papel da insuficiência tricúspide (IT) nesse cenário clínico. Objetivos Explorar a associação entre IT e mortalidade em pacientes submetidos a TAVR e avaliar as alterações na gravidade da IT após a TAVR e sua relação com mortalidade de curto e médio prazo. Métodos Foram feitas pesquisas em bases de dados relevantes de artigos publicados do início até agosto de 2020. Dos 414 estudos triados, selecionamos 24 que relataram o grau de IT pré- ou pós-TAVR. O desfecho primário foi mortalidade por todas as causas, e foram conduzidos modelos de metanálise de efeitos aleatórios (a um nível de significância de 5%). Resultados Dezessete estudos relataram associações entre IT pré-TAVR e mortalidade por todas as causas (> 45.000 participantes), e 13 avaliaram a gravidade da IT pós-TAVR (709 participantes). A IT basal moderada/grave foi associada a maior mortalidade por todas as causas em 30 dias [razão de risco (RR) 1,65; intervalo de confiança (IC) 95% 1,20-2,29] e 1,2 ano (RR 1,56; IC95% 1,31-1,84). Após a TAVR, 43% dos pacientes apresentaram redução de pelo menos um grau na IT (30 dias, IC95% 30-56%), que se sustentou em 12,5 meses em 44% dos participantes (IC95% 35-52%).A persistência de IT significativa foi associada a um aumento de duas vezes na mortalidade por todas as causas (RR 2,12; IC95% 1,53-2,92). Conclusões A IT significativa pré-TAVR está associada a maior mortalidade. Ainda que a gravidade da IT possa melhorar, a persistência de IT significativa após a TAVR está fortemente associada ao aumento da mortalidade. Nossos achados destacam a importância de uma avaliação detalhada da IT pré- e pós-TAVR e podem ajudar a identificar pacientes que possam se beneficiar de uma vigilância mais cuidadosa nesse cenário.

SUMMARY OBJECTIVE: The aim of this study was to evaluate the correlation between P-wave indexes, echocardiographic parameters, and CHA2DS2-VASc score in patients without atrial fibrillation and valvular disease. METHODS: This retrospective cross-sectional study included patients of a tertiary hospital with no history of atrial fibrillation, atrial flutter, or valve disease and collected data from June 2021 to May 2022. The exclusion criteria were as follows: unavailable medical records, pacemaker carriers, absence of echocardiogram report, or uninterpretable ECG. Clinical, electrocardiographic [i.e., P-wave duration, amplitude, dispersion, variability, maximum, minimum, and P-wave voltage in lead I, Morris index, PR interval, P/PR ratio, and P-wave peak time], and echocardiographic data [i.e., left atrium and left ventricle size, left ventricle ejection fraction, left ventricle mass, and left ventricle indexed mass] from 272 patients were analyzed. RESULTS: PR interval (RHO=0.13, p=0.032), left atrium (RHO=0.301, p<0.001) and left ventricle diameter (RHO=0.197, p=0.001), left ventricle mass (RHO=0.261, p<0.001), and left ventricle indexed mass (RHO=0.340, p<0.001) were positively associated with CHA2DS2-VASc score, whereas P-wave amplitude (RHO=-0.141, p=0.02), P-wave voltage in lead I (RHO=-0.191, p=0.002), and left ventricle ejection fraction (RHO=-0.344, p<0.001) were negatively associated with the same score. The presence of the Morris index was associated with high CHA2DS2-VASc (p=0.022). CONCLUSION: Prolonged PR interval, Morris index, increased left atrium diameter, left ventricle diameter, left ventricle mass, and left ventricle indexed mass values as well as lower P-wave amplitude, P-wave voltage in lead I, and left ventricle ejection fraction values were correlated with higher CHA2DS2-VASc scores.

Resumo: A partir de uma revisão bibliográfica, o artigo propõe a verificação de aspectos históricos da imprensa brasileira com o objetivo de observar características da censura ao jornalismo no país. A pesquisa parte do questionamento a respeito das estratégias adotadas principalmente por governos, observando se as ações correspondem a fatos conjunturais ou se são resultantes de uma tradição, carregada de traços históricos e culturais que se mostram adaptáveis a diferentes períodos. Conforme observado, a censura não ocorre em momentos exclusivos da história brasileira, mas acompanha a atividade jornalística desde os primórdios. O exercício busca estabelecer relações entre a bibliografia brasileira e portuguesa que podem contribuir para a compreensão das particularidades do jornalismo atual produzido no país.

Abstract: Based on a bibliographical review, the article proposes to verify historical aspects of the Brazilian press in order to observe characteristics of journalism censorship in the country. The research starts from the questioning about the strategies adopted mainly by governments, observing if the actions correspond to conjunctural facts or if they are the result of a tradition, loaded with historical and cultural traits that are adaptable to different periods. As noted, censorship does not occur at exclusive moments in Brazilian history, but has accompanied journalistic activity since the beginning. The exercise seeks to establish relationships between Brazilian and Portuguese bibliography that can contribute to understanding the particularities of current journalism produced in the country.

Resumo As Áreas de Proteção Ambiental (APA) são Unidades de Conservação (UC) de Uso Sustentável que possuem características naturais relevantes da paisagem, as quais devem ser preservadas. No entanto, essa categoria de manejo geralmente tem apresentado muitas dificuldades e limitações em virtude do seu baixo grau de restrição ambiental, sofrendo diversas críticas oriundas principalmente dos seus objetivos e da sua eficiência de conservação. Esses fatores podem ocasionar maior pressão sobre os habitats naturais e, eventualmente, gerar níveis consideráveis de vulnerabilidade em seu território. Nesse sentido, o objetivo do presente trabalho foi diagnosticar a vulnerabilidade ambiental das APAs inseridas no domínio do Bioma Mata Atlântica na região sudeste brasileira. Com auxílio do software ArcGIS, foram utilizados dados em Sistema de Informação Geográfica (SIG) de declividade, geologia, pedologia e uso e ocupação da terra para a determinação do Índice de Vulnerabilidade Ambiental. Os resultados mostraram que as áreas estudadas se enquadram predominantemente nas classes de baixa e média vulnerabilidade. Apesar desse cenário, é necessário implantar ações e medidas que visem reduzir a degradação das áreas de vegetação nativa remanescente, em virtude do avanço principalmente das atividades agrícolas e urbanas existentes nas APA. Portanto, conclui-se que o presente estudo é essencial para o planejamento territorial, podendo subsidiar informações para os planos de manejo dessa e outras categorias de UC.

Abstract Environmental Protection Areas (EPA) are Conservation Units (CU) for Sustainable Use that have relevant natural characteristics of the landscape, which must be preserved. However, this management category has generally presented many difficulties and limitations due to its low degree of environmental restriction, suffering several criticisms arising mainly from objectives and its conservation efficiency. These factors can cause greater pressure on natural habitats, and, eventually, generate considerable levels of vulnerability in their territory. In this sense, the objective of the present work was to diagnose the environmental vulnerability of the EPA inserted in the domain of the Atlantic Forest Biome in the southeastern region of Brazil. With the aid of the ArcGIS software, data were used in the Geographic Information System (GIS) of slope, geology, pedology and land use and occupation to determine the Environmental Vulnerability Index. The results showed that the areas studied fall predominantly into the low and medium vulnerability classes. Despite this scenario, it is necessary to implement actions and measures that aim to reduce the degradation of the areas of remaining native vegetation, due to the advance mainly of the existing agricultural and urban activities in the study area. Therefore, it is concluded that the present study is essential for territorial planning, subsidizing information for the management plans for this and other CU categories.

SUMMARY OBJECTIVE: Risk stratification of sudden cardiac death in patients with coronary artery disease is of great importance. We evaluated the association between ventricular repolarization and induction of malignant ventricular arrhythmias on electrophysiological study of patients with coronary artery disease. METHODS AND RESULTS: A total of 177 patients (65±10.1 years, 83.6% male, mean left ventricular ejection fraction [LVEF] 37.5±13.6%) were analyzed. For each 10 ms increment in the QT interval, there was a 7% increase in malignant ventricular arrhythmias inducibility; QT cutoff point of 452 ms had an accuracy of 0.611 for predicting malignant ventricular arrhythmias (p=0.011). Male gender (odds ratio [OR]=4.18, p=0.012), LVEF <35% (OR=2.32, p=0.013), amiodarone use (OR=2.01, p=0.038), and prolonged QT (OR=1.07, p=0.023) were associated with malignant ventricular arrhythmias. In patients with ventricular dysfunction, QT >452 ms was associated with significantly increased risk of malignant ventricular arrhythmias (OR=5.44, p=0.0004). In those with LVEF ³35%, QT dispersion (QTd) was significantly higher in patients with inducible malignant ventricular arrhythmias. QTd >20 ms had 0.638 accuracy and 81.3% negative predictive value in predicting malignant ventricular arrhythmias. CONCLUSION: QT interval is an independent factor associated with malignant ventricular arrhythmias in patients with coronary artery disease. The combination of ventricular dysfunction and prolonged QT interval is associated with a 5.44-fold increase of malignant ventricular arrhythmias induction. Male gender, amiodarone use, and decreased left ventricular ejection fraction are also associated with increased risk of inducibility of malignant ventricular arrhythmias on the electrophysiological study.

Resumo: O artigo objetiva refletir sobre as concepções de gênero, poder, multiplicidade e multiculturalismo, em María Lugones, analisando o modo como sua teoria se associa ao pensamento pós-colonial. Para tanto, aborda a perspectiva do pensamento pós-colonial e a noção de colonialidade do poder, considerando o sistema moderno/colonial de gênero. As teorias pós-coloniais criticam o paradigma epistemológico do Ocidente e a hierarquização baseada na distinção entre humanos e não humanos, colonizador e colonizado. Em sua análise do sistema moderno/colonial, María Lugones introduz a ideia de gênero na reflexão acerca das relações de poder. A teoria de Oyèronké Oyӗwùmí é uma referência para María Lugones, que desenvolve uma concepção de intersecção de raça, classe, gênero e sexualidade, propondo um feminismo decolonial baseado na identificação de formas de resistência e coalizão para emancipação.

Abstract: The article reflects on María Lugones’s concepts of gender, power, multiplicity, and multiculturalism, analyzing the way her theory is associated with postcolonial thought. In order to do so, it aproaches the perspective of postcolonial thought, and the notion of coloniality of power, considering the colonial/modern gender system. The postcolonial theories criticize the Western epistemological paradigm, and hierarchy based on the distinction between human and non-human, colonizer and colonized. When analyzing the colonial/modern system, María Lugones’s perspective introduces the concept of gender in the analyses of the power relationships. Oyèronké Oyӗwùmí’s theory of gender is a reference to María Lugones. In the analysis of the coloniality of power, Lugones develops the idea of the intersection of race, class, gender, and sexuality, proposing decolonial feminism based on the identification of forms of resistance, and coalition in social movements for emancipation.

Resumo Fundamento A fibrilação atrial (FA) é classificada, de acordo com a amplitude das ondas fibrilatórias (f), em ondas finas (FAf) e ondas grossas (FAg). Objetivos Correlacionar a amplitude das ondas f com variáveis clínicas, laboratoriais, eletrocardiográficas e ecocardiográficas que indiquem alto risco de tromboembolismo e avaliar o seu impacto no sucesso da cardioversão elétrica (CVE). Métodos Estudo retrospectivo, observacional, que incluiu 57 pacientes com FA não valvar persistente submetidos a CVE. A amplitude máxima das ondas f foi aferida na derivação V1. FAg foi definida quando f≥1,0 mm e FAf quando f<1,0mm. Os achados foram correlacionados com as variáveis indicadas. Valores de p<0,05 foram considerados estatisticamente significativos. Resultados FAg (n=35) associou-se a maior sucesso na CVE (94,3% vs. 72,7%, p=0,036) mesmo após ajuste para variáveis como idade e IMC (p=0,026, OR=11,8). Pacientes com FAf (n=22) necessitaram mais choques e maior energia para reversão ao ritmo sinusal (p=0,019 e p=0,027, respectivamente). Não houve associação significativa entre a amplitude das ondas f e parâmetros clínicos, ecocardiográficos e laboratoriais. Conclusões A amplitude de f não se associou a parâmetros ecocardiográficos, clínicos e laboratoriais que indicam alto risco de tromboembolismo. FAg associou-se a maior chance de sucesso na reversão ao ritmo sinusal por meio da CVE. Maior número de choques e energia foram necessários para reversão ao ritmo sinusal em pacientes com FAf.

Abstract Background Atrial fibrillation (AF) is classified according to the amplitude of fibrillatory waves (f) into fine waves (fAF) and coarse waves (cAF). Objectives To correlate the amplitude of f waves with clinical, laboratory, electrocardiographic, and echocardiographic variables that indicate a high risk of thromboembolism and to assess their impact on the success of electrical cardioversion (ECV). Methods Retrospective, observational study that included 57 patients with persistent non-valvular AF who underwent ECV. The maximum amplitude of f waves was measured in lead V1. cAF was defined when f ≥ 1.0mm and fAF when f < 1.0mm. The findings were correlated with the indicated variables. Values of p < 0.05 were considered statistically significant. Results cAF (n = 35) was associated with greater success in ECV (94.3% vs. 72.7%, p = 0.036) even after adjusting for variables such as age and BMI (p = 0.026, OR = 11.8). Patients with fAF (n = 22) required more shocks and more energy to revert to sinus rhythm (p = 0.019 and p = 0.027, respectively). There was no significant association between f-wave amplitude and clinical, echocardiographic, and laboratory parameters. Conclusions The amplitude of f wave was not associated with echocardiographic, clinical and laboratory parameters that indicate a high risk of thromboembolism. cAF was associated with a higher chance of success reverting to sinus rhythm employing ECV. A greater number of shocks and energy were required for reversion to sinus rhythm in patients with fAF.

Abstract To evaluate possible new drugs for the treatment of Colorectal Carcinoma (CRC) using in silico tools was the main objective of this study. The method of analysis used was the in silico evaluation of tumor markers and their interaction with selected drugs, through the study of its pharmacokinetic and pharmacodynamic characteristics. A potential therapeutic target pointed out in this study was the Cell Division Cycle 25 B (CDC25B), belonging to the CDC25 phosphatase family. Overexpression of CDC25 phosphatases is often associated with a wide variety of cancers. In addition, CDC25B is an oncogenic protein that induces neoplastic transformation. In CRC, CDC25B is overexpressed to activate the CDC2/cyclin B complex and improve the growth and survival of these tumors. Four drugs were identified for evaluation, with α-amyrin being selected for docking, because it was that had the best characteristics according to the methodology used. The α-amyrin ligand obtained the interaction energy value of -7.6 G (Kcal/mol), while the standard CDC25B ligand obtained -10.0 G (Kcal/mol). TThe results showed that the CDC25B protein was the only structure cocrystallized with α-amyrin and presented favorable outcomes in docking, being a candidate for further studies for its use in the CRC targeted therapy.

RESUMO: A mensuração do hormônio adrenocorticotrófico endógeno (ACTHe) é útil na investigação do hipercortisolismo e hipoadrenocorticismo. No entanto, como o hormônio é bastante instável, as amostras de sangue necessitam um manejo adequado no processamento e armazenamento, assim como o envio para laboratórios pode ser um passo limitante, uma vez que poucos laboratórios oferecem este ensaio no Brasil. O objetivo deste trabalho foi comparar a estabilidade pré-analítica do ACTHe durante longo período de envio de amostras congeladas em gelo seco (GS), ou com barras de gelo reciclável (BGR). Um total de 56 amostras pareadas para mensuração de ACTHe foram analisadas. As amostras de sangue foram adequadamente manejadas, sendo o plasma transferido para micro tubos plásticos estocados a -80ºC. Os 56 pares de amostras foram enviados à tarde para chegada ao laboratório na manhã seguinte em dois isopores, um com GS e outro com BGR. Apesar de uma alta correlação entre os resultados dos diferentes métodos de envio (r Spearman = 0,958, P<0,001), o teste de Wilcoxon para amostras pareadas mostrou que o método de envio influencia os resultados P<0,001). Apesar desta diferença, os resultados não afetaram a interpretação dos resultados. Desta forma, quando o envio em gelo seco não for possível, o envio das amostras com barras de gelo reciclável pode representar um risco à estabilidade pré-analítica do ACTHe.

ABSTRACT: Endogenous adrenocorticotrophic hormone (eACTH) measurement is useful in hypercortisolism and hypoadrenocorticism investigation; however, since the hormone is highly unstable, blood samples require proper processing and storage, as well as shipping is often a step limiting since few laboratories offer this assay in Brazil. The aim of this note was to compare overnight dog´s eACTH preanalytical stability when frozen samples were shipped in dry ice (DI), or with recyclable ice bars (RIB). A total of 56 paired samples for eACTH measurement were analyzed. Blood samples were properly handled, plasma aliquots transferred into plastic microtubes, and stored at -80ºC. The fifty-six paired samples were overnight shipped in two thermic isolated boxes with DI or involved by RIB. Despite there was a high correlation between results from both shipping methods (r Spearman = 0.958, P<0.001), the Wilcoxon matched-pairs rank test showed that the shipping method may influence results (P<0.001). However, this difference does not affect results interpretation. By this way, when DI shipping was not possible, RIB shipping may represent a risk to eACTH preanalytical stability.

RESUMO. Apesar dos avanços no diagnóstico de demência, a avaliação neuropsicológica continua sendo uma importante ferramenta. A Escala de Avaliação de Demência de Mattis (Mattis Dementia Rating Scale — DRS) foi projetada para avaliar pessoas com suspeita de demência e permite a análise de diferentes domínios cognitivos. Dados os numerosos casos de demência com início precoce, são necessárias normas específicas para a população de meia-idade. Objetivo: Fornecer dados normativos para a população brasileira de meia-idade na escala Mattis e investigar a influência da escolaridade, idade, sexo e quociente de inteligência (QI) nos resultados. Métodos: 120 funcionários e cuidadores saudáveis de um hospital, com idade entre 45 e 64 anos e com pelo menos quatro anos de escolaridade foram incluídos no estudo, os quais foram divididos igualmente em seis grupos. Havia três níveis educacionais para cada faixa etária (45–54 e 55–64 anos): 4 a 7 anos de estudo (4–7), 8 a 11 (8–11) e 12 anos ou mais (12+). Os resultados são apresentados como valores médios, desvios padrão e percentis. Foram realizadas comparações entre os grupos de acordo com idade, sexo e escolaridade. Idade, anos de estudo e QI foram também analisados como variáveis contínuas através da correlação de Spearman. Resultados: Para a educação, a comparação entre os grupos mostrou diferenças nos resultados da escala total e subescalas, exceto na subescala Construção. Não foram identificadas diferenças para idade e sexo. As correlações observadas para anos de estudo e QI foram semelhantes. Nenhuma correlação foi encontrada para idade. Conclusão: O presente estudo contribui para a avaliação da demência em indivíduos com menos de 65 anos e reforça a importância de considerar o nível educacional na interpretação dos escores.

ABSTRACT. Despite the advances in the diagnosis of dementia, neuropsychological assessment remains an important tool. The Mattis Dementia Rating Scale (DRS) was designed to evaluate people with suspected dementia and allows for the analysis of different cognitive domains. Considering the numerous cases of early-onset dementia, specific reference standards aimed at the middle-aged population are necessary. Objective: To provide normative data for the middle-aged Brazilian population in DRS and to investigate the influence of education level, age, sex, and intelligence quotient (IQ) on the results. Methods: Overall, 120 healthcare professionals and caregivers from a hospital, who were healthy, aged between 45 and 64 years, and had at least four years of formal education, were included in the study. They were equally divided into six groups. In each age group (45–54 and 55–64 years), there were three educational levels: 4–7, 8–11, and 12 or more (12+) years of formal education. The results are presented as mean values, standard deviations, and percentiles. Comparisons between groups were carried out for age, sex, and education level. Age, years of formal education, and IQ were also analyzed as continuous variables by Spearman's correlation. Results: Concerning education level, the comparison between groups showed differences in the results for the total scale and subscales, except for the Construction subscale. No differences were found for age and sex. Correlations observed for years of formal education and IQ were similar. No correlation was found for age. Conclusions: The present study contributes to the evaluation of dementia concerning people younger than 65 years of age and reinforces the importance of education in the interpretation of the scores.

RESUMO Estudos normativos de testes neuropsicológicos têm sido realizados no Brasil nos últimos anos. Entretanto, devido à heterogeneidade da educação da população brasileira, dados adicionais são necessários. Objetivo: O presente estudo fornece dados normativos de testes de funções executivas para brasileiros de meia-idade e investiga a influência da idade, sexo, educação e quociente de inteligência (QI) sobre o desempenho nestes testes. Métodos: Um total de 120 profissionais e cuidadores saudáveis de um hospital, selecionados aleatoriamente, foram submetidos aos testes de Fluência – animais e FAS – Teste de Trilhas (TT) e Teste do Desenho do Relógio (TDR). Foram divididos em seis grupos de 20, sendo dois por idade (45-54; 55-64) e três por anos de estudo (4-7; 8-11; 12+ anos). Resultados: Dados normativos são apresentados em médias e percentis. A educação influenciou as diferenças nos testes, exceto no TDR. Análises post-hoc evidenciaram diferenças entre os três níveis educacionais em TT e FAS. Surgiram diferenças por idade no TT e Fluência – letra F. Foi encontrada correlação moderada entre escolaridade e resultados do TT e Fluência. As correlações para QI foram semelhantes. Conclusão: O estudo fornece dados normativos para brasileiros de meia-idade, com quatro ou mais anos de escolaridade, em testes cognitivos frequentemente utilizados para avaliação de funções executivas. Os resultados confirmam a forte influência da educação, mesmo na comparação entre os níveis médio e superior.

ABSTRACT Normative studies of neuropsychological tests were performed in Brazil in recent years. However, additional data are needed because of the heterogeneity of education of the Brazilian population. Objective: The present study provides normative data of executive function tests for middle-aged Brazilians and investigates the influence of age, sex, education and intelligence quotient (IQ) on performance in these tests. Methods: A total of 120 healthy staff and caregivers from a hospital were randomly selected and submitted to Fluency – animals and FAS, Trail Making Test (TMT) and Clock Drawing Test (CDT). They were divided into six groups of 20: two groups for age (45-54 and 55-64 years) and three groups for years of schooling (4-7; 8-11; 12+ years). Results: Normative data are presented in mean values and percentiles. Education influenced differences in the tests, except the CDT. Post hoc analyses revealed differences between the three educational levels on the TMT and FAS. Age differences emerged on the TMT and fluency letter F. Moderate correlation was found between schooling and results on TMT and Fluency. The correlations for IQ were similar. Conclusion: This study provides normative data for middle-aged Brazilians with four or more years of schooling in frequently used cognitive tests to assess executive functions. The results confirm the strong influence of education, even in the comparison between middle and higher levels.

OBJECTIVE: To report initial experience from the use of extracorporeal membrane oxygenation (ECMO) in patients who received lung transplantation. METHODS: Retrospective study of a single tertiary center in the Brazilian state of São Paulo, a national reference in lung transplantation, based on the prospective collection of data from electronic medical records. The period analyzed extended from January 2009 (beginning of the program) until December 2018. RESULTS: A total of 75 lung transplants were performed, with ECMO used in 8 (10.7%) cases. Of the patients, 4 (50%) were female. The mean age was 46.4±14.3 years. The causes of the end-stage lung disease that led to transplantation were pulmonary arterial hypertension in 3 (37.5%) patients, bronchiectasis in 2 (25%) patients, pulmonary fibrosis in 2 (25%) patients, and pulmonary emphysema in 1 (12.5%) patient. In our series, 7 (87.5%) cases were sequential bilateral transplantations. Prioritization was necessary in 4 (50%) patients, and in 1 patient, ECMO was used as a bridge to transplantation. The ECMO route was central in 4 (50%), peripheral venovenous in 2 (25%) and peripheral venoarterial in 2 (25%) patients. The mean length of the intensive care unit (ICU) stay was 14±7.5 days and of the hospital stay was 34.1±34.2 days. The mean ECMO duration was 9.3±6.6 days with a 50% decannulation rate. Three patients were discharged (37.5%). CONCLUSION: Lung transplantation requires complex treatment, and ECMO has allowed extending the indications for transplantation and provided adjuvant support in the clinical management of these patients.

RESUMO: A caracterização clínica-epidemiológica de doenças auxilia no direcionamento do diagnóstico. O objetivo deste trabalho foi descrever aspectos epidemiológicos, clínicos e laboratoriais de uma série de casos de hiperadrenocorticismo (HAC) canino. Foram avaliados 115 prontuários de cães diagnosticados pelo teste de supressão por baixa dose de dexametasona e/ou teste de estimulação com ACTH. Os casos de HAC ACTH-dependentes representaram 81,3% da população, e 18,7% foram ACTH-independentes. As fêmeas foram mais acometidas, representando 69,3% dos casos. A média de idade foi 10,3 ± 2,5 anos e 64,9% eram gonadectomizados. A maioria dos cães foi de porte pequeno, de até 10 kg (73,9%). As raças mais frequentes foram Poodle (27%), Dachshund (17,4%) e Yorkshire (10,4%). As manifestações clínicas mais relatadas foram polifagia (86%), polidipsia (82,6%), poliúria (80%), abdome pendular (82,6%), atrofia cutânea (79,1%), fraqueza muscular (78,3%) e dispneia (74,8%). Entretanto, eventualmente sinais clínicos pouco associados ao HAC se manifestaram de forma mais importante que os sinais clássicos da doença. O hemograma revelou neutrofilia (66%), eosinopenia (58,3%) e linfopenia (42,6%) como principais alterações hematológicas. Na bioquímica sérica foi observado aumento de fosfatase alcalina (81,74% dos casos), aumento da atividade da ALT (62,6%), hipercolesterolemia (66%) e hipertrigliceridemia (54,7%). A urinálise revelou hipostenúria em 14,9% e isostenúria em 13,5%; além de proteinúria em 50% dos casos. A ecografia abdominal evidenciou hiperplasia bilateral de adrenal (92,2%) com assimetria de adrenais em 20,8% dos casos, além de hepatomegalia (80,9%), lama biliar (67,8%) e hiperecogenicidade hepática (47,8%). Concluiu-se que fêmeas castradas de pequeno porte, principalmente das raças Poodles e Dachshunds, apresentaram maior frequência na população estudada e que as principais alterações observadas clínicas e nos exames complementares foram polifagia, poliúria, polidipsia, aumento abdominal, hiperplasia da adrenal, aumento de fosfatase alcalina e hiperlipidemia. Estes resultados corroboram para melhor caracterização da doença no Brasil. Este estudo concluiu que a população estudada se assemelha ao perfil populacional de cães com HAC descrito em estudos Europeus e Norte Americanos de forma que o perfil dos casos ao redor do globo parece similar.

ABSTRACT: Diseases’ clinical-epidemiological characterization assists in directing the diagnosis. The objective of this study was to describe epidemiological, clinical and laboratorial aspects of a case series of canine hyperadrenocorticism (HAC). One-hundred fifteen records of dogs diagnosed by the low dose dexamethasone suppression test and/or ACTH stimulation test were evaluated. Of the cases, 81.3% were HAC ACTH-dependent and 18.7% HAC ACTH-independent. Females were more affected, representing 69.3% of the cases. The mean age was 10.3±2.5 years and 64.9% were gonadectomized. Most of the patients were small size dogs, weighting less than 10kg (73.9%). The most frequent breeds were: Poodle (27%), Dachshund (17.4%), and Yorkshire Terrier (10.4%). The most frequent clinical manifestations were polyphagia (86%), polydipsia (82.6%), polyuria (80%), abdominal enlargement (82.6%), thin skin (79.1%), muscular weakness (78.3%) and panting (74.8%). However, eventually unusual HAC-associated signs would be present in some dogs in a more important way compared with the classic disease´s clinical signs. The CBC showed neutrophilia (66%), eosinopenia (58.3%) and lymphopenia (42.6%) as main hematological abnormalities. The most common findings in serum biochemistry were increased alkaline phosphatase activity (81.74%), increased ALT activity (62.6%), hypercholesterolemia (66%) and hypertriglyceridemia (54.7%). Urinalysis revealed hyposthenuria in 14.9% and isostenuria in 13.5%; besides proteinuria in 50% of the cases. Abdominal ultrasound showed bilateral adrenal hyperplasia (92.2%) with adrenal asymmetry in 20.8% of the cases, in addition to hepatomegaly (80.9%), biliary sludge (67.8%) and hepatic hyperechogenicity (47.8%). It was concluded that small size gonadectomized female dogs, mainly Poodles and Dachshunds, presented higher frequency in the population studied, and that the main changes observed in clinical and complementary tests were polyphagia, polyuria, polydipsia, abdominal enlargement, adrenal hyperplasia, increased phosphatase alkaline and hyperlipidemia. These results corroborated to a better disease characterization at Brazil. This work concluded that the population studied resembles the profile describe in European and North American epidemiologic studies, and that the HAC dog´s clinical picture looks similar worldwide.
